    Clinical observation of Xiaoji Lipi powder and anorexia plaster combined with conventional treatment for children with anorexia(spleen dysfunction syndrome)

    Objective To observe the efficacy of Xiaoji Lipi powder and anorexia plaster combined with conventional treatment for children with anorexia of spleen dysfunction syndrome, as well as its effects on TCM syndrome scores, trace elements and hemoglobin in children. Methods A total of 86 children with anorexia who visited the Pediatric Outpatient Department of Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from October 2023 to December 2024 were selected as research subjects. They were randomly divided into a control group and an observation group, with 43 cases in each group. Both groups were treated with oral lysine inositol vitamin B12 oral liquid, while the observation group was additionally given Xiaoji Lipi powder and anorexia plaster. Both groups received a 4-week course of treatment. TCM syndrome scores, efficacy, trace elements(Zn, Fe, Ca), hemoglobin and adverse reactions were compared between the two groups. Results The effective rate of the observation group was 95.35%(41/43), which was higher than 81.40%(35/43) of the control group, and there was a statistically significant difference(P<0.05). After treatment, the scores of symptoms, such as poor appetite, reduced food intake, hiccups and belching, abdominal distension, sallow complexion and irregular defecation, as well as the total scores in both groups were significantly lower than those before treatment;moreover, the improvement of each symptom score and total score in the observation group was significantly better than that in the control group, there were statistically significant differences(P<0.05). After treatment, the levels of trace elements(Zn, Fe, Ca) and hemoglobin in both groups were significantly higher than those before treatment, and the improvement of each index in the observation group was significantly superior to that in the control group, there were statistically significant differences(P<0.05). There was no statistically significant difference in adverse reactions between the two groups(P>0.05). Conclusion The application of Xiaoji Lipi powder and anorexia plaster combined with conventional treatment for children with anorexia of spleen dysfunction syndrome can significantly improve appetite, increase food intake, and promote the recovery of nutritional status and the absorption of trace elements. It is significantly effective, safe and reliable, and is worthy of promotion.

    Exploration of prescription patterns in Professor Wang Xiaoming's treatment for pediatric food accumulation and stagnation based on data mining

    Objective To collate and analyze Professor Wang Xiaoming's traditional Chinese Medicine(TCM) prescriptions for pediatric food accumulation and stagnation, and summarize her clinical experience in medication for pediatric food accumulation and stagnation. Methods Prescription data from the outpatient department of Wang Xiaoming's Distinguished TCM Practitioner Studio in Zhejiang Province from January 1, 2019 to December 31, 2024 were collated and screened. Statistical software was employed to perform frequency analysis, analysis of medicinal properties (nature, flavor, channel tropism), analysis of medicinal efficacy, association rule analysis, and cluster analysis of the constituent herbs. Results A total of 382 prescriptions were screened out, identifying 19 high-frequency herbs. These herbs predominantly targeted the spleen, lung, stomach, and liver channels, with bitter, sweet, and pungent flavors being most common, and cold, warm, and neutral properties prevailing. The most frequently used herbs belonged to the categories of digestion-promoting, deficiency-tonifying and qi-regulating medicinals. The second-order association rule with the highest support was Fuling-Jineijin.The third-order association rule with the highest support was Baizhu-Fuling-Jineijin.Cluster analysis of high-frequency core herbs revealed two combinations. Conclusion Professor Wang's treatment for pediatric food accumulation and stagnation focuses on the core pathogenesis of "food accumulation leading to impaired spleen transportation, and qi stagnation, often accompanied by internal heat or liver qi stagnation." She establishes the core therapeutic principles of "promoting digestion and removing stagnation, strengthening the spleen and boosting qi, and regulating qi and harmonizing the middle Jiao." Innovatively, she emphasizes the simultaneous treatment of the liver and spleen and the clearance of stagnated heat.

