    Effects of Ganlu Xiaodu micropill and its volatile oil on RIG-Ⅰ/NF-κB signaling pathway in mice infected with influenza virus

    Objective
    To observe the effects of Ganlun Xiaodu micropill(GXM) and its volatile oil on the levels of IL-4, TNF-α, IFN-α/β and NS1 mRNA, RIG-Ⅰ mRNA and NF-κB protein in lung tissue of mice infected with influenza virus(IV), in order to explore the molecular mechanism of the antiviral effect of GXM and its volatile oil.
    Methods
    A total of 36 SPF BALB/c mice, half female and half male, were randomly divided into 6 groups, with 6 rats in each group. They were normal group(A group), model group(B group), gavage group(C group), intranasal group(D group), combination group(E group) and oseltamivir group(F group). Except the normal group, the remained 5 groups were modeled with influenza virus FM1-6-E2. On the seventh day after the model, the mice were killed by picking up the eyeball and releasing blood. The lung tissue was detected by transmission electron microscope and the image was analyzed. The content of IL-4, TNF-α and IFN-α/β in serum was determined by ELISA,and the expression of NS1 and RIG-ⅠmRNA in lung tissue of mice was detected by qRT-PCR. Western blot method was used to determine the expression of NF-κB protein in lung tissues of mice.
    Results
    (1) The ultrastructure of lung tissue in the model mice after IV infection showed necrosis of type Ⅱ cells, destruction of alveolar structure, thickening and shedding of cilium in the alveoli, and pulmonary inflammation was formed. All the treatment groups were improved in the ultrastructural abnormal change of the alveolar type Ⅱ epithelial cells.(2) After IV infection in mice, the content of IL-4, TNF-α and IFN-α/β in serum and the expression of NS1 mRNA, RIG-Ⅰ mRNA and NF-κB protein in lung tissues were all higher than those in A group. The difference was significant between B group and A group; Compared with the B group, the serum levels of IFN-α/β in the serum of C,D,E and F groups increased significantly, while the content of IL-4, TNF-α and the expression of NS1 mRNA, RIG-Ⅰ mRNA and NF-κB protein were obviously down-regulated (P<0.05). There was significant difference in the anti-IV effect among different dosage forms of GXM treatment groups(P<0.05),the effect being E group>C group>D group.
    Conclusion
    GXM has the function of anti-IV infection. It inhibits the pathological reaction of inflammation, enhances the content of immune factors and reduces the immune inflammatory injury induced by IV by inhibiting the activation of RIG-Ⅰ/NF-κB signaling pathway.

    Objective
    To observe the effect of Qingzao Jiufei decoction (QJD) and its decomposable agent on the toll-like receptors(TLR) 2/myeloid differentiation primary response gene 88 (MyD88) / (NF-κB) signal pathway in MP infected mice,and to explore its mechanism of action of resistance to MP.
    Methods
    Totally 144 BALB/c mice, about 20 g in weight, were randomly divided into normal group(A),model group(B),QJD group(C),QJD decomposable agentⅠgroup(D),QJD decomposable agent Ⅱ group(E) and azithromycin group(F),with 24 mice in each group. Each treatment group was made into MP infection model and the corresponding group was given corresponding drugs by intragastric administration after modeling. The serum and the lung tissues were detected after 3,7,10 and 14 days of MP infection. HE staining was used to observe the pathological change of lung tissues, qPCR was used to detect the changes of TLR-2 mRNA in lung tissues, ELISA was used to detect the expressions of MyD88 and TNF-α in serum, and Western blot was used to detect the expression levels of NF-κB in lung tissues.
    Results
    The interstitial inflammatory changes of lung occurred 3d after MP infection,which were most significant on the 7d,and then gradually became less and less significant. Pulmonary inflammation in each treatment group was improved, especially in Group C and Group F on the 10d and 14d. The expression level of TLR-2 and NF-κB in the lung tissue of mice after MP infection increased, and the content of MyD88 and TNF-α in serum was also increased(P<0.05).The peak time of each indicator was different with NF-κB appearing earliest (3rd day),TLR-2 and TNF-α peaking on the 7th day, while MyD88 was relatively late,peaking on the 10th day. On the 7th day in Group C(P<0.05),the expression level of TLR-2 and NF-κB in lung tissue decreased, and the content of MyD88 and TNF-α in serum was also decreased. Group D also played a similar role to Group C on the 10th day and 14th days, but weaker than Group C (P>0.05); in Group E on the 14th day TLR-2 mRNA expression slightly increased.
    Conclusion
    The mechanism of QJD against MP infection is associated with reducing the release of proinflammatory factors and regulating the conduction of TLR-2/NF-κB signaling pathway,in which the decomposition agent Ⅰ plays a major role.

    Effect of Shenzong granule on micro-inflammation in the rats with primary nephrotic syndrome

    Objective
    To study the effect of Shenzong granule on micro-inflammation in the rats with nephritic syndrome.
    Methods
    Of 40 SPF male Wistar rats,8 were chosen randomly as the normal control group (group A),and the remained 32 rats were made into nephrotic syndrome models,which were then divided into 4 groups with 8 in each group:model group(B),hormone group(C),Shenzong group(D) and hormone plus Shenzong group(E).Group C,D and E were intervened respectively with hormone,Shenzong granule and hormone plus shenzong granule;detect the level of serum neuclear factor κB(NF-κB),interleukin 1β(IL-1β),tumor necrosis factor α(TNF-α),IL-6,and C-reactive protein(CRP).
    Results
    The level of NF-κB,IL-1β,TNF-α,IL-6 and CRP in rats with nephrotic syndrome was significantly reduced in group E,and the difference was statistically significant (P<0.05).
    Conclusion
    The micro-inflammation in rats with nephrotic syndrome is improved by hormone and Shenzong granule. Its working mechanism may be through reducing IL-1β and TNF-α level,the IL-6 secretion,and decreasing the content of CRP and NF-κB,which results in slowing down the reaction of inflammation factors and blocking the inflammation.

