空心螺钉与小儿髋部锁定加压接骨板治疗儿童移位型股骨颈骨折疗效观察

Comparison of the effect of hollow screw and pedicle hip locking compression plate in treatment of displaced femoral neck fracture in children

摘要:
目的
比较空心螺钉与小儿髋部锁定加压接骨板治疗儿童移位型股骨颈骨折的疗效。
方法
选择2014年9月至2016年10月湖南省儿童医院收治的移位型股骨颈骨折患儿24例作为研究对象,随机分为观察组和对照组各12例。观察组患儿采用空心螺钉内固定,对照组患儿采用小儿髋部锁定加压接骨板内固定,比较两组患儿的骨折复位质量、Ratliff评定结果、髋关节Harris评分、术中出血量、手术时间和骨折愈合时间。
结果
内固定术后两组患儿骨折复位质量优良率、Ratliff评定结果和髋关节Harris评分比较,差异无统计学意义(P>0.05)。观察组患儿平均术中出血量、手术时间、骨折愈合时间显著少于对照组,差异均有统计学意义(P<0.05)。
结论
空心螺钉与小儿髋部锁定加压接骨板治疗儿童移位型股骨颈骨折疗效相当,但空心螺钉在出血量、手术时间、愈合时间等方面有一定优势。

Abstract:
Objective
To compare the effect of hollow screw with that of pedicle hip locking compression plate in the treatment of displaced femoral neck fracture in children.
Methods
Twenty-four children with displaced femoral neck fracture treated in our hospital from September 2014 to October 2016 were selected as study group and they were divided randomly into observation group and control group, 12 cases in each group. Hollow screw internal fixation was performed in the observation group, and hip locking plate was used for internal fixation in control group. The results of fracture reduction, Ratliff evaluation, Harris score of hip joint, blood loss during operation, operation time and healing time were compared between the two groups.
Results
There was no significant difference between the two groups in the quality of fracture reduction, Ratliff evaluation or hip Harris score(P>0.05). The mean blood loss during operation was less in the observation group than that in the control group; the time of operation and the time of fracture healing were shorter than the control group, the difference being statistically significant(P<0.05).
Conclusion
Hollow screw and pediatric hip locking compression plate have similar effect in the treatment of displaced femoral neck fracture in children. However, the hollow screw has certain advantages in terms of blood loss, operation time and healing time.
