彩超实时监测水压灌肠治疗小儿急性肠套叠的应用

Application of real-time monitoring of water pressure enema by color ultrasonography in children with acute intussusceptions

摘要:
目的
探讨彩超实时监测小儿急性肠套叠温生理盐水灌肠复位的应用价值。
方法
选择2012年1月至2016年1月沈阳市儿童医院收治的小儿急性肠套叠患儿616例为研究对象,均采用彩超实时监测温生理盐水灌肠复位。包块较大者可用低频探头放大实时监视,小肠套叠可用高频探头实时监视。
结果
小儿急性肠套叠复位成功609例,复位成功率为98.8%。7例水压灌肠复位不成功,均为回盲部包块,后经手术证实,4例肠坏死,1例为囊肿型肠重复畸形,1例为肠壁淋巴瘤,1例为肠息肉。
结论
低频、高频彩色超声联合应用实时监测小儿急性肠套叠水压灌肠复位无辐射、患儿相对舒适、成功率高,是治疗小儿急性肠套叠的较好方法。

Abstract:
Objective
To study the application value of real-time monitoring of water pressure enema by color ultrasonography in the treatment of children with acute intussusception.
Methods
Totally 616 children with acute intussusception treated in Shenyang Children's Hospital from Jan. 2012 to Jan. 2016 were selected as the research subjects. All of them were reducted by warm saline enema under real-time monitoring of color ultrasonography. For those with larger masses, low-frequency probe was used to magnify the real-time monitoring, and high-frequency probe was used for the small intussusceptions.
Results
There were 609 cases of uccessfully reduction in the children with acute intussusception, the success rate being 98.8%.The reduction by water pressure enema was not successful in 7 cases, which were masses in ileocecum. Later it was confirmed by surgery that there were 4 cases of intestinal necrosis, one case of cystic intestinal duplication, one case of lymphoma of the intestinal wall, and one case of intestinal polyp.
Conclusion
The real-time monitoring of water pressure enema by low- and high-frequency color ultrasonography for reduction has no radiation in the treatment of children with intussusceptions. The children feel relatively comfortable and the success rate is high, so it is a better way to manage acute intussusceptions in children.
