NICU中高危新生儿先天性甲状腺功能减低症筛查的临床分析

Clinical analysis of the screening for congenital hypothyroidism in moderate and high-risk neonates in NICU

摘要: 目的:探讨NICU中高危新生儿先天性甲状腺功能减低症(简称甲减)的发病情况。
方法:2013年6月至2015年6月内蒙古包头市第八医院NICU收治住院的高危新生儿230例,出生3~7 d后,静脉取血检测甲状腺功能三项,包括促甲状腺素、游离T3、游离T4。根据2012国内颁布的《妊娠和产后甲状腺疾病诊治指南》中关于先天性甲减诊断指标进行诊断与治疗。
结果:5例符合原发性亚临床甲减的诊断标准,检出阳性率达1/46。
结论:NICU中检出先天性甲减病例较高,尤以亚临床甲减及暂时性甲减多见,需注意筛查并追踪甲状腺功能指标,辅以必要的治疗。但是否作为常规项目筛查,需更大样本的临床研究确定。

Abstract: Objective:To study the incidence of congenital hypothyroidism in moderate-and high-risk neonates in NICU.
Methods:A total of 230 high-risk neonates were treated in NICU of No.8 Hospital of Baotou from June 2013 to June 2015.On 3~7 d after birth,the thyroid function was detected,including thyroxin,T3 and T4.According to the Diagnosis and Treatment Guidelines to Thyroid Diseases During and After Pregnancy published in 2012,the diagnosis was made and treatment was performed.
Results:There were 5 cases meeting the diagnosis standard of primary subclinical hypothyroidism,the positive detection rate being 1/46.
Conclusion:The proportion of congenital hypothyroidism cases in NICU is relatively high,and subclinical and temporary hypothyroidism is more common.Therefore it′s necessary to pay attention to screening and to follow the thyroid indexes,and proper treatment can be given in time of need.However,a larger sample is needed for clinical research to determine whether it can be regarded as a routine screening.