    Objective
    To observe the curative effect of Xiyanping injection in the treatment of Mycoplasma pneumoniae pneumonia in children.
    Methods
    The data of 571 cases of children with Mycoplasma pneumoniae pneumonia were reviewed, who were treated from Jan. 1,2012 to Dec. 31,2016 in Department of Pediatrics, Central Hospital Affiliated to Shenyang Medical College. The 571 cases of children with Mycoplasma pneumoniae pneumonia were divided into treatment group(n=278) and control group(n=293). Both groups were given conventional treatment of relieving cough, removing sputum, relieving asthma, supplementing liquid and inhaling oxygen. The control group was treated with erythromycin injection by intravenous dripping, twice a day. The observation group received intravenous dripping of Xiyanping injection in addition to the erythromycin, once a day. Both groups were treated for 7 days as a course. Observe the clinical effect and the time for symptoms and signs to improve in the two groups.
    Results
    The effect of the observation group was better than that of the control group, the difference being statistical(Z=-3.94,P<0.05). The cure rate of observation group was 70.86% (197/278), significantly higher than that of control group(54.94%, 161/293),and there was statistical difference(P<0.05). The time of fever disappearance, cough relief and the disappearance of lung rales was shorter in observation group than that in control group, and the difference was statistically significant(P<0.01).
    Conclusion
    Xiyanping injection has good effect in the treatment of children with Mycoplasma pneumoniae pneumonia, which combined with antibiotic, can help to relieve the clinical symptoms and signs, deserving clinical application.

    Clinical effect of the treatment for spleen-and-stomach-deficiency type anorexia by using self-made Spleen and Stomach soup

    Objective
    To study the effect of the treatment for spleen-and-stomach-deficiency type anorexia by using self-made Spleen and Stomach soup.
    Methods
    A total of 58 children with spleen-and-stomach-deficiency type anorexia were divided into control group of 29 cases and observation group of 29 cases by random number table method,who were admitted to our hospital from February 2015 to August 2016.Clinical effect in the two groups was observed.The control group received conventional indigestion tablets, and took vitamin supplement therapy according to doctor's advice,and the observation group accepted self-made Spleen and Stomach soup for treatment.The treatment lasted for two courses,two weeks in a course.
    Results
    The total effective rate of control group was 72.4% and that of the observation group was 93.1%,the observation group was higher than control group,the difference being statistical(P<0.05).
    Conclusion
    The self-made Spleen and Stomach soup has significantly effect in the treatment of children with spleen-and-stomach-deficiency type anorexia,which is worthy of clinical promotion.

    Objective
    To observe the clinical effect and the safety of Xiyanping combined with ribavirin in the treatment of children with hand-foot-mouth disease(HFMD).
    Methods
    A total of 70 HFMD children were included as the research subjects,who were treated in our hospital from Feb. 2013 to Apr. 2016,and they were randomly divided into observation group(39 cases)and control group(31 cases).Both groups were treated with ribavirin,and Xiyanping was given to the children in the observation group. Observe the clinical effect in the two groups, as well as the time of herpes disappearance,fever abatement,cough relief and disappearance of sore throat. After 3 days,the CK-MB level,the incidence of adverse reactions,the rate of turning into critical condition and the complication rate were determined.
    Results
    There was no statistical difference in the clinical effect between the two groups (P>0.05).The time of herpes disappearance,fever abatement,cough relief and the disappearance of sore throat was significantly shorter in the observation group,and the CK-MB level after treatment was significantly lower,the difference being statistical (P<0.05). There was no statistical difference in the incidence of adverse reactions,complications and the rate of turning into critical condition (P>0.05).
    Conclusion
    Xiyanping combined with ribavirin has significant effect in the treatment of children with hand-foot-mouth disease,and it's also very safe.

