    Talking about Shengjing school of children's acupuncture and massage

    The Shengjing school of children's acupuncture and massage develops and expands continuously with the mode of combining college education with teachertraining education. The school has its unique academic characteristics, advocating massage as medication. The determination of recipe depends on the treatment principle, reducing the number of acupuncture points and cooperating with acupoint selection to increase curative effect. At the same time, the core specific acupoints, representative methods and important acupoints of different diseases were summarized. This school also attaches great importance to children's health care, and according to the physiological and pathological characteristics of children and the difference in solar terms, 24solarterm health care and viscera health massage methods have been developed. The six methods of the back and spine and the acupuncture method through acupoints are the special techniques of the school. At the same time, it is good at using acupuncture and massage to treat children's cerebral palsy and other difficult diseases. The school is also somewhat regional, and enjoys a high reputation in Shenyang. The academic thought and techniques of the school have been spread all over the country through being invited to give lectures and holding training courses of acupuncture and massage.

    Literature research on the treatment principles of Shanghai Xu's pediatrics in the treatment of childhood pneumonia

    Objective
    To summarize the academic thoughts and the evolution of Shanghai Xu's pediatrics in the treatment of childhood pneumonia by way of literature research.
    Methods
    The literatures on Shanghai Xu's pediatrics in the treatment of childhood pneumonia were searched by computer in the databases from the inception to December 2018. According to the inclusion and exclusion criteria, these literatures were screened, and those about treatment principles and academic thoughts were summarized and analyzed.
    Results
    A total of 80 literatures were included, among which 44 were about academic experience, more than those about clinical and basic researches, and the literatures were classified into 3 types: clear method (25), combination of clearing and warming method(22) and warming method(15). In the literatures about academic experience, the method of clearing lung and reducing phlegm appeared often, followed by the method of warming Yang to relieve heat, warming lung to reduce watery phlegm, and the combination of warming and clearing. In total, the warming method appeared 37 times, clearing method 25, combination 8, and others 6. Clinically a total of 863 patients were treated by clearing lung with cooling drugs, more than the method of clearing lung to reduce phlegm and removing stasis(535) to reduce phlegm(330).
    Conclusion
    The clearing methods of clearing lung with cooling drugs and clearing lung to reduce phlegm are often used in Xu's pediatrics in the treatment of childhood pneumonia. The warming method is its academic features, including warming Yang to relieve heat, warming lung to reduce watery phlegm and warming Yang to induce resuscitation. The people of following generation have also established the treatment principles of food therapy, expelling the pathogenic wind, relieving heat and clearing lung. In the future, the treatment principles can be further studied on the basis of basic experiments.

    Study on the promotion way of the Three-Character-Classic-school massage

    Objective
    To screen for the best training program of Three-Character-Classic-school massage for children, in order to have better promotion and application.
    Methods
    The training was divided into 3 types: short-term(2-3 days), medium-term(2 weeks) and long-term(3-6 months). Take the childhood diarrhea as an example. A total of 660 children with diarrhea were included according to the inclusion and exclusion criteria, who were treated in Qingdao TCM Hospital and 5 other hospitals form October 2009 to June 2015. The type of diarrhea were: improper diet, spleen insufficiency, wind and cold, Yanginsufficiency of spleen and kidney, and wetness and heat. The trainees were tested at the first term(302 cases) after two weeks of training; the trainees were tested at the second term(240 cases) after 3 months of training; the trainees were tested at the third term(118 cases) after 6 months of training. Observe the clinical effect and the combination use of drugs after 3 days of massage treatment.
    Results
    (1) In the treatment by clinical physicians who were trained according to different training programs, the clinical total effective rate of the first, second and third term was 94.0%(284/302), 95.0%(228/240) and 96.6%(114/118), respectively; the incidence rate of adverse reactions were 0. (2) The rate of combination use of drugs was 32.87%(99/302) in the trainees with 2 weeks of training, 6.67%(16/240) in those with 3 months, and 4.24%(5/118) in those with 6 months.
    Conclusion
    The Three-Character-Classic-school massage treatment is significantly effective and safe in the clinical management of childhood diarrhea, which can be learned in 2 weeks. Long-term training can reduce the rate of combination use of drugs. It is easy to learn and suitable to be promoted.

