中药联合针刺治疗儿童抽动障碍临床疗效观察

Observation on the clinical therapeutic effect of traditional Chinese medicine combined with acupuncture on tic disorders in children

摘要: 目的 观察中药联合针刺治疗小儿抽动障碍的临床疗效。方法 回顾性研究2022年1月至2023年6月天津中医药大学第一附属医院收治的121例抽动障碍患儿,按治疗方法不同分为观察组56例和对照组65例。对照组予中药联合常规西药治疗,观察组予中药联合针刺治疗,疗程8周。观察两组的临床疗效、耶鲁综合抽动严重程度量表评分及临床疗效总评量表积分情况。结果 观察组总有效率为82.1%(46/56),对照组为73.8%(48/65),两组比较差异无统计学意义(P>0.05)。治疗后观察组中医证候疗效优于对照组,差异有统计学意义(P<0.05)。治疗后两组耶鲁综合抽动严重程度量表评分及临床疗效总评量表量表-疾病严重程度积分均明显下降,与治疗前比较差异有统计学意义(P<0.05),组间比较差异无统计学意义(P>0.05)。亚组分析显示:对不同中医辨证分型、不同分型、不同严重程度抽动障碍,两组治疗均较治疗前有效,但组间比较差异无统计学意义(P>0.05)。结论 中药结合针刺与中药结合西药治疗抽动障碍疗效相当,中药结合针刺治疗有助于改善患儿的中医证候,值得临床推广应用。

Abstract: Objective To observe the clinical efficacy of traditional Chinese medicine (TCM) combined with acupuncture in the treatment of tic disorders in children.Methods Totally 121 children with tic disorders treated in the First Teaching Hospital of Tianjin University of Traditional Chinese Medicine from January 2022 to June 2023 were selected and divided into observation group(n=56) and control group(n=65) according to different treatment methods. The control group was treated with TCM drugs combined with western drugs, and the observation group was treated with TCM drugs combined with acupuncture. The treatment period was 8 weeks. Observe the clinical efficacy, Yale global tic severity scale(YGTSS) score, and clinical global impression(CGI) score of the two groups.Results The total effective rate of the observation group was 82.1%(46/56), while the control group was 73.8%(48/65). There was no statistical difference between the two groups(P>0.05). After treatment, the observation group was superior to the control group concerning the effect on TCM syndrome(P<0.05). After treatment, the YGTSS score and CGI-SI scale score of the two groups significantly decreased; compared with those before treatment, the difference was statistically significant (P<0.05), but there was no statistically significant difference between the two groups (P>0.05). Subgroup analysis showed that: for different types of TCM syndrome differentiation, different types, and different severity of TD, the treatment was more effective than before treatment in both groups, but there was no statistical difference between the two groups(P>0.05).Conclusion The therapeutic effect of TCM drugs combined with acupuncture is equivalent to that of TCM drugs combined with Western medicine in the treatment of tic disorders. TCM drugs combined with acupuncture is helpful to improve the TCM syndrome of children and is worthy of clinical application.