    Clinical characteristics of intensive chicken pox in children and the treatment

    Objective
    To explore the clinical characteristics of children's intensive chicken pox and its effective treatment method,providing the basis for clinical work.
    Methods
    A total of 98 children with intensive chicken pox were chosen,who were treated in Hunan Children's Hospital from Feb. 2015 to Feb. 2017,and their clinical data were analyzed.
    Results
    (1)Cause analysis:about 91.84%(90/98) children had contact with children with chicken pox;about 6.12%(6/98) children had contact with children with herpes zoster,2.04%(2/98) with unknown cause. (2) Clinical manifestations:all the 98 cases of children had a high fever,fatigue and sleepiness,including skin sores(68.37%,67/98),gastrointestinal(57.14%,56/98),cough(66.33%,65/98),muscle pain(20.41%,20/98),tic(11.22%,11/98),palpebral conjunctiva herpes(59.18%,58/98),oral herpes(43.88%,43/98) and genital herpes (27.55%,27/98).(3)Laboratory results:white blood cell count,neutrophil percentage and number of platelets were decreased obviously,while lymphocyte percentage,C-reactive protein,lactate dehydrogenase,aspartate transaminase,serum creatinine and blood urea nitrogen content were higher. (4) Complications:94.90%(93/98) of the children had pulmonary bacterial infections;liver injury occurred in 91.84%(90/98) of the children,encephalitis in 83.67%(82/98), thrombocytopenia in 54.08%(53/98),and myocarditis in 42.86%(42/98). (5) Prognosis:Two out of 98 children died due to long-term use of glucocorticoid and reduced platelet,which resulted in multiple organ failure,while the other 96 children recovered.
    Conclusion
    The condition of intensive chicken pox is relatively complex;in addition to high fever,fatigue and sleepiness,skin sores and cough are the most common clinical manifestations,which often involves the liver and lungs;treatment should focus on proper medication and be alert to the occurrence of complications.

    Objective
    To analyze the influence of exogenous nitric oxide inhalation on the symptom of hypoxic respiratory failure and the cardiac function in neonates.
    Methods
    A total of 68 neonates with hypoxic respiratory failure treated in Kaifeng Children's Hospital from Aug. 2013 to Sep. 2014 were selected as the research subjects,and then were divided randomly into two groups of 34 cases,the observation group and the control group. Both groups were treated by mechanical ventilation with oxygen and continuous mechanical intermittent positive pressure ventilation,and the ventilation parameters were adjusted in time with the changes in blood gas of the neonates. The observation group was additionally treated with nitric oxide inhalation. Compare the following indexes between the two groups before and after treatment:oxygenation index,blood PaO2,blood PaCO2 and mean airway pressure. Make a contrast of left ventricular ejection fraction,cardiac index and heart rate between the two groups. Meanwhile,the adverse reactions were compared.
    Results
    After treatment,the oxygenation index was 5.23±1.88 and PaO2 was(52.29±2.86)mm Hg in the observation group,significantly lower than those of control group:8.64±1.75 and (66.34±1.57)mm Hg;PaCO2 was (45.67±2.38)mm Hg,mean airway pressure was (12.95±1.27)cm H2O,left ventricular ejection fraction(LVEF) was (79.83±1.35)%,cardiac index was (3.64±0.15)L/(min·m2) and heart rate was (79.36±1.11)/min in the observation group,which were significantly higher than those in the control group:(35.61±2.29)mm Hg,(9.53±1.37)cm H2O,(74.25±1.52)%,(3.15±0.38)L/(min·m2)and (75.27±1.18)/min,the difference being statistically significant(P<0.05).There was no statistical difference in the incidence of adverse reactions (P>0.05) between the two groups.
    Conclusion
    Exogenous nitric oxide inhalation has good effect in the treatment of neonates with hypoxic respiratory failure;it can significantly improve the cardiac function of the neonates with milder adverse reactions,which is worth clinical popularizing and applying.

    Effect of lung protective ventilation strategy on the treatment for children with acute respiratory distress syndrome

    Objective
    To observe the effect of lung protective ventilation strategy on the treatment for children with acute respiratory distress syndrome.
    Methods
    From January 2014 to January 2016,80 cases of acute respiratory distress syndrome in Hunan Children's Hospital were randomly divided into the control group and the observation group,40 cases in each group. The control group received traditional mechanical ventilation strategy,and the observation group adopted the lung protective ventilation strategy,the other treatments being the same. The indicators were observed after treatment,including inspiratory peak pressure,positive end expiratory pressure,mean airway pressure,inhaled oxygen concentration,respiratory rate,arterial blood gas analysis,and complications.
    Results
    The peak inspiratory pressure,mean airway pressure and suction parameters of oxygen concentration in observation group were significantly lower than those in control group,while positive end expiratory pressure and frequency of respiration parameters were significantly higher,the difference being statistically significant(P<0.05). The pH and PaO2 of the observation group were significantly lower than those of the control group,and the PCO2 was significantly higher than that of the control group,the difference being statistically significant (P<0.05).The incidence rate of ventilator induced lung injury,intraventricular hemorrhage and patent ductus arteriosus had no statistical difference after treatment in observation group(P>0.05),while the incidence rate of adverse reactions was lower than that of control group,the difference being statistically significant(P<0.05).
    Conclusion
    In the treatment of children with acute respiratory distress syndrome,the lung protective ventilation strategy can optimize the ventilator parameters,improve the arterial blood gas value of children,and reduce the incidence of adverse reactions,which has important clinical value and application significance.

    Changes and significance of immune indexes and serum myocardial enzyme indexes in neonates with pneumonia

    Objective
    To study the changes of immune indexes and the serum myocardial enzyme indexes and its significance in neonates with pneumonia.
    Methods
    A total of 90 neonates were selected as the research subjects,who were treated in Pediatric Department of Heze Municipal Hospital from Nov. 2015 to Nov. 2016,and they were included as the observation group. Another 90 healthy neonates delivered in our hospital during the same period were included as the control group. The situation of cellular immunity and erythrocyte immunity as well as the level of serum myocardial enzymes were determined in the two groups.
    Results
    The situation of cellular immunity and erythrocyte immunity in neonates with pneumonia was significantly worse than that in control group,the difference being statistical(P<0.05).The level of serum myocardial enzymes in observation group was significantly higher than that in control group with statistical difference(P<0.05).
    Conclusion
    Compare with the healthy neonates,the level of serum myocardial enzymes is abnormally high in neonates with pneumonia,while the cellular immunity and erythrocyte immunity are worse. Clinically the neonates with pneumonia can be diagnosed by the changes in their immunity situation and serum myocardial enzymes,based on which the effective treatment plan can be made.

