婴幼儿功能性消化不良综合征中西医结合诊疗与管理专家共识

Abstract

Abstract:

Infantile functional dyspepsia syndrome is a common syndrome complex of digestive system in children aged 0 to 3 years. Clinically, it is mainly characterized by such symptoms as decreased appetite, belching and abdominal distension, retching or vomiting, repeated crying, abdominal pain, and abnormal defecation, which seriously affect the growth, development and quality of life of infants. Both traditional Chinese medicine (TCM) and western medicine have certain advantages in the treatment and management of this disease. Led by the National Clinical Collaboration Group for Integrated Traditional Chinese and Western Medicine on Pediatric Functional Dyspepsia—a major difficult disease program, the consensus was developed through cooperation with integrated TCM and western medicine experts in pediatric gastroenterology, experts in pediatric Tuina and external therapy, and experts in methodology. Based on the concept of integrating TCM and western medicine, this consensus clarifies the diagnostic criteria and key points of TCM syndrome differentiation, and establishes a comprehensive intervention system centered on protecting and reinforcing the spleen and stomach. Western medicine focuses on nutritional management and feeding guidance, recommending appropriate formula milk and scientific complementary feeding. TCM interventions mainly include pediatric Tuina, acupoint application, and dietary therapy, combined with personalized care guidance. This consensus provides a basis for standardized clinical diagnosis, treatment and family care, which is of great significance in increasing the level of integrated TCM and western medicine in the management of this disease.

食欲下降	（1）食量减少所需正常量的1/2以上；（2）排除器质性疾病、精神因素及药物性因素的影响；（3）食欲下降期间身高、体质量未见明显增长
嗳气、腹胀、肛门排气增多	（1）嗳气：反复出现，每日≥2次；（2）腹胀：多于餐后明显，可伴嗳气及肛门排气增多，嗳气及肛门排气后可缓解；（3）肛门排气增多：肛门排气较平时增多；每日≥2次
干呕或呕吐	（1）干呕：反复出现，每日≥2次；不存在自行诱导的干呕，经过适当评估，干呕无法用其他疾病来完全解释。（2）呕吐：反复出现，每日≥2次；不存在自行诱导的呕吐，无呕血、误吸、呼吸暂停、生长迟缓、喂养或吞咽困难、姿态异常；经过适当评估，呕吐无法用其他疾病来完全解释
反复哭闹或腹痛	（1）反复哭闹：反复出现，每日≥2次；表现为无明显诱因下出现反复的哭闹（无法表述腹痛的婴幼儿），监护人难以阻止和安抚但无生长迟缓、发热或其他疾病的证据。（2）腹痛：反复出现，每日≥2次；表现为无明显诱因下出现腹痛（可表述腹痛的幼儿），但无生长迟缓、发热或其他疾病的证据
排便异常	（1）满足以下条件之一：①排便次数或排便量较平时增多（6月龄以上婴幼儿排便次数每日>4次，6月龄以下婴儿排便量较平时增多）且粪便不成形，表现为稀烂便或糊状便；②粪便见奶瓣或未消化的食物残渣，较平时增多，或伴酸臭味；③粪便干结（Bristol粪便分类法中第一型及第二型），排便次数每周>2次，无排便障碍。（2）如果热量摄入充足，不会出现生长迟缓。（3）排除器质性疾病及药物性因素的影响