    Observation on the curative effect of Jianpi Yiqi ointment in treating anorexia(spleen and stomach qi-deficiency syndrome) in children

    Objective To explore the clinical effects of Jianpi Yiqi (tonifying spleen and benefiting qi) ointment in the treatment of pediatric anorexia (spleen stomach qi-deficiency syndrome). Method One hundred children with anorexia(spleen and stomach qi-deficiency syndrome) admitted to the Pediatric Department of Jingzhou Hospital of Traditional Chinese Medicine from January 2023 to February 2024 were selected as the research subjects, and they were randomly divided into a control group and an observation group, each consisting of 50 cases. The control group was treated with oral administration of Xingpi Yanger granules, while the observation group was treated with oral administration of Jianpi Yiqi ointment. The total effective rate of treatment, the excretion rate of urinary D-xylose, the ratio of salivary amylase activity, the levels of gastrin, motilin and neuropeptide Y, the improvement in symptom scores, and the body weight and height before and after treatment were observed and compared between the two groups. Results The total effective rate of treatment for children with anorexia(spleen and stomach qi-deficiency syndrome) in the observation group was 98.0%(49/50), significantly higher than that in the control group (84.0%, 42/50), with a statistically significant difference(P<0.05). After treatment, the urine D-xylose excretion rate, salivary amylase activity ratio, gastrin level, motilin level, and neuropeptide Y level in the observation group were higher than those in the control group, with statistically significant differences(P<0.05). After treatment, the symptom scores(food intake, appetite, and eating time) in the observation group were lower than those in the control group, with statistically significant differences(P<0.05). There was no statistically significant difference in body weight or height between the two groups before and after treatment(P>0.05). Conclusion Jianpi Yiqi ointment in the treatment of children with anorexia(spleen and stomach qi-deficiency syndrome) can significantly improve the therapeutic effect on the children, significantly improve a series of clinical indicators in them, and promote the relief of the clinical symptoms.

    Study on the assessment of "the TCM Constitution Scale for Children Aged 1+ to 7 Years"

    Objective To test the reliability and validity of "the TCM Constitution Scale for Children Aged 1+ to 7 Years", and to optimize it. Methods Healthy children from 5 hospitals were selected for on-site inclusion. The reliability and validity of the scale were analyzed by means of decision value-critical ratio, item-total correlation method, Cronbach's α coefficient, split half reliability, retest reliability, and factor analysis,etc.,and the items with problems were summarized. Results Independent sample t-test was performed for the high and low subscales of 9 subscales, and the items of each subscale had t>3.000, P<0.05, without modification or deletion. Pearson correlation coefficient test was performed on 9 subscales, and the correlation coefficients of items Qi deficiency 9 and blood stasis 40 with the total score were all < 0.4, so it was considered to change or delete them. The Cronbach's α coefficient of the total scale and each subscale was >0.6, and the internal consistency was acceptable. After deleting items 5, 9, 13, 23, 29, 36, 40, 52, 58, 59, and 62, Cronbach's α coefficient increased, and it was considered to delete or change them. Principal component analysis was used to extract common factors based on eigenvalue greater than 1, and a total of 15 common factors were extracted, with a cumulative contribution rate of 62.226%. In order to further fit nine TCM constitution theories, the number of extraction factors was set at 9, and the cumulative contribution rate was shown to be 50.342%.In the structural validity analysis of each subscale, the one with 1 component had the dampness-heat quality and the peace quality; the one with 2 components had the Qi-deficiency quality, Yang-deficiency quality, phlegm-dampness quality, blood-stasis quality, Qi-stagnation quality and special quality subscale; the one with 3 components had the Yin-deficiency quality subscale. The factor loading for all subscale items was >0.4. Conclusion The reliability and validity of the "the TCM Constitution Scale for Children Aged 1+ to 7 Years" for the assessment of children's physical constitution are acceptable, but it still needs to be adjusted to improve its reliability and validity. Later, a discussion with relevant experts will be conducted, and the items with problems will be deleted or modified to form the second version of test of the "the TCM Constitution Scale for Children Aged 1+ to 7 Years", in order to make this scale more suitable for clinical identification of children's physical constitution.

