基于数据挖掘的王晓鸣教授治疗小儿积滞组方规律探索

摘要:

目的 通过对王晓鸣教授治疗小儿积滞中药处方的整理、分析,总结王教授治疗小儿积滞用药经验。 方法 整理筛选2019年1月1日至2024年12月31日浙江省王晓鸣名老中医工作室门诊的处方资料,采用统计软件对药物进行频次、性味归经、药物功效、关联规则及聚类分析。 结果 共筛选出382首处方,高频药物19味。药物归经以脾经、肺经、胃经、肝经居多,苦、甘、辛味最常见,寒、温、平药性为主,使用最多的药物为消食、补虚及理气药。关联规则二阶支持度靠前的有茯苓-鸡内金,三阶支持度最靠前的为白术-茯苓-鸡内金。高频核心药物聚类挖掘到2类组合。 结论 王教授治疗小儿积滞紧扣“食积内停,脾运失健,气机郁滞,易兼郁热或肝郁”之核心病机,确立了“消食导滞、健脾益气、理气和中”的核心治则,并创造性地强调“肝脾同治”与“郁热清解”。

Abstract:

Objective To collate and analyze Professor Wang Xiaoming's traditional Chinese Medicine(TCM) prescriptions for pediatric food accumulation and stagnation, and summarize her clinical experience in medication for pediatric food accumulation and stagnation. Methods Prescription data from the outpatient department of Wang Xiaoming's Distinguished TCM Practitioner Studio in Zhejiang Province from January 1, 2019 to December 31, 2024 were collated and screened. Statistical software was employed to perform frequency analysis, analysis of medicinal properties (nature, flavor, channel tropism), analysis of medicinal efficacy, association rule analysis, and cluster analysis of the constituent herbs. Results A total of 382 prescriptions were screened out, identifying 19 high-frequency herbs. These herbs predominantly targeted the spleen, lung, stomach, and liver channels, with bitter, sweet, and pungent flavors being most common, and cold, warm, and neutral properties prevailing. The most frequently used herbs belonged to the categories of digestion-promoting, deficiency-tonifying and qi-regulating medicinals. The second-order association rule with the highest support was Fuling-Jineijin.The third-order association rule with the highest support was Baizhu-Fuling-Jineijin.Cluster analysis of high-frequency core herbs revealed two combinations. Conclusion Professor Wang's treatment for pediatric food accumulation and stagnation focuses on the core pathogenesis of "food accumulation leading to impaired spleen transportation, and qi stagnation, often accompanied by internal heat or liver qi stagnation." She establishes the core therapeutic principles of "promoting digestion and removing stagnation, strengthening the spleen and boosting qi, and regulating qi and harmonizing the middle Jiao." Innovatively, she emphasizes the simultaneous treatment of the liver and spleen and the clearance of stagnated heat.
