儿童反复呼吸道感染外周血Th17细胞、唾液中分泌型免疫球蛋白A/白蛋白比值变化及其意义

Changes of Th17 cells in peripheral blood and the ratio of sIgA/ALB in saliva in children with recurrent respiratory tract infection and their significance

摘要:
目的:探讨儿童反复呼吸道感染(RRTIs)外周血Th17细胞、唾液中分泌型免疫球蛋白A(sIgA)与血清白蛋白(ALB)比值变化及其意义。
方法:2014年10月至2015年7月广州中医药大学附属中山市中医院儿科收治RTTIs患儿100例,其中急性期和缓解期各50例。同期选取门诊体检的健康儿童50例为正常对照组。流式细胞仪检测Th17细胞表达率,酶标仪检测唾液中sIgA,免疫比浊法检测ALB。
结果:RRTIs患儿急性期组、缓解期组外周血Th17细胞数较正常对照组患儿增高,差异有统计学意义(P<0.05);RRTIs患儿急性期组外周血Th17细胞数高于缓解期组,差异有统计学意义(P<0.05)。外周血中Th17阳性细胞数与呼吸道感染严重程度呈正相关(P<0.05)。正常对照组唾液中sIgA与血清白蛋白比值显著高于RRTIs患儿急性期组、缓解期组,差异有统计学意义(P<0.05);RRTIs患儿急性期组与缓解期组唾液中sIgA与血清白蛋白比值比较差异无统计学意义(P>0.05)。
结论:RRTIs患儿外周血Th17细胞表达增加,且与其病情的严重程度呈正相关。RRTIs患儿存在黏膜免疫功能低下,增强呼吸道免疫功能能从根本上减少RRTIs的发作。

Abstract:
Objective:To observe and explore the significance and changes of Th17 cells in peripheral blood and ratio of sIgA/ALB in human saliva.
Methods:A total of 100 RRTIs children were divided into two groups: acute group(n=50) and remission group(n=50).Another 50 normal healthy children were included as the control group. The expression rate of Th17 cells was detected by flow cytometry, the expression rate of sIgA in saliva was detected by enzymatic-reader,and the ALB was detected by immune turbidity method.
Results:The circulating frequencies of Th17 cells in acute and remission group were higher than those in healthy control group(P<0.05), while those in acute group were higher than those in remission group(P<0.05).There were significantly difference in these groups(P<0.05). The circulating frequencies of Th17 cells were positively correlated with the severity of recurrent respiratory tract infection(P<0.05). The ratio of sIgA and serum albumin in saliva in healthy control group was significantly higher than that of acute group and remission group in RRTIs children(P<0.05),but that was no significant difference between acute and remission group(P>0.05).
Conclusion:The circulating frequencies of Th17 cells in RRTIs children are increased, and are positively correlated with the severity of the RRTIs. The function of mucosal immune function in RRTIs children is low, so enhancing the immune function of the respiratory tract can fundamentally reduce the attack of RRTIs.
