儿童大叶性肺炎的舌下络脉规律研究

Regularity of sublingual collaterals in children with lobar pneumonia

摘要:
目的
观察儿童大叶性肺炎疾病过程中舌下络脉的变化规律及与舌质、舌苔的关系。
方法
2018年3月至2019年3月黑龙江中医药大学附属第一医院儿科收治大叶性肺炎患儿206例为研究对象,为大叶性肺炎组。同期就诊的100例支气管肺炎患儿为支气管肺炎对照组,同期选择健康体检的儿童100例为正常对照组。观察比较3组儿童的舌下络脉异常分布情况,同时观察大叶性肺炎患儿舌质、舌苔,分析不同舌象与舌下络脉的关系。
结果
大叶性肺炎组舌下络脉异常率为617%(127/206),显著高于支气管肺炎对照组与正常对照组,差异有统计学意义(P<005);大叶性肺炎患儿异常舌下络脉与红紫舌、厚腻苔关系密切。
结论
舌下脉络对于儿童大叶性肺炎病程中血瘀征象具有较高的诊断意义,是舌苔、舌质的必要补充,可以更好的指导临床诊疗。

Abstract:
Objective
To observe the changes of sublingual collaterals in children with lobar pneumonia and their correlation with tongue texture and tongue coating.
Methods
From March 2018 to March 2019, 206 children with lobar pneumonia and 100 children with bronchial pneumonia were treated in the Department of Pediatrics, the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine, who were included in the study as lobar pneumonia group and bronchial pneumonia group. A total of 100 health children were chose as the normal control group. The abnormal distribution of sublingual collaterals was compared and analyzed among the three groups. Meanwhile, the tongue texture and tongue coating of children with lobar pneumonia were observed to analyze the correlation.
Results
The abnormality rate of sublingual collaterals was 61.7% in lobar pneumonia group, which was significantly higher than that in bronchopneumonia group and normal control group, and the difference was statistically significant(P<0.05). The abnormal sublingual collaterals in lobar pneumonia children were shown as red purple tongue and thick and greasy tongue coating.
Conclusion
The sublingual collaterals play a significant role in the diagnosis of the blood stasis sign in children with lobar pneumonia, and is a necessary supplement to tongue texture and tongue coating, which can further guide clinical practice.
