    Expert consensus in integrated traditional Chinese and western medicine prevention and treatment for influenza in children

    Children are the highrisk population of influenza(flu) and its severe cases. The morbidity is high in the flu season, and in severe cases it may result in death. Currently, the effective western medicines for flu viruses are limited, and the treatment with western medicines alone has its limitations. In the theory of traditional Chinese medicine, flu belongs to seasonal cold, which is highly infectious, and it also belongs to "plague" in traditional Chinese medicine has had valuable experience in the prevention and treatment of "plague", but it also has its limitations. Therefore, the integration of Chinese with western medicine which takes advantage of both traditional Chinese and western medicine, will have its unique advantages, especially in the early intervention of mild cases. Based on the review on the advances in modern prevention and treatment of flu in children, the analysis of literatures on Chinese medicine treatment for child flu, the drug assessment questionnaire and the application experience of the experts, the consensus aims to standardize the combined use of Chinese and western medicines, in order to find solutions to the clinical problem of the prevention and treatment for influenza in children, especially for mild cases.

    Influenza virus infection is one of the most prevalent infectious diseases in the world.Influenza virus infection can induce bronchitis and pneumonia,but severe lethal pneumonia is observed when complications involve bacterial infections.Animal models play an important role in the study of co-infection with influenza virus and Streptococcus pneumoniae.Different animal models have different advantages and limitations.This article introduces commonly used animal species and determination methods of pathogens,and synthesize the choice of modeling time points for different research purposes and the determination methods of models.The current research progress of co-infection models at home and abroad is summarized,with a view to provide reference for basic and applied research on co-infection.

    Objective: Traditional Chinese medicine has obvious advantages in treating mesenteric lymphadenitis,and the external treatment is especially effective.This article summarizes the reports on TCM external therapy for mesenteric lymphadenitis in the past five years.It is found that acupuncture,massage,acupoint application and umbilical therapy,enema with traditional Chinese medicine,percutaneous treatment with traditional Chinese medicine on acupoints,ultrasonic induction of traditional Chinese medicine,microwave physiotherapy and other methods are more effective than conventional treatment with pure western medicine.This provides reference and methods for clinical treatment and broaden the thoughts on clinical treatment methods to a certain extent hoping to further promote the external treatment of TCM in clinical practice.

    Coronary artery thrombosis (CAT) is a serious complication of Kawasaki disease (KD), which hasbecome one of the important causes of children's acquired heart disease. The pathogenesis of KD-CAT is still being explored, and itstreatment plan and medication has become a research hotspot. This paper reviews the application of KD-CAT from the aspects of traditional Chinese medicine and western medicine. In western medicine the drugs forKD-CAT are divided into antiplatelet, anticoagulant and thrombolytic drugs. In traditional Chinese medicine the drugs for clearing heat and detoxification, supplementing Qi and nourishing Yin, and promoting blood circulationand removing blood stasis are used as the main line of treatment, and it is emphasized that the prescription of "promoting blood circulation and removing blood stasis" must be used throughout the whole process of KD-CAT treatment.