    Research advances in the treatment of pediatric epilepsy with Pingxian granules

    Epilepsy, a chronic neurological disorder, has become a significant public health problem impacting children's health and development. Pingxian granules, formulated by Professor Zhang Shiqing, National Renowned TCM Physician, have gradually been proved in the effect on pediatric epilepsy, demonstrating substantial application potential and offering a novel therapeutic strategy in the treatment of pediatric epilepsy. Through sorting out the previous clinical and basic research results in the treatment of pediatric epilepsy by Pingxian granules, this paper discusses the mechanisms of action and development prospects, as well as current technical limitations, aiming to offer ideas for further exploring the therapeutic mechanism of Pingxian granules and provide reference for the prevention and treatment of pediatric epilepsy in traditional Chinese medicine.

    Vitamin D, an essential fat-soluble vitamin for the human body, has long been recognized for its critical role in regulating calcium and phosphorus metabolism and supporting skeletal health. In recent years, a great number of clinical studies have proved that there is a significant association between vitamin D deficiency or insufficiency and the development of multisystem disorders in pediatric population. Particularly during key developmental stages in childhood, the immunomodulatory, anti-inflammatory, and organ-developmental effects of vitamin D have attracted much attention. This article provides a comprehensive review of the research progress in the application of vitamin D in pediatric conditions affecting the respiratory, gastrointestinal, immune, neurological and urinary systems as well as in infectious diseases, with the aim of providing references for the rational clinical use of vitamin D in pediatric population.

    Progress in traditional Chinese and western medicine treatments for idiopathic short stature

    Idiopathic short stature is a collective term for a group of highly heterogeneous conditions with unknown causes, primarily responsible for the short stature in children. If not promptly addressed, it can easily lead to psychological disorders and social adaptation difficulties in patients, significantly impacting their lifelong development quality. Currently, in western medicine, the core treatment approach is recombinant human growth hormone replacement therapy, supplemented by drugs that regulate the developmental process, such as gonadotropin-releasing hormone analogs and aromatase inhibitors. In traditional Chinese medicine, a syndrome-differentiated treatment system is employed, and internal administration of herbal medicines combined with external therapies of acupuncture, massage, and topical application of patches is used to promote bone growth, regulate endocrine metabolism, and improve physical condition, which demonstrates unique therapeutic effects and mechanism advantages.

    Expert investigation and research on the common syndrome types and symptoms of CAP in children

    Objective To screen for the TCM syndrome types and primary and secondary symptoms of community-acquired pneumonia (CAP) in children by using the Delphi method, and to provide evidence and reference for the establishment of diagnostic conditions for CAP in children. Methods Based on the Delphi method, literature and books were retrieved to formulate expert inquiry questionnaires. Expert consultation was conducted, and the results of expert consultation were evaluated from multiple perspectives such as the mean, variable coefficient, and coordination coefficient. Combined with the clinical experience of experts, the items were modified to form the key points of TCM syndrome typing and diagnosis of CAP in children. Results A total of 23 experts from 18 provinces in China were selected for questionnaire surveys. The recovery rate of this round of questionnaires was 92%, the positive coefficient of experts was 92%, and the authority coefficient was 0.96. Based on the mean values of each item and the coefficient of variation and combined with the clinical experience of experts, after appropriate deletion of the items, 8 syndrome types were finally determined: wind-cold lung closure, wind-heat lung closure, phlegm-heat lung closure, toxic-heat lung closure, Yin-deficiency lung heat, lung-spleen qi deficiency, damp-heat lung closure, and heat-pathogen lung closure, with the core diagnostic items being 10 to 24, including 3 to 21 primary symptoms and 2 to 17 secondary symptoms. Conclusion This study summarizes the diagnostic items of eight TCM syndrome types of CAP in children, integrating the macroscopic syndrome differentiation of traditional Chinese medicine with the modern microscopic syndrome differentiation, which can provide a basis for the establishment of diagnostic conditions and the construction of diagnostic criteria for the syndrome in the later stage.

