王雪峰教授从五脏相关辨治儿童多发性抽动症经验探析

摘要： 儿童患多发性抽动症的人数逐渐增多，且由于本病患儿特殊的症状表现，本病的高发引起了社会高度重视，并对患儿及家长造成了一定的心理负担，本病多为5~10岁儿童患病，因此也给成长过程中的患儿心理健康发育造成了负面影响。经过多项研究证明，中医药因其辨证论治的优势对于本病有着良好疗效，且副作用远远小于西医治疗，患儿及家长更易接受、更易配合，以收到良好疗效。王雪峰教授以五脏相关理论为基础，认为儿童多发性抽动症的病因病机离不开五脏，根据“诸风掉眩，皆属于肝”，且本病在中医中属于“肝风”“慢惊风”“抽搐”“瘛疭”“筋惕肉瞤”等范畴，由此王教授以肝为始，通过中医脏腑理论、五行学说，基于“五脏一体观”理论，联系临床症状，并结合儿童体质，“小儿脏腑娇嫩、行气未充、生机蓬勃、发育迅速”的特殊生理特点和“发病容易、传变迅速、脏器清灵、易驱康复”的特殊病理特点，分析本病的病位除位于肝外，与脾、心、肺、肾都密切相关，针对病因病机进行简要分析，并据此确定其不同治则治法，浅析遣方用药。

Abstract: The number of children with multiple tic disorder (TD) has been increasing. The incidence of this disease has drawn much social attention due to the special symptoms of child patients, which brings some psychological burden to both children and their parents. The disease mostly occurs in the children aged 5 to 10 years, which has also led to negative impact on the psychological health development of children during their growth process. Many studies have proved that traditional Chinese medicine (TCM) has great effect on this disease because of its advantages of differential treatment, and has much lower rate of adverse reactions than western treatment. It is more likely to be accepted by children and parents, who are more willing to cooperate in the treatment, and it has resulted in great effect. Based on the five viscera theory, Professor Wang Xuefeng believes that the causes and mechanism of multiple TD are closely related to five viscera. According to TCM saying——the wind leading to dizziness belongs to liver, and due to the fact that this disease belongs to the category of liver wind, slow infantile convulsion, spasm, etc. in TCM, Prof. Wang carries out the treatment with liver as the starting point. Based on the TCM theory of viscera, Five-element theory and the theory of five viscera as a whole, and taking clinical symptoms and physiques of children into account, which include the special physiological features (child viscera being delicate, qi being insufficient and development being fast) and the special pathological features (occurring easily, spreading quickly, viscera being clear and recovery being rapid), Prof. Wang thinks that the disease is not only in the liver, but also related to spleen, heart, lungs and kidneys. A brief analysis is made regarding the causes and mechanisms, based on which different treatment principles are decided on, and prescriptions and medications are analyzed.