    Objective:To investigate the active components and potential molecular mechanism of Pinellia ternata-Mangnolia officinalis pair in the treatment of epilepsy, and to provide a theoretical basis for clinical treatment of epilepsy. 
    Methods:The active components and their targets of P. ternata and M. officinalis were screened via the traditional Chinese medicine systems pharmacology platform(TCMSP) and therapeutic targets database. The protein interaction network was constructed with the STRING database to screen for core targets of P. ternata and M. officinalis in the treatment of epilepsy. The interactive active network map of "active component-keytarget-pathway" was constructed with Cytoscape software. Biological process and KEGG signal pathway enrichment analysis was performed by DAVID database. Finally, docking of the representative -active components with the core target of epilepsy was carried out using MOE software.
    Results:Two hundred and fifty-nine components of P. ternata and M. officinalis were obtained. Data analysis showed that ten active components of P. ternataM. officinalis pair were identified, which acted on fifty-five potential targets as well as ten pathways related to epilepsy. Herb-component-target-pathway network analysis showed that baicalein, baicalin and coniferin, etc. were the main active components. These active components interacted with multiple targets and affected various biological pathways to reduce the abnormal electrical discharge of neurons and decrease the frequency of seizure. The targets involved in the treatment of epilepsy included PPARA, PPARD and PPARG, and the pathways included PPAR signaling, steroid hormone biosynthesis and arachidonic acid metabolism. Molecular docking results showed that PPAR, a representative target for the treatment of epilepsy, had good binding activity with these ten potential active components.
    Conclusion:The active components of P. ternata-M. officinalis pair regulate multiple biological pathways by regulating and interacting with multiple target proteins related to the occurrence and development of epilepsy, showing the characteristics of being multi-component, multi-target and multiple biological pathways. The present study provides a research basis for the exploration of the active components P. ternata-M. officinalis pair, the study of mechanism of action and clinical rational use of this herb pair in future.

    Tic disorder(TD) is acommon sudden, rapid, recurrent neuropsychiatric disorder dominated by rhythmic movement or vocalization in children or adolescents. The exact etiology and pathogenesis are not clear. A small number ofchildren can get relieved by themselves at puberty, but the disease may be refractory in some children. This paper focuses on theresearch frontier of tic disorder at home and abroad. By searching Pubmed, Cochrane Library, CBM, CNKI and Wanfang databases, this paper systematically discusses the diagnosis, treatment and prognosis of ticdisorder from the aspects of epidemiology, comorbidity, pathogenesis and diagnosis and treatment methods by traditional Chinese and Western medicine, in order to provide new treatment ideas and theoretical basis for clinical practice.

    Construction of Atg7-gene-knockout human brain microvascular endothelial cell line based on CRISPR/cas9 technology

    Objective:To construct Atg7-gene-knockout human brain microvascular endothelial cells(HBMEC) by CRISPR/cas9 technology, so as to provide an in vitro cell model for the role of Atg7 in the development of cerebral vessels, the mechanism of Atg7 participating in the formation of blood-brain barrier and thestudy of cerebrovascular diseases.
    Methods:Two pairs of guiding RNA sequences(sgRNA) were designed according to exon 1 of human Atg7. HBMEC was transfected with lentivirus; puromycin and Pyricularia oryzae were used to screen for cells successfully transfected. RT-qPCR and Western blotting were used to detect the mRNA and protein of the target gene, and the morphological changes were observed under the microscope.
    Results:The CRISPR/cas9 plasmid targeting Atg7 was constructed. The results of RT-qPCR, Western blot and immunofluorescence showed that stable Atg7-knockout human brain microvascular endothelial cell line was  obtained by monoclonal screening. No obvious morphological changes were observed under light microscope.
    Conclusion:The successful construction of human brain microvascular endothelial cells with Atg7 gene knockout by CRISPR/cas9 technology is beneficial to the development of cerebral vessels, the mechanism of blood-brain barrier morphogenesis and the study of cerebrovascular diseases.

    Clinical study of modified Xiaoqinglong decoction in the treatment of children with bronchial asthma of external-coldness and internal-drink type during acute onset