    Relationship of the expression of TRPC1 with clinicopathological parameter and prognosis of hepatoblastoma in children

    Objective
    To defect the expression of TRPC1 in the tissues of hepatoblastoma and to analyze its relationship with clinicopathological features and prognosis.
    Methods
    The pathological data and clinical data of 16 children with hepatoblastoma(HB) were collected, who underwent hepatectorny in Hunan Children's Hospital from Jan. 2008 to Dec. 2015. The expression of transient receptor potential channel-related protein 1(TRPC1) in HB tissues was detected by immunohistochemistry, its relationship with clinicopathological features(neoplastic tissues and non-neoplastic tissues, the sizes of neoplasm) was analyzed, and its relationship with prognosis was analyzed in the 3 to 5 years of follow-up.
    Results
    The expression of TRPC1 in the 16 HB children was different, with high expression accounting for 62.5%(10/16) and low expression 37.5%(6/16).The high expression of TRPC1 had statistical difference in the neoplastic and non-neoplastic tissues and in the sizes of neoplasms(P<0.05).The 3-year survival rate was 72.73%(8/11),and the 5-year was 50%(3/6).There was no statistical significance in TRPC1 expression comparing 3-year with 5-year survival rate(P>0.05).
    Conclusion
    The expression of TRPC1 may be related to the development and progression of HB in children, but it may be irrelevant to prognosis.

    Objective
    To explore the feasibility of non-nutritive sucking combined with early micro-feeding to reduce feeding intolerance in preterm infants.
    Methods
    A total of 76 premature infants born in our hospital from March 2014 to August 2015 were divided into the control group and the observation group,each group consisting of 38 cases. Children in the both groups were given warm-keeping and early micro-feeding,and the observation group was added with non-nutritive sucking on the basis of the control group. During the micro-feeding,the premature infants were given no-hole rubble nipple to suck for 5 minutes respectively at the early,middle and late stage,8 times a day,until the sucking reflex and swallowing function were developed. The following indexes were observed:the disappearance time of gastric retention,of gastroesophageal reflux and of abdominal distension,the length of parenteral nutrition,the length of achieving total gastrointestinal nutrition,and the milk volume at the end of the second week as well as the days of recovering to birth body mass.
    Results
    The disappearance time of gastric retention,of gastroesophageal reflux and of abdominal distension were significantly less in observation group than that in control group,and the difference was statistical (P<0.05). The length of parenteral nutrition,the length of achieving total gastrointestinal nutrition and the days of recovering to birth body mass were significantly lower in observation group than in control group,while the milk volume at the end of the second week was significantly higher,the difference being statistical (P<0.05).
    Conclusion
    The therapy of early micro-feeding combined with non-nutritive sucking is feasible in the treatment of premature infants with feeding intolerance,which can rapidly relieve gastric retention,gastrointestinal reflux and abdominal distension,and can also help to recover the body mass to normal,so it is worth popularizing.

    Objective
    To study the clinical effect of modifying iron phase-supplement treatment for infants with nutritional iron-deficiency anemia.
    Methods
    A total of 34 children with moderate-severe iron-deficiency anemia were chosen as the control group, who were treated in Changshu TCM Hospital from Feb. 2000 to Oct. 2001.A total of 169 children with moderate-severe iron-deficiency anemia were included as the observation group, who were treated from June 2005 to June 2013.In the observation group, ferrous succinate tablet at 3-4 mg/(kg·d)(divided into two times) for those children with hemoglobin under 90 g/L. at the first phase, and at the second phase, ferrous succinate tablet was given at 3-4 mg/(kg·week) for those with hemoglobin between 90 g/L and 110 g/L, once a week, until the RDW was less than 14.6%. In the control group, ferrous succinate tablet was taken at 5 mg/(kg·d)(divided into two times) for those with hemoglobin under 90 g/L at the first phase, and at second phase, ferrous succinate tablet was given at 3-4 mg/(kg·d)(divided into two times) for those with hemoglobin between 90 g/L and 110 g/L; at the third phase, ferrous succinate tablet was taken at 3 mg/a time, two time a day, for those with hemoglobin over 110 g/L, until the RDW was under 14.6%.In the observation group, blood routine and reticulocytes were examined in the first week of treatment; after that, blood routine examination was performed once a week for those with hemoglobin under 90 g/L, once in two weeks for those between 90 g/L and 110 g/L, and once in four weeks for those over 110 g/L until the RDW was under 14.6%. In the control group, blood routine and reticulocytes were examined in the first week of treatment, and after that blood routine examination was performed once in two weeks. The time of RBC-related parameters reaching the standard and the adverse reactions in both groups at the second phase were observed.
    Results
    There was no statistical difference in hemoglobin or the time of MCV and RDW reaching standard between the two groups after treatment(P>0.05). The appetite was obviously improved in the first week in both groups; 3 to 5 day after taking medicine, there was black feces. Nausia occurred in 17 cases at the first phase in observation group, but no nausia, abdominal pain or decreased appetite at the second phase. There were 8 cases of abdominal pain, nausia and vomiting in the control group.
    Conclusion
    The modified treatment achieves similar clinical effect to the original one, with fever adverse reactions and higher compliance of parents, and it is easier for physicians to use and is more suitable to be clinically applied.

