手足口病合并急性下肢弛缓性麻痹神经传导分析

Analysis of nerve conduction in hand-foot-mouth disease combined with acute flaccid paralysis

摘要:
目的: 总结手足口病(HFMD)患儿合并急性下肢弛缓性麻痹(AFP)的临床及神经传导检查结果,分析其用于临床诊断、治疗及预后判断的作用。
方法: 2012-06/2014-06在上海交通大学医学院附属新华医院神经内科定期随访5例合并AFP的HFMD患儿,回顾性分析其临床特征及神经传导结果。
结果: 本组神经损害表现提示损害以轴索或前角细胞为主。起病早期运动传导动作电位波幅下降严重程度与肢体活动障碍程度呈正相关;运动传导动作电位波幅的恢复好转,在第1次复检时最明显,股神经最易受累,神经损害表现为运动传导动作电位波幅下降。起病早期运动传导动作电位波幅降低至正常1/3及以下者,在6~12个月后仍留有不同程度的肢体活动障碍。
结论: 手足口病合并急性下肢弛缓性麻痹患儿应尽早发现,及时积极治疗,以期降低致残率或减轻残疾程度。

Abstract:
Objective:To analyze the clinical features and the examination results of nerve conduction in hand-foot-mouth disease(HFMD) combined with acute flaccid paralysis(AFP)which may be helpful in clinical diagnosis,treatment and the prognosis judgement.
Methods:Five cases of HFMD combined with AFP who made regular followup visits to our hospital from June 2012 to June 2014 were selected. The clinical features and followup data of nerve conduction were analyzed retrospectively.
Results:Axonal or anterior horn cells injury was the main type in this disease. There was a positive correlation between the decrease of action potential amplitude and limb dysfunction. The return of action potential amplitude was most prominent in the first review.The femoral nerve injury was the most common. The decrease of action potential amplitude of motor conduction was seen. The cases whose amplitude decreased 30% and below during the early onset could be observed limb dysfunction after 6~12 months.
Conclusion:In the management of HFMDcombined with AFP,it is important to diagnose and treat in time to reduce the rate of disability.