    Objective: To study the clinical effect of modified Xiaoqinglong decoction in the treatment of children with bronchial asthma ofexternal-coldness and internal-drink type.
    Methods: A total of 80 children with bronchial asthma of external-coldness andinternal-drink type were treated in our hospital between February 2019 and August 2020, and they were divided into observation group and control group, with 40 cases in each group. Both groups were treated with conventional western medicine, and the observation group was also treated given modified Xiaoqinglong decoction, and the use of hormone was reduced by a half. After one week of treatment, the clinical effect wascompared between the two groups, and compare the total traditional Chinese syndrome score, FEV1%, FEV1/FVC, PEF andFeNO before and treatment, the childhood asthma control test and the adverse reactions were evaluated.
    Results: After treatment, the total effective rates of the observation group and the control groupwere 92.3% and 71.0%, respectively, with statistically significant differences between the twogroups(P<0.05). After one week of treatment, the total score of TCM syndromes in the observation group was significantly lower than that of the control group(P<0.01), lung function was significantly better(P<0.05), FeNO was significantly lower than that of the control group, and the control test of asthmain the observation group was better than that in the control group(P<0.01). No adverse reactions occurred in both groups during the treatment.
    Conclusion: On the basis of conventional western medicine treatment, the dosage of hormone is halved, and Xiaoqinglong decoction can be used to treat children with  bronchial asthma of external-cold and internal-drink type during acute onset, which is effective and worthy of further clinical promotion.

    Objective: To observe the effect of Yangyin Qingfei mixture on cough (yin-deficiency and lung-heat syndrome) after Mycoplasma pneumoniae infection.
    Methods: From January 2018 to June 2020,120 children with cough after mycoplasma infection (yin-deficiency and lung-heat syndrome) in Pediatric Outpatient Department and ward of Affiliated Hospital of Liaoning Universityof Traditional Chinese Medicine were randomly divided into two groups,60 cases in the treatment group and 60 cases in the control group.The control group was treated with western medicine.The treatment group was given YangyinQingfei mixture on the basis of western medicine.Both groups were treated for 7 days continuously,and the effect was observed.
    Results: After treatment,the scores in cough,dry stool,feverishness in palms and soles and night sweating and the total scores were significantly lower in observation group than in control group(P<0.05).After treatment,the total effectiveness in observation group was better than that in control group(P<0.05).After treatment,the effective rate in observation group was 96.67% (58/60),which was higher than in the control group (81.67%,49/60)(P<0.05).
    Conclusion: Yangyin Qingfei mixture has a significant effect on cough (yin-deficiency and lung-heat syndrome) after mycoplasma infection in children.

    Effects of Li-Pi Compound Preparation and zinc sulfate on proliferation of colonic smooth muscle cells of rats and the brain-gut peptide

    Objective:To observe the effect of 10% Li-Pi Compound Preparation and zinc sulfate drug-containing serum on the proliferation of colonic smooth muscle cells(CSMCs) and their regulation effect on braingut peptide, and to explore the mechanism of the two drugs in the treatment of anorexia.

    Methods:Among the 60 young rats, the CSMCs of 10 rats were cultured, and the remaining 50 rats were randomly divided into five groups: the control group, the low,medium and high-dose Li-Pi Compound Preparation group and the zinc sulfate group, with 10 rats in each group. The control group was given equal volume of drinking water by gavage. The zinc sulfate group was given 20 mg/kg zinc sulfate oral solution by gavage. The low,medium and high-dose groups were given by gavage 2.4, 4.8 and 9.8 g/kg Li-Pi CompoundPreparations respectively. Each group was given gavage once a day for 7 consecutive days, and blood samples were collected to prepare drug-containing serum at 2h after the last gavage.The drug-containing serum of each group was diluted to 10% and was given to rat CSMCs at 9 time points. The effect of drug-containing serum in each group on the proliferation of colonic smooth muscle cells was detected by CCK-8;Western Bolt, ELISA and RT-PCR were used to detect the relative expression levels of LP,NPY and CCK-8 protein concentration in the cells at the optimal starting point of CSMCs proliferation.

    Results:The 10% differentdosedrug-containing serum of Li-Pi Compound Preparations and zinc sulfate could promote the proliferation of CSMCs in rats, and the optimal starting time of proliferation was 24h.Compared with the control group, each drug-containing serum group could increase the concentration of LP and NPY protein in rat CSMCs and the relative expression level of protein and gene(P<0.05),reduce the concentration and the relative expressionlevel of CCK-8 protein(P<0.05).
    Conclusion:Li-Pi Compound Preparation and zinc sulfate can promote the proliferation of CSMCs,and affect the expression of LP,NPY and CCK-8 in CSMCs, which may be one of the important mechanisms in the treatment of anorexia.