    Traditional Chinese medicine theory of tic disorders comorbid with attention deficit hyperactivity disorder and the application

    Tic disorders comorbid with attention deficit hyperactivity disorder (ADHD) is a common comorbidity of neurodevelopmental disorders in children. The core symptoms are motor/vocal tics, attention deficit and hyperactive impulsivity, which seriously affect the psychological and social functions of children. Atomoxetine and clonidine are the main drugs in western medicine treatment, but their clinical use is limited due to gastrointestinal and cardiovascular adverse reactions. Traditional Chinese medicines have the advantages of fewer adverse reactions and stable curative effect, which are easily accepted by the parents of children. For this disease, traditional Chinese medicine believes that its etiology is closely related to insufficient endowment, improper diet, emotional disorder, and invasion of external pathogens. The basic pathogenesis lies in the dysfunction of viscera and the imbalance of Yin and Yang, especially the "liver hyperactivity generating wind", which is the key. As a direct factor leading to tic symptoms, wind pathogen is often combined with sputum and fire and other pathological factors, forming a complex pathogenesis such as wind-sputum disturbance and wind-fire stimulation, which leads to the co-occurrence of tic symptoms with attention deficit and hyperactive impulse symptoms. Clinically, based on the pathogenesis, it can be divided into five main syndrome types: syndrome of liver wind carrying phlegm, syndrome of kidney deficiency and liver hyperactivity, syndrome of external wind causing activity, syndrome of latent fire in spleen and stomach and syndrome of heart and liver fire hyperactivity. The ultimate purpose of treatment is to harmonize Yin and Yang of viscera, and the core treatment principle of calming the liver and stopping wind is established. The pathological factors such as wind, phlegm and fire are concurrently managed according to the principle of "simultaneous application of principal and subordinate treatments". Based on different syndrome types, appropriate treatment principles and methods and prescriptions are selected for treatment.

    Pediatric asthmatic bronchitis is a common disease in pediatrics, which often occurs in winters and springs. Its onset features are characterized mainly by "fever, cough and wheezing". Its symptoms and signs are similar to the symptoms of "cough", "wheezing" and "shortness of breath" in traditional Chinese medicine, so it is mostly classified into the categories of "cough disease", "asthma disease" and "wind-warmness lung heat". Clinically, the "phlegm-heat blocking the lung" syndrome is more common. Therefore, Professor Peng Yu, my mentor, believes that the main pathogenesis is "phlegm-heat coagulation and internal accumulation in the lungs", and the main treatment principle is "using both cold and warm therapies to treat the symptoms and treating the lungs and spleen simultaneously to address the root cause". In my clinical work, guided by my mentor's academic thought, I have used traditional Chinese medicine to treat pediatric asthmatic bronchitis and achieved relatively satisfactory results. This article mainly introduces my experience and insights in treating pediatric asthmatic bronchitis based on my mentor's academic thought, with the aim of reducing or avoiding the use of hormones or antibiotics in children with the disease.

    Analyzing the experience in the treatment of children's tic disorders from the perspective of "liver collaterals"