    Experimental study on the influence of placenta-derived mesenchymal stem cells on the secretion of IL-4 and IFN-γ in T lymphocytes

    Objective
    To observe the influence of placenta-derived mesenchymal stem cells on the secretion of IL-4 and IFN-γ in activated T lymphocytes, and to predict the possibility of applying the placenta-derived mesenchymal stem cells in the clinical treatment of various diseases.
    Methods
    T lymphocytes were isolated from two healthy peoples to make MLC system. The placenta-derived mesenchymal stem cells were obtained, which were co-cultured with MLC system. They were divided into four groups: homologous T cells group Ⅰ, homologous T cell group Ⅱ, co-culture group and positive control group, and each group was inoculated with 10 holes. The culture lasted for 72h. ELISA was used to detect the content of IL-4 and IFN-γ in the cell culture supernatant of each group.
    Results
    There was no significant difference in IL-4 and IFN-γ in the supernatant between homologous groupⅠand group Ⅱ(P>0.05). compared with group Ⅰ and group Ⅱ, the concentration of IL-4 and IFN-γ was significantly increased in co-culture group and positive control group(P<0.01). Compared with co-culture group, the IL-4 concentration in positive control group was significantly increased, while IFN-γ concentration was significantly decreased, and the difference was statistically significant(P<0.01).
    Conclusion
    The placenta-derived mesenchymal stem cells can inhibit the ability of activated T lymphocytes to secrete IL-4 and to promote IFN-γ. They may have the ability to promote Th0 to differentiate into Th1.

    Objective
    To observe the Effect of Qingxin Xiepi method on myocardial enzymes and leucocyte classification in children with hand-foot-mouth disease(heart-spleen-Methods
    Thirty children with hand, foot and mouth disease admitted to Guangzhou Hospital of TCM from May 2016 to November 2017 were selected as the observation group. At the same time, 60 healthy children were selected as the control group. Myocardial enzymes and leucocytes were detected before and after treatment in the observation group and in the control group.
    Results
    (1)LDH,α-HBDH, white blood cell count, neutrophil absolute value, neutrophil percentage, neutrophil to lymphocyte ratio(N/L) and monocyte absolute value in the observation group before treatment were higher than those in the control group, while CK, CK-MB and lymphocyte percentage were lower(P<0.05).(2) After treatment, the levels of LDH, αHBDH, white blood cell count, absolute value of neutrophils, percentage of neutrophils, ratio of neutrophils to lymphocytes(N/L), absolute value of monocytes and percentage of monocytes in the observation group were significantly lower than those before treatment, while the absolute value of lymphocytes and percentage of lymphocytes were significantly higher(P<0.05).(3) The clinical symptoms of fever, oral herpes and hand-foot herpes in the observation group were significantly improved after treatment compared with those before treatment(P<0.05).
    Conclusion
    Myocardial enzymes and leukocyte classification are improved significantly by using the Qingxin Xiepi method in children with hand-foot-mouth disease(heart-spleen-heat type). It provides a theoretical basis for the efficacy of the prescription.

    Clinical observation on treatment for nephrotic syndrome in children with Guben Quyu Huashi decoction

    Objective
    To observe the therapeutic effect of Guben Quyu Huashi decoction on pediatric nephrotic syndrome.
    Methods
    Fifty cases of children with nephrotic syndrome admitted to the Department of Pediatrics of Wuwei Liangzhou Hospital from June 2016 to June 2018 were selected as study subjects, and were randomly divided into control group (20 cases) and observation group (30 cases).Both groups were given routine treatment, and the observation group was also treated with Guben Quyu Huashi decoction for syndrome differentiation and modified treatment one dose per day and 12 weeks as one course of treatment. After one course of treatment, the clinical efficacy, biochemical indexes, urine protein conversion time and edema regression time of the two groups after treatment were observed.
    Results
    After treatment, plasma albumin was significantly higher in the two groups than before treatment, and urine protein and cholesterol were significantly lower in the 24h group than before treatment, with statistically significant differences (P<0.05).After treatment, plasma albumin in the observation group was significantly higher than that in the control group, and urine protein quantification and cholesterol in 24 hours were significantly lower than those in the control group, with statistically significant differences (P<0.05).After treatment, the plasma immunoglobulin of the observation group was significantly higher than that of the control group, and the duration of urine protein conversion to Yin and edema regression was significantly less than that of the control group, with statistically significant differences (P<0.05).The total effective rate in the observation group was higher than that in the control group[96.6%(29/30) vs 70.0%(14/20)], and the difference was statistically significant(P<0.05).After treatment, the incidence of full moon face, infection, recurrence, gastrointestinal reaction and hyperglycemia in the observation group was lower than that in the control group, with statistically significant differences(P<0.05).There was no significant difference in the incidence of acne and hypotension between the two groups(P>0.05).
    Conclusion
    Guben Quyu Huashi decoction can significantly improve the effect of treatment for nephrotic syndrome, rapidly improve the clinical symptoms, with quick effect and less adverse reactions, which is worthy of clinical application.

