氯硝安定肌内注射和灌肠治疗小儿惊厥的疗效观察

摘要：
目的：观察氯硝安定肌内注射、灌肠治疗小儿惊厥的临床疗效和安全性。
方法：90例惊厥患儿随机分为氯硝安定肌注组、氯硝安定灌肠组和对照组各30例。氯硝安定肌注组予以氯硝安定0.05 mg/kg肌内注射；氯硝安定灌肠组予以氯硝安定0.1 mg/kg灌肠治疗；对照组予以鲁米那5 mg/kg肌内注射。观察治疗效果和不良反应。
结果：氯硝安定肌注组总有效率90.0%(27/30),氯硝安定灌肠组总有效率90.0%(27/30)；对照组总有效率63.3%(19/30)。氯硝安定肌注组、氯硝安定灌肠组与对照组比较差异有统计学意义(P＜0.017)。氯硝安定肌注组与氯硝安定灌肠组间比较差异无统计学意义(P＞0.017)。氯硝安定肌注组与氯硝安定灌肠组均未见复发,对照组有8例复发。3组用药后均未发生呼吸抑制、循环障碍。
结论：氯硝安定肌注、灌肠治疗小儿惊厥疗效确切,安全便捷无副反应,值得基层医院推广应用。

Abstract:
Objective：To observe the clinical effect and safety of clonazepam for intramuscular injection and enema in the treatment of infantile convulsion.
Methods：Ninety children with convulsion were randomly divided into 3 groups：group of clonazepam for intramuscular injection(A)，group of clonazepam for enema (B) and control group(C)，with 30 cases in each.Group A was given clonazepam at 0.05 mg/kg for intramuscular injection；group B was given clonazepam at 0.1 mg/kg for enema；control group was given luminal at 5 mg/kg for intramuscular injection.Observe the treatment results and adverse reactions.
Results： Total effective rate was 90.0%(27/30) in both group A and group B，which was 63.3%(19/30) in group C，the difference being of statistical significance(P＜0.017).There was no significant difference between group A and group B(P＞0.017).There was no relapse in either group A or B，while there were 8 cases of relapse in group C.There was no respiratory depression or circulation disorder in all the 3 groups after treatment.
Conclusion：Clonazepam for intramuscular injection and enema is effective in the treatment of infantile convulsion.It is safe and easy to perform with no adverse reaction，which is worthy of being applied in the local hospitals.