    In recent years, the incidence of tic disorders in pediatric clinical practice has been steadily increasing, emerging as a significant issue affecting the physical and mental development of affected children. This condition not only manifests as involuntary muscle tics or abnormal vocalizations but may also be accompanied by emotional and behavioral problems, directly impacting children's academic performance, daily life, and peer interactions. It urgently calls for collaborative attention from families, schools, and the healthcare institutions. Western medicine treatments may cause side effects such as sedation or drowsiness, and parents often worry about these adverse reactions and thus refuse to allow their children to take such medications. Traditional Chinese medicine (TCM) takes a different approach, proposing the concept of collaterals—organic pathways connecting internal organs and external limbs, and communicating between upper and lower parts of the body, which provides a unique perspective for TCM treatment. Based on the profound theoretical foundation of traditional Chinese medicine and rich clinical practice, the supervisor, Professor Feng Xiaochun, puts forward the academic idea of "differentiated treatment for children's diseases based on collaterals", and creatively believes that the core pathogenesis of the disease lies mainly in the "liver collateral imbalance", so the principle of its clinical treatment is to calm the liver and stop the wind and relax the tendons and promote the circulation. This is achieved by selecting herbal formulas with the effects of calming the liver, stopping wind, relieving spasms, and promoting circulation, while skillfully combining the external treatment of massage and herbal plaster application (Pingdong ointment), thus forming an integrated treatment system of "internal medication and external regulation" for childhood tic disorders. This approach has achieved significant therapeutic effects and advanced the prevention and treatment of childhood diseases from experience-based inheritance to precise intervention, bringing new insights to the prevention and treatment of childhood diseases.

    Exploring the mechanism and treatment of precocious puberty combined with obesity based on the theory of "Yang transforms qi and yin takes shape"

    Precocious puberty in children shortens the period of growth and development and is associated with adverse long-term health outcomes. Obesity is an important risk factor for precocious puberty, and the two often coexist and their mechanisms are involved in each other. Based on the theory of "Yang transforms qi and yin takes shape" in Huangdi Neijing, Prof. Zhang Guiju proposes that the pathogenesis of obesity combined with precocious puberty is characterized by insufficient yang transforming qi and excessive yin taking shape in the early stage, excessive use of yang and loss of yin essence in the middle stage, and loss of yin involving yang and insufficient use of yang in the late stage. The treatment emphasizes the staged approach: at the early stage, warm and recuperate the spleen and kidney yang qi, transform phlegm dampness, and eliminate fat and turbidity; at the middle stage, nourish the kidney yin, clear and reduce the phlegm-fire, and dredge and regulate qi; at the late stage, tonify the kidney and fill up the essence, balance yin and yang, and regulate the vital organs. The whole process of eliminating phlegm dampness and fat and regulating qi and vital organs is aimed at warming yang and transforming qi to promote metabolism, nourishing yin and reducing fire to control hyperactivity of yang, tonifying the spleen and kidney to assist growth, and ultimately restoring the balance of yin and yang to ensure the normal developmental rhythms in children.

    Clinical experience of Professor Chang Ke in treating autism spectrum disorder in children with Xinyang decoction

    Childhood autism spectrum disorder is a common mental and behavioral disorder in children. Taking the physiological and pathological characteristics of children into account, Professor Chang Ke analyzes the etiology and pathogenesis of autism spectrum disorder from four aspects: wind, phlegm, stasis and deficiency, and establishes the method of "searching for wind-eliminating phlegm-resolving stasis-replenishing deficiency", and emphasizes the importance of opening up the orifices in the treatment of this disease. Professor Chang Ke primarily employs his self-made formula—the Xinyang decoction—with modifications to treat symptoms associated with this condition. He further adjusts the formula based on the specific stage of the child's illness. Through syndrome differentiation, Professor Chang Ke focuses on expelling pathogenic factors (wind, phlegm, stasis) and opening up the orifices during the active phase of the disease, and during the remission phase, he emphasizes strengthening the body's vital energy (tonifying the kidneys and replenishing essence) to consolidate the foundation. This approach yields excellent therapeutic results, which offers new insights for the differentiated treatment for this condition in clinical TCM practice. One case is attached for verification.

    Analysis of Director Jia Liujin's experience in the clever application of Cangerzi powder in compound formulations

    Professor Jia Liujin has dedicated to the practice of medicine for over six decades, specializing in the in-depth study of single prescriptions and the meticulous research of herbal compatibility. In the treatment of pediatric diseases, he skillfully uses formulations and cleverly employs compound prescriptions to enhance synergistic effects, which results in remarkable therapeutic effects.Regarding the common pediatric disease of allergic rhinitis(AR), Director Jia, based on his rich clinical experience, classifies it into different types, including wind-heat AR,excessive heat of lung and stomach,phlegm-heat trapped in lung,deficiency and coldness of lung-qi,deficiency of lung and spleen-qi,etc. Based on these classifications, he selects the Cangerzi powder as the basic prescription and combines it with different types of prescriptions for compound formulation. The purpose of this article is to summarize the compound prescriptions of Cangerzi powder commonly used by Director Jia in clinical practice, analyze the prescription compatibility and differentiation characteristics, and inherit his academic experience, in order to provide new ideas for clinical diagnosis and treatment.

