Significance of serum uric acid, CysC and NT-proBNP in the detection of children with congenital heart disease and pulmonary arterial hypertension

Abstract: Objective To explore the significance of serum uric acid, cystatin(CysC) and N-terminal pro brain natriuretic peptide(NT-proBNP) in the detection of children with congenital heart disease and pulmonary arterial hypertension(PAH-CHD).Methods A total of 126 children with congenital heart disease(CHD) who were treated in our hospital from January 2018 to December 2019 were enrolled, including 74 cases with pulmonary arterial hypertension(PAH, observation group) and 52 cases without PAH(control group). The levels of serum uric acid, CysC and NT-proBNP were determined. The levels of the three indexes were compared among children with different clinic opathological features. The predictive value of serum uric acid, CysC and NT-proBNP for PAH-CHD was evaluated by receiver operating characteristic(ROC) curves.Results The levels of serum uric acid, CysC and NT-proBNP in observation group were significantly higher than those in control group (P<0.05). The levels of serum uric acid, CysC and NT-proBNP were not related to gender, age or CHD types(P>0.05),but they were related to cardiac function grading and PAH severity(P<0.05).The results of ROC curves showed that AUC values of serum uric acid,  CysC, NT-pro BNP and their combination for diagnosis of PAH-CHD were 0.787,0.712, 0.815 and 0.874, respectively. AUC of combination diagnosis was higher than that of single index.Conclusion The diagnostic efficiency of serum uric acid combined with CysC and NT-proBNP is relatively higher for PAH-CHD, which is of certain clinical value.
