正五聚蛋白3在儿童过敏性紫癜中的临床意义

Clinical significance of PTX3 in children with Henoch-Schonlein purpura

摘要:
目的
检测五聚素3在过敏性紫癜(HSP)及紫癜性肾炎(HSPN)患儿血浆及尿液中的表达水平,探讨其临床意义。
方法
选取2016年3月至2017年9月新乡医学院第一附属医院儿科收治的HSP患儿50例,根据有无累及肾脏分为HSP组32例和HSPN组18例。同期选取门诊健康体检的20例正常儿童作为对照组。检测各组血浆及尿液中五聚素3的表达水平;HSP组和HSPN组患儿急性期、恢复期血浆、尿液五聚素3表达水平。
结果
(1)HSP组和HSPN组血浆和尿液中五聚素3的表达水平显著高于对照组,差异有统计学意义(P<0.05);HSPN组血浆和尿液中五聚素3的表达水平显著高于HSP组,差异有统计学意义(P<0.05)。(2)HSP组和HSPN组恢复期血浆和尿液中五聚素3的表达水平显著低于急性期,差异有统计学意义(P<0.05)。
结论
五聚素3表达水平的检测不仅能反映HSP疾病的活动状态,还可能成为HSP患儿肾损伤的一项新的检测指标。

Abstract:
Objective
To detect the expression of PTX3 in plasma and urine of children with HSP and HSPN, and to explore its clinical significance.
Methods
A total of 50 HSP children were selected, who were treated in the First Affiliated Hospital of Xinxiang Medical College from Mar. 2016 to Sep. 2017, and they were divided into two groups based on whether kidney was involved: HSP group(32 cases) and HSPN group(18 cases). Twenty healthy children receiving physical examination during the same period were included as the control group. The expression level of PTX3 in plasma and urine was determined in the 3 groups; the expression level of PTX3 of HSP group and HSPN group in the plasma and urine at acute stage and recovery stage was also determined.
Results
(1)The expression level of PTX3 in plasma and urine was significantly higher in HSP and HSPN group than in control group(P<0.05); the level was significantly higher in HSPN group than in HSP group(P<0.05).(2)The PTX3 expression level of HSP and HSPN group was significantly lower at recovery stage than at the acute stage(P<0.05).
Conclusion
The detection of the expressions of PTX3 not only reflect the active state of HSP disease, but also may be a new detection index of renal damage in children with HSP.