    Analysis of the effect of psychomotor therapy combined with behavioral therapy on children with attention-deficit-hyperactivity-disorder aged 4 to 6

    Objective:To explore the therapeutic effect of psychomotor therapy combined with behavioral therapy on children with attention-deficit hyperactivity disorder(ADHD) aged 4 to 6.
    Methods:Forty children aged 4 to 6 with ADHD were divided into an observation group(20 cases) and a control group(20 cases) according to a random number table. They were diagnosed and received therapy in Affiliated Hospital of Jiangsu University from January to December 2019. The observation group received 24 weeks of psychomotor therapy combined with behavioral therapy and the control group only received 24 weeks of behavioral therapy. The Conners Parent Symptom Questionnaire(PSQ) was used to evaluate the observation group and the control group before treatment, during treatment (12th week) and after treatment.
    Results:Compared with before treatment, the observation group has different PSQ during and after treatment. The scores of each factor were significantly reduced, with statistical differences(P<0.01). There were statistical differences in the behavior problems(P<0.01) and psychosomatic disorders(P<0.05) of the control group during the treatment, but there were no differences in other factors. The PSQ factor scores of the control group were significantly reduced after treatment, with statistical differences(P<0.01). During and aftertreatment, the scores of PSQ factors of the observation group were all lower than those of the control group, the differences being statistically significant (P<0.01).
    Conclusion:Psychomotor therapy combined with behavioral therapy can effectively solve some problems in children with ADHD. It is effective at 12 weeks of treatment, and the effect is significant after long-term treatment.

    Analysis of the curative effect of modified Sangbaipi decoction on pertussis-like syndrome in children

    Objective: To explore the effect of modified Sangbaipi decoction on children with pertussis-like syndrome.
    Methods: Forty cases of children withpertussis-like syndrome treated in our hospital from January 2017 to December 2019 were randomly divided into control group(20 cases, treated with azithromycin) and observation group(20 cases, treated with Sangbaipi decoction and azithrpmycin). The total effective rate, inflammatory factor level, pulmonary function index, disappearance time of main symptoms, recovery time of blood routine and hospitalization time were compared between the two groups.
    Results: The total effective rate of the observation group(95.0%,19/20) was higher than that of the control group(65.0%,13/20)(P<0.05);the respiratory rate of the observation group after treatment was lower than that of the control group, and the tidal volume and inspiration/expiration ratio after treatment were higher than those of the control group(P<0.05); the white blood cell count and C-reactive protein level in the observation group werelower than those in the control group(P<0.05); the disappearance time of main symptoms and the recovery time of blood routine in the observation group were all lower than those in the control group(P<0.05). Compared with thecontrol group, the hospitalization time was shorter(P<0.05).
    Conclusion: The application of modified Sangbaipi decoction in the treatment of children with pertussis-like syndrome is helpful to enhance the curative effect, improve the lung function, reduce the inflammatory reaction and accelerate the recovery of the disease.

    Advances in research on the treatment for upper airway cough syndrome based on TCM syndrome differentiation

    Upper airway cough syndrome(UACS) used to be described as "postnasal drip syndrome", which is the main cause of chronic wet cough in children at 1 year old and above. The treatment based on syndrome differentiation for this disease bytraditional Chinese medicine has the characteristics of quick efficacy, few side effects, low recurrence rate andenhanced immunity and has been widely recognized. It is summarized that the cause of the disease is wind,phlegm, deficiency and stasis, and the core pathogenesis is "the deficiency of lung and spleen qi, retention of wind and phlegm, binding of phlegm and stasis, and discomfort in lung, noseand throat". It is suggested that the diagnosis by western medicine should be combined with TCM syndrome differentiation and constitution differentiation, and the treatment should be to disperse wind and relieve the lung and fever, strengthen the spleen and reduce phlegm and stasis, which is the main treatment principle, and the symptomatic treatment, such as clearing fever, nourishing yin, moistening dryness and promoting digestion, can also be used. Meanwhile, pay attention to some TCM exterior therapies, including external application of Chinese medicines, massage for children and indirect moxibustion.