    Analysis of correlation between the condition of children with sepsis and vascular endothelial dysfunction

    Objective
    To investigate the relationship between the condition of children with sepsis and vascular endothelial cell dysfunction.
    Methods
    A total of 68 children with sepsis, who were admitted to the PICU of Guizhou People's Hospital from January 2017 to January 2018 and stayed in the Pediatric Surgery for more than 24 hours ,were selected as subjects. According to the severity of the disease, 41 patients were included in sepsis group and 27 patients in septic shock group. At the same time, 39 outpatients with healthy physical examination were selected as the control group. According to the prognosis of the children, there were 53 patients in the survival group and 15 patients in the death group. Detect the levels of inflammation indicators [white blood cell count(WBC), C-reactive protein(CRP), interleukin-6(IL-6), procalcitonin(PCT)] and endothelial cell injury markers[plasma endothelin 1(ET1),von Willebrand factor(vWF) and soluble thrombomodulin(sTM)]. The correlation between inflammation markers and endothelial injury markers was analyzed.
    Results
    (1) The levels of CRP, IL-6, PCT, vWF, sTM and ET-1 gradually increased with the increasing severity of sepsis, and the differences between the two groups were statistically significant(P<0.05).(2) There was significant difference between the death group and the survival group(P<0.05). The WBC was significantly lower in the death group than in the survival group, and it was positively correlated with the prognosis of children with sepsis. The vWF, sTM and ET-1 were significantly higher in the death group than in the survival group, and were significantly negatively correlated with the prognosis of children with sepsis.
    ConclusionThe condition and prognosis of children with sepsis are closely related to vascular endothelial injury.

    Diagnosis and treatment of neonates with early-onset Listeria sepsis

    Objective
    To study the clinical characteristics of Listeria infection in the newborn and to summarize the experience in diagnosis and treatment.
    Methods
    The clinical data of 11 children treated in the Children's Hospital of Zhengzhou University from December 2012 to December 2017 was analyzed, who were confirmed by blood culture to be with monocyte Listeria sepsis.
    Results
    (1) General data: there are 7 boys and 4 girls; 3 of them were delivered naturally and 8 by cesarean section; 5 were premature infants, whose mothers were all with high risk factors of one or more infection during perinatal period, and two mothers were also proved to have monocyte Listeria sepsis by blood culture; all the 11 children had their first onset within 7 days, and 5 within 48h. (2)Clinical manifestations: the main manifestations were fever(in 7 children), shortness of breath(5), groaning(5), cyanosis(7), skin rash(3) and secondary septic shock(5)(1 complicated with acute respiratory distress syndrome and 3 with disseminated intravascular coagulation). (3)Lab tests: the routine blood test showed that 8 children had increased WBC[(21.4~46)×109/L], 2 were almost normal and one had decreased WBC(2.78×109/L);platelets were significantly lower in 3 children[(7~20)×109/L];all had increased Creactive protein(>100 mg/L); 6 children were complicated with intracranial infection, and the blood culture showed it was Listeria monocytogenes, of whom the conventional blood culture was performed in the first 4 cases for 3 to 5 days, and for the other 7 with positive report; MALDITOF MS was used for 2 to 3 days; the culture of cerebrospinal fluid showed 4 positive cases, for treatment, ampicillin alone was used in one case, meropenem combined with vancomycin was used in 6 cases, and meropenem with ampicillin in 4 cases; for prognosis and followup: 9 children were cured with good growth and development, 1 asked to be discharged and was lost to follow up, and one was dead.
    ConclusionListeria infection is a severe infectious disease in the early stage of a newborn and is commonly with an early onset. There are many complications and the mortality is high. The mothers of these neonates are often with high risk factors. Microbiological examination should be performed as early as possible to clarify the diagnosis. Attention should be paid to the possibility of Listeria infection when choosing the antibiotics in order to reduce the mortality and improve the prognosis.

