丙酸氟替卡松气雾剂加用孟鲁司特钠咀嚼片治疗螨虫过敏的咳嗽变异性哮喘疗效观察

Effect of fluticasone propionate aerosol combined with montelukast sodium chewable tablets in treating cough variant asthma with allergy to mites

摘要: 目的:观察丙酸氟替卡松气雾剂加用孟鲁司特钠咀嚼片治疗螨虫过敏的咳嗽变异性哮喘患儿的临床效果。
方法:2014年3月至2016年3月东莞市长安医院儿科收治螨虫过敏的咳嗽变异性哮喘患儿90例,随机分为对照组和观察组各45例。对照组给予丙酸氟替卡松气雾剂治疗,观察组在对照组治疗基础上给予孟鲁司特钠咀嚼片口服,3个月为1个疗程,两组均治疗2个疗程。观察两组患儿临床治疗效果、日间咳嗽评分、夜间咳嗽评分以及肺功能指标,如第1秒呼气容积(FEV1)、呼气峰流速(PEF)、FEV1占预计值的百分比(FEV1%),两组患儿复发情况和不良反应发生情况。
结果:观察组临床总有效率为91.1%(41/45),显著高于对照组75.6%(34/45),差异有统计学意义(P<0.05)。观察组患儿日间、夜间咳嗽评分、咳嗽缓解、消失时间均显著优于对照组,差异有统计学意义(P<0.05)。两组治疗后FEV1、PEF及FEV%均显著优于治疗前,差异有统计学意义(P<0.05),观察组FEV1、PEF及FEV%均显著优于对照组,差异有统计学意义(P<0.05)。两组复发率和不良反应发生率比较差异无统计学意义(P>0.05)。
结论:临床中应用丙酸氟替卡松鼻喷雾剂联合孟鲁司特钠咀嚼片治疗螨虫过敏的咳嗽变异性哮喘患儿,具有比较显著的临床效果,能对患儿的临床症状进行有效改善,而且安全可靠,具有临床推广和应用价值。

Abstract: Objective:To observe the clinical effect of fluticasone propionate aerosol combined with montelukast sodium chewable tablet in treating cough variant asthma in children with mite allergies.
Methods:From March 2014 to March 2016, 90 children with cough variant asthma who were allergic to mites were enrolled in Department of Pediatrics, Chang'an Hospital of Dongguan City, and were divided into control group and observation group by digital random method. The control group was treated with fluticasone propionate aerosol, and the observation group was given montelukast sodium chewable tablets in addition to the treatment for control group, 3 months as a course of treatment; the two groups were treated for 2 courses. Observe the clinical effect, day cough score, night cough score, and the pulmonary function indexes, such as FEV1, peak expiratory flow rate(PEF) and FEV1% of the two groups. The recurrence of the two groups and the incidence of adverse reactions were also observed.
Results:The total effective rate in the observation group was 91.1%(41/45), which was significantly higher than that in the control group(75.6%, 34/45); the difference was significant(P<0.05). The cough score, cough remission and disappearance time were significantly better in the observation group than those in the control group(P<0.05). The FEV1, PEF and FEV% of the two groups after treatment were significantly higher than those before treatment(P<0.05). The FEV1, PEF and FEV% of the observation group were significantly better than those of the control group(P<0.05). There was no significant difference in the recurrence rate and adverse reaction rate between the two groups(P>0.05).
Conclusion:Clinical application of fluticasone propionate nasal spray combined with montelukast sodium chewable tablets in the treatment of mite-allergic children with cough variant asthma has a significant clinical effect, which can effectively improve the clinical symptoms of children, and is safe and reliable, so it should be clinically promoted and used.