    Objective
    To observe the curative effect of Metapex paste on chronic periapical periodontitis with sinus of deciduous teeth.
    Methods
    One hundred and eleven children(110) with chronic periapical periodontitis with sinus were selected from Department of Stomatology, Hunan Children's Hospital from January 2016 to December 2016. The subjects were randomly divided into control group and observation group of 55 cases. The control group received zinc iodoform paste, and the observation group received Metapex paste. The clinical efficacy and visual analogue scale were compared between the two groups.
    Results
    The effect in observation group was better than that in the control group, the difference being statistically significant (P<0.05). There was no significant difference in VAS score between the two groups before treatment (P>0.05). At 1, 3 and 7 days after treatment, the VAS scores of the observation group were significantly lower than those of the control group, and there was statistical difference (P<0.05).
    Conclusion
    Metapex paste has a better therapeutic effect on deciduous teeth with chronic periapical periodontitis with sinus, which can be used as the preferred solution for clinical treatment.

    Objective
    To observe the effect of creatine phosphate on pediatric coxsackie virus myocarditis(CVM).
    Methods
    From Jan. 2012 to Jan. 2016,80 CVM patients were treated in the Cardiovascular Department of Harbin Children's Hospital;these patients were divided into observation group and control group randomly,each with 40 cases. Both groups were routinely given antiviral treatment and symptomatic treatment. Patients in control group were treated by 1,6-diphosphate fructose(FDP) by intravenous dripping on the basis of routine therapy;patients in observation group was treated by creatine phosphate by intravenous dripping on the basis of routine therapy,both groups being treated for 2 weeks. The level of lactic dehydrogenase(LDH),hydrocybutyrate defydrogenase(HBDH),creatine kinase(CK) and creatine kinase isoenzyme(CK-MB) of both groups was observed before and after treatment;electrocardiogram,complication such as cardiac dilatation,improvement of heart failure and clinical efficacy were observed before and after therapy in both groups.
    Results
    LDH,HBDH,CK,CK-MB in both groups were significantly lower after treatment than before treatment(P<0.05). LDH,HBDH,CK,CK-MB in observation group were significantly lower than control group after treatment(P<0.05). ST-T changes and the total effective rates in the treatment of premature beats had significant difference between the two groups(P<0.05).Conduction block,dilatation of heart,the total effective rates in the treatment of heart failure were of no significant difference between the two groups(P>0.05). The total effective rate was 97.5%(39/40)in observation group,significantly higher than that of control group 72.5%(29/40)(P<0.05).
    Conclusion
    Creatine phosphate has significant effect in the treatment of pediatric CVM.

    Objective
    To study the clinical effect of phosphocreatine and fructose diphosphate for children with myocarditis.
    Methods
    A total of 82 children with Myocarditis treated from February 2015 to April 2016 in our hospital were selected.The subjects were randomly divided into two groups,41 cases in each group.The observation group took fructose diphosphate;the control group took phosphocreatine;the treatment lasted for 7-10 d as a course.The clinical effect of the two groups was compared,and level of cTnI,cTnT,LDHI-1 and CK-MB was also observed.
    Results
    The total effective rate of observation group (92.68%,38/41) was higher than that of control group(65.85%,27/41)(P<0.05).The level of cTnI,cTnT and LDH-1 had no significant difference before treatment between the two groups.After treatment,these levels of observation group were significantly lower than those of control group (P<0.05),but CK-MB level was not statistically different (P>0.05).
    Conclusion
    Compared with phosphocreatine,fructose diphosphate has a better effect on children with myocarditis,and can improve the children's cardiac function and alleviate inflammation,which is worthy of clinical application.

    Objective
    To analyze the distribution characteristics of Mycoplasma pneumoniae, Chlamydia pneumoniae and Legionella pneumophila in 100 hospitalized children with acute respiratory infection.
    Methods
    A total of 100 children with acute respiratory infection admitted to Respiratory Department of Hunan Children's Hospital from January 2014 to January 2016 were selected as the research objects. The detection situation of Mycoplasma pneumoniae, Chlamydia pneumoniae, Legionella was summarized in different genders, different years of age and different seasons.
    Results
    Among the 100 children with acute respiratory tract infection, there was 35 cases of Mycoplasma pneumoniae, 22 cases of Chlamydia pneumoniae and 10 cases of Leginella pneumophila, and there were 33 cases of other pathogens. There was no statistical difference in the detection rate of Mycoplasma pneumoniae and Chlamydia pneumoniae between different genders(P>0.05). The defection rate of Chlamydia pneumoniae in boys was significantly lower than in girls, the difference being statistical(P<0.05).There was statistical difference in the age distribution of Mycoplasma pneumoniae(P<0.05), but no difference in Chlamydia pneumoniae or Legionella pneumophila(P>0.05). There was no statistical difference in the season distribution of all the three pathogens(P>0.05).
    Conclusion
    Mycoplasma pneumoniae, Chlamydia pneumoniae and Legionella pneumophila are important pathogens of acute respiratory tract infection, which should arouse clinical attention.