    Obstructive sleep apnea(OSA) is a common sleep disordered breathing disease in children. Adenotonsillectomy is the firstline treatment for pediatric OSA, but many children are reported to have persistent symptomatic or polysomnographic evidence of OSA after surgeries. Drug-induced sleep endoscopy (DISE) is an examination technique that has been gradually noticed and has significantly developed in recent years. It can allow clinicians to identify areas of collapse in the upper airway in the state of similar physiological sleep, and develop individualized treatment plans for children and improve success rates of surgical intervention. At present, there have been numerous controversies about surrounded this technique. This article aims to provide a systematic review of its recent development for further research.

    In recent years, the proportion of children with tic disorders in China and abroad has increased year by year, and the public awareness of such diseases has gradually increased.This article summarizes recent research progress on tic disorders from two aspects: Western medicine research and TCM research.At present, there are many TCM treatment methods in treating tic disorders and they are effective .The traditional Chinese medicine treatment is safe and effective, flexible and diverse, with less toxic and side effects, less pain and higher compliance, so it's more suitable for children.

    Analysis of the etiology of Henoch-Schonlein purpura

    Henoch-Schonlein purpura is a leukocytic vasculitis involving capillaries and small arteries, often accompanied by abdominal pain, joint pain and kidney damage. It is more common happens in children. The etiology of Henoch-Schonlein purpura is not clear,and it may be related to many factors such as infections, genetics, foods, drugs and intestinal micro-ecological imbalance, etc. In addition, the onset of atopic dermatitis can also cause Henoch-Schonlein purpura. The disease is often recurrent and has many complications. Early diagnosis and treatment are beneficial to recovery. This article reviews the latest progress on the etiology of Henoch-Schonlein purpura.

    Neonatal jaundice, also known as "fetal yellow" in traditional Chinese medicine, is a common disease in neonatal period. According to TCM theory, the key to its etiology is dampness, and the main organs involved are liver, gallbladder, spleen and stomach, while lung and large intestines are usually not involved. Based on TCM theory, lungs helps to regulate the human body, and the flow blood and fluid can be smooth by the regulation of lung; in the large intestine, water is absorbed again and the food waste is dealt with. Through the combination of lung and large intestine, the fluid metabolism of the human body is regulated; therefore, they are closely related to the onset and treatment of jaundice. Based on the theory of "Lung and large intestine unification", the relationship between the theory and the diagnosis and treatment of neonatal jaundice was studied in this paper, aiming to enrich the methods of TCM treatment for "fetal yellow" and to help the clinical practice.

    Secretory otitis media is more common in preschool children, and it is latent and repetitive. If it is not cured after a long time of treatment, it may result in reduced hearing ability, and even leads to disorders in speech development, cognition and balance function. The key to the treatment of the disease is to identify it early and to perform early diagnosis and treatment. This article deals with its characteristics through reviewing the epidemiology data in China and abroad and socio-economic burdens; meanwhile, based on the general information of the elementary hospitals in China, the effective methods of early screening for secretory otitis media in children are studied.

    Professor Hou Jianghong has been dealting with pediatric diseases with the idea of "preventing diseases before they occur", which is manifested by the following: focusing on regulation before getting ill, on prevention at the point of getting ill, on control when being ill, and on preventing recurrence after recovery. Prof. Hou pays more attention to prevention than to treatment in dealing with children with rhinitis, stagnation, wind-warm syndrome and common cold with convulsion, who thinks that prevention is the most important thing and it should be combined with protection. Prof. Hou sticks to the principle of "regulating spleen and stomach" and "being careful in maintenance" from the beginning to the end. During the treatment, the aim of getting a vigorous spleen and keeping from diseases all year round is achieved by strengthening spleen to enhance lung function, invigorating the spleen to strength the body's resistance, supporting spleen to prevent liver invasion and nourishing spleen to promote growth and development of the human body.

