儿童Graves病的免疫机制研究进展

Research advance in the immunological mechanism of Graves' disease in children

摘要:
Graves病的发病机制涉及遗传、环境、自身免疫等因素。其中免疫机制是中心环节,近年的研究热点集中在T细胞免疫表型、各亚群功能及细胞因子的免疫调节等方面。甲状腺自身抗体促甲状腺素受体抗体、甲状腺过氧化物酶抗已广泛应用于Graves病的临床诊断、疗效评估及预后判断;CD4+ T细胞和CD8+ T细胞,Th1细胞和Th2细胞发挥着复杂而重要的免疫效应,CD4/CD8、Th1/Th2比例失衡可导致免疫紊乱;CD4+ CD25+调节性T细胞、自然杀伤细胞发挥免疫抑制作用,是Graves病的保护性因素;白细胞介素2、白细胞介素6、肿瘤坏死因子α、α干扰素、γ干扰素、胰岛素样生长因子Ⅰ等细胞因子发挥不同的免疫调节作用。

Abstract:
The pathogenesis of Graves' disease(GD) involves genetic, environmental, autoimmune and other factors. Among them, the immune mechanism is the central part. T-cell phenotype, and immune regulation of each subgroup and cytokines have been studied intensively in recent years. Thyroid autoantibodys, such as TRAb and TPOAb, have been widely used in clinical diagnosis,therapeutic evaluation and prognosis prediction of GD; CD4+T and CD8+T cells, as well as Th1 and Th2 cells have the complex and important immunologic function,so the imbalance of CD4/CD8 and Th1/Th2 can lead to immunological disorders; furthermore,CD4+CD25+ regulatory T cells and NK cells can exert their immunosuppression effect,acting as the protective factor of GD; IL-2,IL-6,TNF-α,IFN-α,INF-γ,IGF-1 and other cytokines play different roles in the immune regulation.