    Objective
    To analyze and compare the expression level of interleukin-6(IL-6) and tumor necrosis factor α(TNF-α) in the serum of children with cerebral palsy at different ages.
    Methods
    A total of 50 children with cerebral palsy were chosen as the observation group, who were hospitalized for treatment in the First Affiliated Hospital of Xinxiang Medical College from Oct. 2014 to Aug. 2015. They were divided into two groups according to age: small-age observation group(3 to 12 months) and older-age observation group(13 to 36 months), with 25 children in each. Another 30 normal-health children were selected as the control group, and they were divided into two groups according to age: small-age control group and older-age control group, with 15 children in each. The peripheral venous blood was taken, and the IL-6 and TNF-α level was determined by enzyme-linked immunosorbent assay.
    Results
    The IL-6 level and TNF-α level were respectively (6.53±1.44)ng/L, (6.42±1.34)ng/L, (398.22±92.07)ng/L and (390.8±93.57)ng/L in the small-age and older-age observation groups, all being higher than the control groups, which were (4.87±1.42)ng/L,(4.57±1.48)ng/L, (268.46±87.87)ng/L and (233.52±71.78)ng/L, and there was statistical difference(P<0.05).There was no statistical difference in the IL-6 and TNF-α level between different ages in the observation groups or in the control groups(P>0.05).
    Conclusion
    IL-6 and TNF-α exist persistently in the serum of children with cerebral palsy, which aggravates the brain damage; besides, the level of IL-6 and TNF-α won't decrease with increase of age. Therefore, intervention treatment should be performed as early as possible, which can effectively reduce the influence of inflammatory factor on brain tissues.

    Objective
    To provide reference for clinical practice and improve understanding about brucellosis in children by reporting the clinical data of 5 patients diagnosed with brucellosis in non-epidemic area,and analyzing the characteristics of the disease and the diagnostic and therapeutic process in order to reduce misdiagnosis or missed diagnosis.
    Methods
    Five confirmed cases of brucellosis treated in Luoyang Central Hospital between Jan. 2013 and Dec. 2015 were retrospectively analyzed,including their epidemiology,clinical manifestations,laboratory test results,diagnostic process and prognosis.
    Results
    Five cases consisted of three boys and two girls,3 preschool children and 2 school-aged children,all coming from local rural areas and had a history of contacting sheep or sheep-related products. The most common manifestation was fever which occurred in all cases. Fever process was between 10 to 45 days.Except for 1 case whose fever type was undulant fever,all the rest had irregular fever.Joint pain existed in 1 case,profuse sweating in 1 case,hepatosplenomegaly in 2 cases, impaired liver function in 3 cases.The brucella agglutination test was positive in 5 cases.The blood culture was positive in 1 case of all cases.C-reactive protein was increased in 2 cases (19.1,15 mg/L).ESR was increased in 2 cases (34,25 mm/h). There was one case of arthritis.
    Conclusion
    The incidence of brucellosis is low in children,which is usually presented with various clinical manifestations and lack of specificity in laboratory examination.For children with fever of unknown origin which does not respond to general antibiotic treatment this disease should be considered. Pay special attention to epidemiology history and relevant examination of brucellosis,which is helpful to the early diagnosis.

    Efficacy and safety of acitretin A in the treatment of children with generalized pustular psoriasis

    Objective
    To investigate the efficacy and safety of acitretin A in the treatment of children with generalized pustular psoriasis.
    Methods
    From January 2011 to January 2015, 12 children with pustular psoriasis were admitted to Hunan Children's Hospital were enrolled. All children were given acitretin A orally, at the initial dose of 0.4-0.8 mg / (kg·d), once daily. The same dose was maintained during the following two weeks of treatment, after which, the treatment effect was evaluated. If it was ineffective, the dose was increased according to the children's condition and the treatment was continued. If the skin lesion of children disappeared more than 90%, the dose was gradually reduced until it was stopped. The effect was assessed based on the skin lesion area and PASI scores before and after treatment; the GPT, albumin, triglyceride, of Ca2+and K+ were determined; the adverse reactions were observed and the relapse in children were analyzed.
    Results
    Eleven cases were cured and the treatment was markedly effective in 1 case, the total effective rate being 100.0%. PASI score was (1.5±1.4) points after treatment, which was significantly lower than that before treatment (7.8±2.1), the difference being statistically significant (P<0.05). After treatment the level of alanine aminotransferase and serum albumin was significantly higher than before treatment, while triglyceride level was significantly lower than before treatment, and the difference was statistically significant (P<0.05). There was no significant difference in the levels of Ca2+ and K+ of children before and after treatment (P>0.05). There were 6 cases of pruritus, 2 cases of mucous membrane dryness, 1 case of slight dyslipidemia, 3 cases of elevated liver enzymes, and 3 cases of embolism increased; the adverse reactions were mild in all the 12 cases; there was no significantly abnormal liver and kidney function, and the adverse reactions disappeared after symptomatic treatment (diet conditioning, changing the dosage, etc.), which did not affect the medication process. Children were followed up for 1 to 2 years, and a total of 5 cases of children relapsed, the recurrence rate being 41.7%.
    Conclusion
    Acitretin A has higher efficacy and safety in the treatment of children with generalized pustular psoriasis.