    Clinical study on Wenyang Bushen method in the treatment of primary refractory nephrotic syndrome in children

    Objective
    To discuss the clinical study on Wenyang Bushen method in the treatment of children with primary refractory nephrotic syndrome(RNS).
    Methods
    Totally 68 child patients with RNS treated in Zhengzhou Children's Hospital between January 2015 and December 2016 were selected and randomly divided into observation group and control group, with 34 cases in each group. The two groups were given routine symptomatic treatment. Both groups were given corticotropin+prednisone, and observation group was additionally treated by Wenyang Bushen method. The treatment lasted for 24 weeks. The clinical efficacy, TCM symptom scores, serum biochemical indicators (triglyceride, cholesterol, urea nitrogen, serum creatinine, cortisol), T lymphocytes levels(CD3+, CD4+, CD8+, CD4+/CD8+), adverse reactions and recurrence at 1 year of followup were observed in the two groups.
    Results
    The total effective rate in observation group after treatment was significantly higher than that in control group [88.24% (30/34) vs 67.65% (23/34)] (P<0.05). After treatment, the scores of puffy face and swollen limbs, difficult urination, shortness of breath and fatigue and poor appetite in the two groups were significantly lower than those before treatment, and the scores in observation group were significantly lower than those in control group (P<0.05). The levels of serum triglyceride, cholesterol, serum urea nitrogen and serum creatinine after treatment in the two groups were significantly lower than those before treatment, and the levels in observation group were significantly lower than those in control group (P<0.05). The serum cortisol level after treatment in the two groups was significantly higher than that before treatment, and the level in observation group was significantly higher than that in control group(P<0.05). After treatment, the CD3+ and CD4+ and CD4+/CD8+ in the two groups were significantly higher than those before treatment, and the indexes in observation group were significantly higher than those in control group (P<0.05). After treatment, the CD8+ in the two groups was significantly lower than that before treatment, and the index in observation group was significantly lower than that in control group (P<0.05). There was no significant difference in the incidence rate of adverse reactions between the two groups (P>0.05). The average frequency of recurrence within 1 year in observation group was significantly less than that in control group (P<0.05), and the recurrence rate was significantly lower than that in control group (P<0.05).
    Conclusion
    Wenyang Bushen method combined with corticotropin has significant efficacy on children with RNS, and it can significantly improve the related symptoms, effectively improve lipid metabolism disorders, reduce kidney damage, regulate immune system function, and reduce the risk of recurrence, so it can be safely applied in clinical practice.

    Effect of vitamin A combined with iron on iron-deficiency anemia in preschool children

    Objective
    To evaluate the clinical efficacy of vitamin A combined with iron in the treatment of iron-deficiency anemia in preschool children.
    Methods
    Ninety-two children were randomly divided into treatment group and control group, 46 cases in each group. The two groups were given ferrous succinate tablets orally, 5 mg/(kg·d), three times a day, for 30 consecutive days as a course of treatment. The treatment group was also given vitamin A capsules, 1 capsule/time, once a day, for 7 consecutive days, which was stopped for 3 days, and then resumed in the same pattern, within 30 days. The clinical efficacy, hemoglobin, serum ferritin, serum iron content and total iron binding capacity of the two groups were observed and compared before and after treatment.
    Results
    The hemoglobin, serum ferritin, serum iron content and total iron binding capacity in the treatment group were higher than those in the control group(P<0.05), and the curative effect was better than that in the control group(100.0% vs 80.4%)(P<0.05).
    Conclusion
    Vitamin A combined with iron is more effective than iron alone in the treatment of iron deficiency anemia in preschool children.