    As one of Jiangsu Province's intangible cultural heritages, the Qianzimen Pediatrics has exerted profound influence in cities such as Yangzhou, Nanjing, cities in the Huai River Basin, and Beijing. The sixth-generation inheritor Li Yaoqian is honored as a renowned veteran TCM practitioner in Jiangsu. Adhering to the Qianzimen Pediatrics' philosophy of "valuing moderation, simplicity, frugality, and remarkable efficacy", he emphasizes the importance of regulating qi flow and balancing tonification and drainage in treating pediatric pulmonary diseases. Based on the medication characteristics of the Yangzhou Medical School and the principle of analogy, he proposes that the drug pair of Gouteng-Jicaihua possesses antispasmodic and antitussive effects, effectively alleviating childhood cough. Both Gouteng and Jicaihua are commonly used drugs in clinical TCM. Although they are classified under liver-soothing and wind-calming herbs and heat-clearing and dampness-resolving herbs, respectively, the two herbs exhibit synergistic effects in the treatment of cough,and the clinical efficacy is remarkable.

    Predictive value of neutrophil-to-albumin ratio for convulsions with mild gastroenteritis accompanied by reversible splenial lesion syndrome in children

    Objective To explore the predictive value of neutrophil-to-albumin ratio for convulsions with mild gastroenteritis (CwG) accompanied by reversible splenial lesion syndrome (RESLES) in children,and provide a reference for early identification of high-risk child patients. Methods A total of 40 patients with CwG accompanied by RESLES were retrospectively enrolled as the study group, who were treated in the Department of Pediatrics of Shanxi Bethune Hospital from January 2017 to June 2025, and 63 CwG patients without RESLES hospitalized in the same period were selected as control group. The clinical characteristics and laboratory data of the two groups were analyzed. Results (1)There were no significant differences between the study group and the control group in terms of age, gender, BMI, presence or absence of fever, number of vomiting, number of diarrhea, duration of seizures, frequency of seizures, history of previous seizures and family history(P>0.05).(2)The platelet count, blood sodium, and albumin in the study group were lower than those in the control group, and the differences were statistically significant(P<0.05).The neutrophil count and CRP in the study group were higher than those in the control group, and the differences were statistically significant(P<0.05).The positive detection rate of fecal rotavirus antigen in the study group was higher than that in the control group, and the difference was statistically significant(P<0.05).(3)Composite indicators of blood inflammation: The neutrophil percentage/albumin count(NPAR), neutrophil count × monocyte count/lymphocyte count(SIRI), neutrophil count/lymphocyte count (NLR) and CRP × neutrophil count/lymphocyte count(IBI) in the study group were higher than those in the control group, and the differences were statistically significant(P<0.05).The lymphocyte count×albumin count/neutrophil count(LANR) and albumin/fibrinogen(AFR) in the study group were lower than those in the control group, and the differences were statistically significant(P<0.05).(4)Multivariate analysis showed that NPAR and the positivity of intestinal rotavirus antigen were independent risk factors for CwG acompanied by RESLES in children. A receiver operating characteristic curve (ROC) for NPAR yielded a 0.848 of the area under the curve, with the optimal cutoff value being 12.92, predictive sensitivity being 80.0%, and specificity being 74.6%. Conclusion High NPAR level and positive rotavirus antigen are independent risk factors for CwG acompanied by RESLES in children. The level of NPAR has certain predictive value for CwG acompanied by RESLES.