    Objective
    To observe the clinical efficacy of the treatment for pediatric facial paralysis by facial three-needle acupuncture combined with routine western medical therapy.
    Methods
    Totally 33 children with peripheral facial paralysis were observed. The routine western medical therapy was used in acute stage (within 7 days of onset),and facial three-needle acupuncture combined with routine western medical therapy was applied during quiescent and recovery stages (from 8 days to 20 days),once a day,  five times a week,7 days as a course for 8 courses.The curative effectiveness was assessed before acupuncture treatment (7 days after onset) and after acupuncture treatment (2 months or any time within 2 months on recovery) by the same physician.
    Results
    The total effective rate of the three-needle therapy was 96.97%(32/33),significantly higher than that before acupuncture (78.79%,26/32),the difference being statistically significant (Z=-6.707,P<0.05).
    Conclusion
    The facial three-needle acupuncture combined with routine western medical therapy has significant effect in treating pediatric peripheral facial paralysis.

    Objective
    To investigate the effect of Azithromycin combined with glucocorticoid in children with severe/refractory Mycoplasma pneumonia.
    Methods
    A total of 102 children with severe/refractory Mycoplasma pneumonia treated in our hospital were selected as the research object,and were divided into the control group and the research group randomly,51 cases in each group. The control group was treated with Azithromycin intravenous drip,and the research group was treated with Azithromycin and methylprednisolone sodium succinate. The clinical symptoms and signs, the disappearance time, the effective and the adverse rates of the two groups were observed during the treatment.
    Results
    The symptoms disappearance time,hospitalization time and the incidence of adverse reactions of the study group were significantly lower than those in the control group(P<0.05). The total effective rate of the study group(96.1%) was significantly higher than that of the control group(76.5%,P<0.05).
    Conclusion
    Azithromycin combined with glucocorticoid can improve the clinical symptoms and signs of patients with Mycoplasma pneumonia, shorten hospital stay,improve the curative effect and reduce the incidence of adverse reactions.

    Objective
    To investigate the clinical features of Langerhans cell histiocytosis(LCH) in children.
    Methods
    The clinical data of 68 LCH children admitted to Department of Hematology, Hunan Children's Hospital from January 2010 to December 2015 were selected. According to medical history, physical examination, blood routine, liver and kidney function tests, imaging examination and bone marrow cell morphological examination, organ function was evaluated; all cases were divided into three groups. There were 32 cases in group Ⅰ (multi-system high risk), 20 cases in group Ⅱ (multi-system low-risk), and 16 cases in group Ⅲ (single system with multiple bone lesions and limited special parts involved). Totally 65 cases of confirmed LCH children were given LCH-Ⅲ treatment. Three cases of LCH were not given without chemotherapy after diagnosis, among which 1 was neonatal LCH with skin involved, and 2 cases were single lesion of clavicle and treated by focal curettage. Analyze its clinical features, treatment and follow-up results.
    Results
    In the 68 cases of LCH rash, bone damage, hepatosplenomegaly and other symptoms were more common. Sixty-five patients received chemotherapy, and the total effective rate was 78.46% (51/65) at 6 weeks, and 83.08% (54/65) at 12 months. The follow-up lasted until December 2016, and the cure rate was 58.06% in group Ⅰ,60.00% in group Ⅱ, and 71.43% in group Ⅲ.
    Conclusion
    LCH children have different clinical manifestations, lesions are widely distributed and are often accompanied by multiple organ dysfunction; therefore, grouped treatment according to clinical characteristics can improve their prognosis, but for those with multiple organ dysfunction, the efficacy needs to be improved.

    Objective
    To study the effect of evidence-based nursing on the development and rehabilitation of neonates with hypoxic ischemic encephalopathy.
    Methods
    A total of 220 children were included as the research subjects,who were treated in Hunan Children's Hospital from Jan. 2014 to Jan. 2016,and they were randomly divided into basic group(110 children) and evidence-based group(110 children).Basic nursing was carried out in the basic group,while for the evidence-based group,the evidence-based problem was made clear first and the evidence-based nursing scheme was designed,and then the evidence-based nursing was carried out,including disease condition monitoring,education about the disease and rehabilitation training. 〖JP2〗Evidence-based evaluation was performed after the nursing. Observe the treatment results and the incidence of pneumonia in the two groups;one-year follow-up was carried out for the two groups of children,and compare the intelligence development,motor development,and Gesell score of cerebral palsy rehabilitation.〖JP〗
    Results
    The total effective rate of evidence-based group was 90.9%(100/110),significantly higher than that of basic group(76.4%,84/110),and there was statistical difference(P<0.05).The incidence rate of pneumonia was 4.5%(5/110)in the evidence-based group,which was significantly lower than that in basic group(25.5%,28/110),the difference being statistical(P<0.05).After one year,the intelligence development,motor development and Gesell score were significantly higher in evidence-based group than in basic group,and the difference is of statistical significance(P<0.05).
    Conclusion
    The evidence-based nursing can effectively improve the development and rehabilitation of the neonates with hypoxic ischemic encephalopathy.