    Objective
    To study the effect of modified Wuwu decoction of Huangqi and Guizhi combined with acupuncture and massage on the speech recovery of children with spastic cerebral palsy.
    Methods
    A total of 120 children with spastic cerebral palsy, who were admitted to Xiangyang First People's Hospital Affiliated to Hubei Medical College between June 2016 to June 2017, were chosen as the subjects, and they were randomly divided into two groups, the observation group and control group,60 in each group. Both groups were given massage, acupuncture and speech training. In addition, the observation group was treated with modified Wuwu decoction of Huangqi and Guizhi. The treatment lasted for 3 months. The treatment effect was observed, and the two groups of children were evaluated on the ability to respond to people, the ability to respond to articles, the GCS and ADL.
    Results
    The total effective rate of the observation group was 96.7%(58/60), significantly higher than the control group(81.7%,49/60)(P<0.05).After 3 months and 6 months of treatment, the score in the ability to respond to people and articles was both higher than that before treatment, and the observation group was better than the control group(P<0.05). The GCS score and ADL score were both higher after treatment(P<0.05), and it was higher in observation group than in control group(P<0.05).
    ConclusionThe modified Wuwu decoction of Huangqi and Guizhi combined with acupuncture, massage and speech training can significantly improve the speech recovery in children with spastic cerebral palsy. The children's ability to respond to people and articles is greatly increased, and the speech recovery is faster and better, which has better clinical effect and is worthy of clinical application.

    Objective
    To observe the clinical effect of needle-embedding therapy on allergic rhinitis in children based on collateral disease theory.
    Methods
    A total of 60 patients who met the standards were selected from the outpatient department of our hospital from January 2017 to June 2018, and were randomly divided into two groups:30 patients in the treatment group and 30 patients in the control group. Patients in the control group were treated with kirritan syrup orally, and patients in the treatment group were treated with needleembedding therapy on the basis of the treatment for control group; both groups were treated for 14 days. Nasal obstruction examination and nasal symptom scoring were performed at the time of visit and at 14 days of treatment, and the above results and curative effects were observed and compared.
    Results
    One patient in the treatment group failed to complete the treatment due to missing the interview, while one patient in the control group quit the treatment due to treatment for myocarditis; the rest of the children could cooperate to complete the treatment. The total effective rate of the treatment group was 93.1%(27/29), which was significantly higher than that of the control group(69.0%,20/29),and there was statistical difference(P<0.05). Nasal symptom score of the two groups was much lower than that before treatment(P<0.05), and it was lower in the treatment group than in the control group(P<0.05).The nasal resistance of the two groups was lower than that before the treatment(P<0.05), but it was more obvious in the treatment group(P<0.05).
    Conclusion
    Based on the theory of collateral disease, needle-embedding therapy has a remarkable effect in the treatment of allergic rhinitis in children, which is worthy of clinical application.

    Objective
    To observe the clinical efficacy of caffeine citrate in very early preterm infants.
    Methods
    From May 2013 to April 2018, 42 cases of very early preterm infants admitted to Zhengzhou First People's Hospital were selected as the subjects, and they were randomly divided into 21 cases in the observation group and 21 cases in the control group. The control group was treated with aminophylline and a ventilator, and the blood concentration of aminophylline was regularly monitored, so that the blood concentration was maintained at 5 to 10 mg/L. The observation group was treated with caffeine citrate and a ventilator. Observe the clinical efficacy of the two groups, the number of reintubation after treatment, the number of apnea after withdrawal, the ventilation time and the adverse reactions.
    Results
    The total effective rate of the observation group after treatment was 90.4%(19/21), which was significantly higher than that of the control group(61.9%,13/21), and the difference was statistically significant(P<0.05). There was statistical difference between the two groups in the number of reintubation and the number of apnea after withdrawal, and the ventilation time(P<0.05). After treatment, the indicators of reintubation on the machine in both groups were lower than before treatment, and the difference was statistically significant(P<0.05). The clinical efficacy of the observation group was better than that of the control group, and the difference was statistically significant(P<0.05). There was no significant difference in the incidence of ADR between the two groups(P<0.05).
    Conclusion
    Citrate coffee for an early clinical efficacy is better than that of aminophylline.

