双侧强化训练对痉挛型偏瘫儿童粗大运动功能和步行功能的影响

Effect of bilateral intensive training on gross motor function and walking function in children with spastic hemiplegia

摘要: 目的:观察双侧强化训练对痉挛型偏瘫儿童粗大运动和步行功能的疗效。
方法:以2014年2月至2019年12月河南中医药大学第一附属医院儿童脑病康复诊疗中心诊断收治的30例偏瘫患儿为研究对象,随机分为对照组和观察组各15例。对照组应用常规康复治疗,观察组应用常规康复治疗的基础上,运动疗法应用双侧强化训练,两组均治疗3个疗程。治疗前后对患儿分别采用粗大运动功能评定量表(GMFM-88项)、足印分析法、10 m步行测试、1 min步行测试进行评定。
结果:两组治疗后GMFM-88项测试评分、步长和步宽、10 m步行时间及1 min步行距离均优于治疗前,差异有统计学意义(P<0.05),且观察组优于对照组,差异有统计学意义(P<0.05)。
结论:双侧强化训练结合常规康复治疗可以改善痉挛型偏瘫儿童的粗大运动功能,纠正异常步态,提高步行的速度、耐力和稳定性。

Abstract: Objective:To observe the effect of bilateral intensive training on gross motor function and walking function in children with spastic hemiplegia.
Methods:The 30 children with spasmodic hemiplegia diagnosed and treated in the First Affiliated Hospital of Henan University of Traditional Chinese Medicine from Feb. 2014 to Dec. 2019 were randomly divided into control group(15 cases) and observation group(15 cases). Both groups received routine rehabilitation treatment, and bilateral intensive training was given to the observation group as exercise therapy; both groups were treated for 3 courses. The children were assessed before and after treatment by gross motor function scale assessment(GMFM-88), footprint analysis, 10-meter walk test and 1-minute walk test.
Results:The scores in GMFM-88 test, step length, step width,10 m  walking time and 1 min walking distance in the two groups were better after treatment than before treatment(P<0.05), and the observation group was better than the control group(P<0.05).
Conclusion:Bilateral intensive training combined with conventional rehabilitation can improve the gross motor function, correct abnormal gait, and increase walking speed, endurance and stability of children with spastic hemiplegia.