    Objective
    To explore the influence of family rehabilitation guidance on the self-care living ability of children with cerebral palsy.
    Methods
    Totally 46 children were included as the subjects,who were treated in Shenyang Children's Hospital from Jan. 2015 to Jan. 2016,and they were randomly divided into two groups:observation group (23 cases) and control group (23 cases).Both groups were given conventional nursing as well as dieting guidance,safety guidance and basic nursing. In addition,family-based rehabilitation training was carried out. After the training in the rehabilitation training room,individualized rehabilitation nursing plan was made by the special nurses and experienced nurses based on the children's condition,and the parents were educated about how to help the children develop the self-care living ability,which lasted for 6 months. Observe the ADL scores before and after rehabilitation.
    Results
    There was no statistical difference in ADL scores in control group between before and after treatment(P>0.05).ADL scores were significantly higher after treatment than before treatment in observation group,the difference being statistical(P<0.05).After 6 months of treatment,the scores in observation group were significantly higher than those in control group,and the difference was of statistical significance(P<0.01).
    Conclusion
    Family rehabilitation guidance can improve the motor function and the self-care living ability of children with cerebral palsy,which results in less dependence on their family members.

    Objective
    To investigate the curative effect of Ommaya reservoir drainage on reducing intracranial pressure in children with hydrocephalus.
    Methods
    From January 2015 to June 2016,50 children underwent Ommaya capsule implantation in Department of Neurosurgery,Hunan Children's Hospital,and they were enrolled in this study. Before the treatment,children and their families were mentally guided. After treatment observe the changes of the patient's condition,check the drainage status of the Ommaya sac draining device,and regularly check the intracranial pressure in children and other physiological indexes. After the treatment incision care and regular examination of children's body temperature,cerebrospinal fluid color and blood was performed. Low intracranial pressure due to excessive drainage should be prevented.
    Results
    Complications after Ommaya capsule implantation happened in 11 cases,including 3 cases of secondary intracranial infection,2 cases of drainage tube obstruction,1 case of secondary intraventricular hemorrhage and 1 case of rejection. Low intracranial pressure caused by excessive drainage occurred in 4 cases,while the remained children were with intracranial pressure reducing to normal levels,the complication rate being 22.0%. After targeted treatment,the symptoms of complications were effectively improved,and there was no recurrence.
    Conclusion
    Ommaya capsule implantation for children with hydrocephalus can reduce intracranial pressure,and the rate of complications is low,which is worthy of clinical use.

    Objective
    To observe the effect of nursing intervention with 10% chloral hydrate taken orally in children requiring medical sedation.
    Methods
    A total of 324 children admitted to Hunan Children's Hospital from January to December 2015 undergoing medical examination and requiring sedation were randomly divided into control group and observation group with 162 cases in each. The control group was sedated with 10% chloral hydrate,and routine nursing was performed. The observation group was intervened in addition to the routine nursing for the control group,such as the missionary health knowledge,psychological intervention and diet intervention. The duty nurse is responsible for guiding children to take oral chloral hydrate. Analyze and compare the sedative effect,sedation onset time,the incidence of adverse reactions and re-administration,and the satisfaction rate of parents between the two groups.
    Results
    The effective rate of sedation in the observation group was 84.57% (137/162),which was significantly higher than that in the control group (58.02%,94/162) (P<0.01). The proportion of sedation onset time at 5 ~ 15 min in observation group was significantly higher than that in control group,while the proportion of the onset time of sedation >30 min was significantly lower than that in control group (P<0.01). The incidence of adverse reactions and the rate of re-administration in the observation group were significantly lower than those in the control group,with significant differences (P<0.01). The satisfaction rate of the family members in the observation group with sedation nursing intervention was significantly better than that in the control group,the difference being statistically significant (P<0.01).
    Conclusion
    Effective nursing intervention can significantly improve the efficiency of oral administration of 10% chloral hydrate,reduce the onset time of sedation,and reduce adverse reactions. The satisfaction rate of family members is high,so it is worth applying widely clinically.

    Professor Han Xinmin has much experience in treating Tic disorders in children,He always advocates that differentiation of TCM should be combined with western medicine classification of diseases. He thinks that wind-phlegm syndrome is common in early stage of the illness and liver-kidney yin-deficiency syndrome in the late stage. Acupuncture should be combined with medicine and syndrome differentiation with symptom differentiation. The use of prescriptions and drugs is flexible and various.

    Professor Wang Lie's experience in the treatment of children with deficiency of body fluid

    The body fluid in children has the features of much demand, rapid metablism and being more likely to be disordered. Through the clinical treatment and analysis of the children with symptoms of body fluid deficiency, we differentiate them into three types: heat damaging fluid, inner-heat exhausting fluid and loss of fluid, which is a respectively treated by clearing, moistening and tonifying. The prescriptions, including Baihu decoction, Zengye decoction and Shengmai powder, are chosen for treatment, in order to regulate the body. This paper primarily summarized the principles of TCM treatment for deficiency of body fluid in children, which filled in the gaps in liquid therapy of modern medicine. In addition, for the diarrhea damaging fluid, the Li and Se therapy also achieves good clinical effect; however, further studies are needed to prove the repeatability of the treatment principles.

    Nose-bleeding is a common disease of nose in school-age children,which is mainly characterized by the bleeding of nose,which may be in one side or both sides. The traditional Chinese medical books of the past and present refer to it as "blood trouble".Prof. Wang Lie thinks that it is mainly heat syndrome and solid syndrome,which is located in nose,and is related to lung,stomach,spleen and liver. The pathogenesis is the excessive heat of lung,so the treatment should focus on clearing lung and cooling blood,hence the development of Liangxue Zhinv decoction,which is modified in application according to the disease status of the children. This decoction is based on the children's physical characteristics,and is simple in composition,which is effective in clinical application and achieves great effect in decreasing the relapse rate of nose-bleeding.