    Objective
    To report the clinical features and the process of diagnosis and treatment of lateonset methylmalonic aciduria complicated with methylenetetrahydrofolate reductase(MTHFR) deficiency in one case, which was suspected to be acute disseminated encephalomyelitis(ADEM).
    Methods
    The clinical and laboratory data of the patient was analyzed.
    Results
    The patient was a boy. When he was 12, he caught a cold, after which he had tiredness, poor spirit and memory, reduced school performance, weakness in lower limbs and memory, reduced school performance, weakness in lower limbs and difficulty in walking. One month later, he received an MRI of head and the whole spinal cord, which showed there was abnormal signal in cerebral cortex, posterior part of left paracele and T8-9 level spinal cord. Routine test of cerebrospinal fluid(CSF), biochemical and etiological test, and antibody detection related to immune encephalitis revealed nothing abnormal, and it was clinically diagnosed as ADEM, which was improved after treatment. When he was 13.5 years old, the symptoms aggravated without any cause. The body had difficulty in walking and felt weak in lower limbs. Reexamination by head MRI indicated slight encephalatrophy. The effect of intermittent treatment was not significant, and the muscular tension of lower limbs was increasing progressively. The detection results were as follows: urinary methylmalonic acid 29.63 mmol/mol creatinine (reference value 0.2-3.6 mmol/mol creatinine), 3-hydroxypropionic acid 6.68 mmol/mol creatinine (reference value 0.0-1.1 mmol/mol creatinine), serum free carnitine 16.17 μmol/L(reference value 20.0-60.0 μmol/L), propionyl carnitine 2.35 μmol/L(reference value 1.00-5.00 μmol/L); the level of serum homocysteine increased significantly(49 μmol/L)(reference value 0.0-15.0 μmol/L); there was heterozygous mutation of C.482G> A and c.626dupT in MMACHA gene, and heterozygous mutation of c. 665C > T in MTHFR gene. Finally, the boy was diagnosed with late-onset methylmalonic aciduria complicated with hyperhomocysteinemia and MTHFR deficiency. He was treated by intramuscular injection of hydroxycobalamin and taking levocarnitine, betaine and folic acid orally, as well as rehabilitation treatment, and got significantly improved after six months.
    Conclusion
    It is the first MMA case with the onset of ADEM, which is complicated with methylmalonic aciduria combined with two inheritary metabolic diseases: hyperhomocysteinemia and MTHFR. Metabolic screening is really necessary for the neuvous system diseases with atypical clinical symptoms.

    Objective
    To explore the effect of multidisciplinary cooperation mode in rehabilitation of children with cerebral palsy complicated with dysphagia.
    Methods
    A total of 100 children with cerebral palsy complicated with dysphagia who were hospitalized in the Department of Rehabilitation Medicine of Liaoning Children's Hospital from April 2017 to April 2018 were selected, and they were randomly divided into observation group and control group with 50 cases in each group by drawing lots. The control group was given swallowing treatment and rehabilitation nursing, which was conventional for children with cerebral palsy, while the observation group was given rehabilitation treatment and nursing by a multi-disciplinary cooperation mode composed of rehabilitation nurses, rehabilitation physicians, nutritionists, therapists, hyperbaric oxygen physicians and psychologists. Before treatment and 8 weeks after treatment, the swallowing function and nutritional status were compared between the two groups.
    Results
    The effect in the observation group was better than that in control group after treatment(P<0.05). The improvement in nutritional status in both groups was better after treatment than before treatment(P<0.05), and it was better in observation group than in control group(P<0.05).
    Conclusion
    Multidisciplinary cooperation mode can effectively improve the swallowing function and nutritional status children with cerebral palsy complicated with dysphagia, which is beneficial to the rehabilitation of children.

    Analysis of death of children under 5 years old at Tiexi District of Shenyang from 2013 to 2017

    Objective
    To investigate the mortality of children under five years of age in the region.
    Methods
    Analyze the mortality of the children under 5 years of age in Shenyang Tiexi District supervised from October first,2012 to September 30,2017.
    Results
    There was no significant difference in neonatal mortality, infant mortality or the mortality of children under 5 years old in Tiexi from 2013 to 2017(P>0.05). Infant deaths in Tiexi District accounted for 64.74%(112/202) of infant deaths and 85.64%(173/202) of deaths of children under 5 years of age from 2013 to 2017. Infant deaths accounted for the largest proportion of deaths in children under 5 years of age. Neonatal death was the largest proportion of infant deaths. The main death factors of neonatal deaths among children under 5 years of age were premature low birth weight, birth asphyxia and congenital abnormality.
    Conclusion
    In order to reduce the death of children under 5 years of age, we should start with health education in many ways, improve the consciousness of health care, strengthen perinatal health care, carry out good pre-pregnancy health care, and set up help fund, so that the children can get effective and timely treatment.

