大叶性肺炎患儿外周血细胞间黏附分子1和肺泡灌洗液RANTES检测及其意义

Detection of ICAM-1 in peripheral blood and RANTES in bronchoalveolar lavage fluid in children with lobar pneumonia and its significance

摘要:
目的
探讨大叶性肺炎患儿外周血淋巴细胞细胞间黏附分子1(ICAM-1)和肺泡灌洗液调节激活正常T细胞表达和分泌的细胞因子(RANTES)检测的意义及两者在大叶性肺炎发病机制中的相关性。
方法
选择2015年7月至2016年12月郑州儿童医院呼吸科收治住院的呼吸系统疾病患儿95例为研究对象。按临床诊断分为大叶性肺炎组35例,支气管肺炎组30例和气管狭窄组(对照组)30例。采用流式细胞术检测患儿外周血淋巴细胞ICAM-1表达情况,ELISA法测定其肺泡灌洗液RANTES水平。
结果
(1)大叶性肺炎组ICAM1表达率显著高于支气管肺炎组及对照组,支气管肺炎组ICAM-1表达率显著高于对照组,差异有统计学意义(P<0.05)。(2)大叶性肺炎组和支气管肺炎组肺泡灌洗液RANTES水平均显著高于对照组,差异有统计学意义(P<0.05),大叶性肺炎组与支气管肺炎组间RANTES水平比较,差异无统计学意义(P>0.05)。(3)大叶性肺炎组ICAM-1与RANTES呈正相关(r=0.781,P<0.01),支气管肺炎组和对照组ICAM-1与RANTES无相关性。
结论
ICAM-1和RANTES参与了大叶性肺炎的发病过程且具有协同作用,并可作为大叶性肺炎的检测指标,为研究大叶性肺炎的发病机制提供了理论依据。

Abstract:
Objective
To explore the significances of detection of intercellular adhesion molecule-1(ICAM-1) in peripheral blood and regulated upon activation normal T cell expressed and secreted(RANTES) in bronchoalveolar lavage fluid in children with lobar pneumonia and their correlation in pathogenesis of lobar pneumonia.
Methods
A total of 95 children with respiratory diseases were included  as the research subjects, and they were divided into 3 groups: lobar pneumonia group(35 cases),bronchopneumonia group(30 cases) and tracheal stenosis group(control group,30 cases). The expression of ICAM-1 was detected by flow cytometry in lymphocyte of peripheral blood in 35 children with lobar pneumonia and 30 children with bronchopneumonia and 30 children with tracheal stenosis, and the level of RANTES was assayed with ELISA technique in the bronchoalveolar lavage fluid.
Results
(1)The expression rate of ICAM-1 in lobar pneumonia group was much higher than that in bronchopneumonia group(P<0.05) and control group(P<0.01),and it was higher in bronchopneumonia group than in control group(P<0.05).(2)The level of RANTES in lobar pneumonia group and bronchopneumonia group was higher than that in control group(P<0.05);however,no significant difference was found between lobar pneumonia group and bronchopneumonia group.(3)There was a positive correlation between ICAM1 and RANTES in lobar pneumonia group(r=0.781,P<0.01); however,no correlation was found in bronchopneumonia group and control group.
Conclusion
ICAM-1 and RANTES are involved in the development of lobar pneumonia and play a synergistic role in it, so they can be used as indicators of lobar pneumonia, which provides theoretical basis for studying the pathogenesis of lobar pneumonia.