    Huaiqihuang granules is a Chinese patent medicine commonly used in modern pediatric clinic, which is composed of bacterial substance of Sophora japonica, wolfberry fruit and polygonatum. It has the effect of comprehensiveregulation and nourishment on the human body, and is widely used in pediatric clinic. This paper reviewsthe research progress in the clinical application of Huaiqihuang granules in pediatric department in recent years, which shows that there have been more and more researches in Huaiqihuang granules, but there is a lack of large-sample multicenter research and real-world research, and there is less research in its application safety, so the treatmenteffect of Huaiqihuang granules on different types of various diseases needs to be further and carefully studied. With the deepening of experimental research, especially the application of cytobiological and molecular biological technology, the functional mechanism and clinical application of Huaiqihuang granules should be further investigated.

    Observation on the effect of Weijing Ditan decoction in the treatment of children with protracted bacterial bronchitis

    Objective:To observe the therapeutic effect of self-made Weijing Ditan decoction on children with protracted bacterial bronchitis.
    Methods:From March 2017 to March 2018,82 children with protracted bacterial bronchitis were treated in the First Affiliated Hospital of Heilongjiang TCM University, and they were randomly divided into experimental group(42 cases) and control group(40 cases). The experimental group was treated withWeijing Ditan decoction,while the control group was treated with ambroxol hydrochloride oral solution combined with amoxicillin and clavulanate potassium, for 3 weeks. The effective indexes were cough symptom score, TCM syndrome score and lung CT abnormality score.
    Results:After 3 weeks of treatment,the total effective rate of the experimental group was 88.1%(37/42), which was significantly higher than that of the control group (70.0%,28/40) (P<0.05).After treatment, the cough symptom score, the primary andsecondary symptom scores of Tanzhuoyunfei and Feishixuanjiang syndrome of the two groups were significantly lower than thosebefore treatment (P<0.05), and the reduction effect of the experimental group was significantly better than that of the control group,the differences being statistically significant(P<0.05). After treatment, among the 17 cases of obstructive ventilation disorder,16 cases turned to normal after 3 weeks of treatment,and 1 case turned to moderate after 2 weeks of treatment.After treatment, the CT severity scores of 17 cases in the experimental group and 14 cases in the control group were significantly reduced compared with those before treatment,and the influenceof CT in experimental group was more significantly reduced(P<0.05).
    Conclusion:Weijing Ditan decoction in the treatment of children with protracted bacterial bronchitis can significantly improvecough symptoms and TCM syndromes, and significantly improve pulmonary ventilation function and HRCT performance.

    Objective: To observe the effect of stability training of core muscle group combined with crawling training on motor and intellectual development of children with spastic cerebral palsy.
    Methods: Totally 95 children with spastic cerebral palsy treated in our hospital from February 2018 to October 2019 were selected and randomly divided into control group (47 cases) and observation group (48 cases). The control group was given routine crawling training, and the observation group was given stability training of core muscle group in addition to crawling training. The crawling ability and intellectual development of the two groups were observed after 3 months.
    Results: After the intervention, the modified Tardieu spasm scale score of the observation group was significantly lower than that of the control group, and the gross motor function scale score of the observation group was significantly higher than that of the control group(P<0.05); after the intervention, the development index score and psychomotor development index score of the observation group were significantly higher than those of the control group (P<0.05).
    Conclusion: Stability training of core muscle group combined with crawling training can improve the crawling ability of children with spastic cerebral palsy, activate and coordinate the movement of upper and lower limbs, enhance the ability of innervation, and promote their intellectual development.