    Objective
    To observe the therapeutic effect of traditional Chinese medicine decoction on children with purpura nephritis of Yin-deficiency and blood-heat type, and to explore its effect on peripheral blood Th17 cells in children with purpura nephritis.
    Methods
    The typical cases of Henoch-Schonlein purpura nephritis were collected and sorted out, and 100 cases of children were eventually identified as the research subjects. They were randomly divided into the observation group(50 cases) and the control group(50 cases). The observation group was treated with traditional Chinese medicine decoction orally, while the control group was treated with routine western medicine. After 4 weeks, the curative effect was evaluated and the changes of Th17 in serum were observed.
    Results
    The clinical total effective rate of the observation group was 82.0%(41/50), significantly higher than that of control group(58.0%,29/50), the difference being statistical significance(P<0.05). There was no statistical difference in the expression of peripheral Th17 cells between the two groups before treatment(P>0.05); however, after treatment it was lower, and it was even lower in observation group than in control group(P<0.05).
    Conclusion
    In the course of treatment of Henoch-Schonlein purpura nephritis, oral administration of traditional Chinese medicine decoction has better effect, which can reduce Th17 cells in children, and can regulate the immune function of children with Henoch-Schonlein purpura nephritis.

    Objective
    To observe the clinical effect of massage for children with herpangina.
    Methods
    A total of 200 children were included as the research subjects, who were treated for herpangina in Fushun Traditional Chinese Medicine Hospital from May 2016 to October 2017, and they were randomly divided into two groups: control group(100) and observation group(100). Both groups were given Xiaoer Niuhuang Qingxin powder, orally taken, and the observation group was also treated with massage to clear fever and relieve heat. The massage was performed once a day, 5 days as a course. The clinical effect, the time for body temperature to return normal, and the time of herpes disappearing were observed in the two groups.
    Results
    The clinical total effective rate was 93.0%(93/100) in the observation group, which was significantly higher than that in the control group(80.0%,80/100), the difference being statistical(P<0.05). The time for body temperature to return to normal and the time of herpes disappearing were both shorter in the observation group(P<0.05).
    Conclusion
    The clinical effect of massage as the adjuvant treatment for children with herpangina is good, which should be promoted clinically.

    Objective
    To observe the clinical effect of massage combined with acupoint hot compress on children with congenital muscular torticollis.
    Methods
    Eightythree children with congenital muscular torticollis in Henan Affiliated Hospital of Henan Massage Vocational College were randomly divided into two groups. The 41 patients in the control group were treated with massage therapy, and 42 patients in the observation group were treated with acupoint hot compress in addition to massage. The clinical efficacy and the diameter of the mass in neck were compared between the two groups.
    Results
    The total effective rate of the observation group was significantly higher than that of the control group[(95.2%,40/42) vs (63.4%,26/41),P<0.05]. The diameter of mass was lower in observation group than in the control group, and the difference was statistically significant(P<0.05).
    Conclusion
    Massage combined with acupoint hot compress can reduce the mass, improve the local muscle contracture state, and restore the neck activity, which benefits the prognosis of children. It is worthy of clinical promotion.

    Objective
    To investigate the clinical characteristics and diagnosis of Gaucher disease.
    Methods
    The clinical features,bone marrow aspiration and gene mutation types of the infant with Gaucher disease Ⅱ were analyzed.
    Results
    The main clinical manifestations of the infant included hepatosplenomegaly, opisthotonos and mental and motion development retardation. Gaucher's cells could be seen in the bone marrow aspirates. GBA mutations were c.1448T>C(p.Leu483Pro) which was confirmed by glucocerebrosidase gene analysis.
    Conclusion
    Main features of Gaucher disease Ⅱ are hepatosplenomegaly with nerve system involvement. Glucocerebrosidase activity and gene analysis are important to the diagnosis of Gaucher disease.
