品管圈在住院患儿营养风险筛查中的应用研究

Application of the quality control circle in the screening of nutritional risks in hospitalized children

摘要: 目的 探讨品管圈活动在住院患儿营养风险筛查中的应用效果。 方法 基于品管圈程序,确立“提高住院患儿营养风险筛查率”为活动主题,设计问题针对普外科、消化内科、风湿免疫科、神经内科展开现状调查,对于导致住院患儿未进行营养风险筛查因素实施详尽的分析探讨,拟定对策对4个科室进行实施,比较品管圈活动前后住院患儿营养风险筛查率。 结果 针对改善重点,通过品管圈的手法实施,活动前后柏拉图对比进步率为88.83%,目标达成率为93.35%,住院患儿营养风险筛查率显著提高,活动后筛查率为84.4%(1 207/1 430),高于活动前40.5%(530/1 309),差异有统计学意义(P<0.05)。无形成果均呈正向成长,圈员们对品管手法的运用更加熟练,团队凝聚力也大幅提升。 结论 品管圈活动可有效提高住院患儿营养风险筛查率,利用全面质量管理PDCA循环法,解决临床实际问题,在提高住院患儿营养风险筛查率同时,为存在营养风险的患儿提供营养干预及营养管理,为临床营养支持提供依据。

Abstract: Objective To explore effect of quality control circle (QCC) activity in the screening of nutritional risks in hospitalized children. Methods Based on QCC procedure, we established the activity theme as "improving the screening rate of nutrition risk in hospitalized children". Targeted questions were designed for multiple divisions to carry out surveys, including Division of General Surgery, Division of Gastroenterology, Division of Rheumatology Immunology and Division of Neurology. The factors were analyzed and discussed in detail concerning why nutritional risk screening was not performed for hospitalized children. Subsequently, the rate of conducting nutritional risk screening before and after QCC was compared in the abovementioned four divisions. Results In view of the improvement focus, through the implementation of QCC protocol, the improvement rate of Pareto comparing before with after the activity was 88.83%, and the goal achievement rate was 93.35%. The rate of nutritional risk screening in hospitalized children was significantly improved. The screening rate after the activity was 84.4%(1 207/1 430), higher than that before the activity(40.5%,530/1 309), and the difference was statistically significant(P<0.05). Intangible results showed positive growth, the circle members were more skilled in the use of quality control techniques, and team cohesion was also significantly improved. Conclusion QCC can effectively increase the rate of nutritional risk screening in hospitalized children. The PDCAbased QCC protocol can effectively improve the screening rate of nutrition risk in hospitalized children, thus solve the clinical practical problems. While improving the rate of nutritional risk screening in hospitalized children, nutritional intervention and nutrition management should be provided for children with nutritional risks, so as to provide evidence for clinical nutritional support.
