Exploring the treatment for short stature in children with "the method of nourishing the spleen" based on the theory of "the spleen governing the four internal organs
DONG Dan, LI Ying, WANG Xuefeng
Chinese Pediatrics of Integrated Traditional and Western Medicine    2024, 16 (5): 387-391.   DOI: 10.3969/j.issn.1674-3865.2024.05.006
    Abstract140)      PDF(pc) (1048KB)(689)       Save
The causes of short stature in children can be summarized as insufficient innate endowment or improper postnatal nourishment. The spleen is the source of life after birth, and when the spleen is healthy, the nutrients it generates can be distributed throughout the body, providing sufficient material basis for the internal organs and abundant qi and blood, allowing normal growth and development of children. On the other hand, if the spleen and stomach functions are impaired, the source of qi and blood generation is lacking, which affects other organs and causes dysfunction of other internal organs, leading to delayed growth and development in children. Additionally, children have the physiological state and constitutional feature of "spleen often being insufficient". Therefore, treating childhood short stature based on the spleen is of great significance. Taking "nourishing and strengthening the spleen" as the basic treatment principle and based on the theory of "the spleen governing the four internal organs", the author lists eight common Chinese medical syndrome types: spleen deficiency, spleen deficiency with food stasis, spleen deficiency with dampness, spleen deficiency with liver hyperactivity, spleen- and stomach-yin deficiency, lung- and spleen-qi deficiency, spleen and kidney deficiency, and heart and spleen deficiency. This paper summarizes the experience in the differentiated treatment of short stature in children based on the basic formula of Sijunzi soup, aiming to provide clinical references for the treatment of short stature in children with traditional Chinese medicine.

CHEN Xi, PENG Yaqi, LI Wei, YANG Hongsheng, WANG Junhong
Chinese Pediatrics of Integrated Traditional and Western Medicine    2024, 16 (1): 88-92.   DOI: 10.3969/j.issn.1674-3865.2024.01.021
    Abstract135)      PDF(pc) (1055KB)(617)       Save
Autism spectrum disorders(ASD) is characterized by sustained impaired social ability, repetitive behaviors or interests, and stereotyped behavior. It is a common pediatric neuropsychiatric disease with great harm in TCM pediatrics, and in 2020, the domestic incidence has reached 7‰. The optimal treatment period is often missed due to delayed diagnosis, limited treatment measures, failure to adhere to treatment, improper parental care and so on. The "Original Spirit" of ASD is damaged, and the lesion is in the brain. This paper analyzes the correlation between brain and spleen and kidney from the material basis, structural basis, operational basis and pathological connection, and concludes that the deficiency of spleen and kidney is the fundamental pathogenesis of ASD, and phlegm and blood stasis is the important pathological factors of ASD. Professor Wang Junhong thinks that the physiological and pathological characteristics of children should be considered clinically, and the basic treatment should be to tonify the kidney and spleen, eliminate phlegm and remove blood stasis. At the same time, there should be no improper tonification, and the focus is to regulate the whole body movement and chemical function of children. In the treatment, the classic prescription Liuwei Dihuang pills and Liujunzi decoction are selected as the main prescription of Bushen Yunpi Yizhi prescription. According to the viscus syndrome, the treatment combines comprehensive analysis of the four examination methods and achieves good effects. Based on clinical cases, this article introduces the key points of syndrome differentiation, treatment and medication experience of Bushen Yunpi Yizhi prescription in the treatment of children with ASD.

    Abstract100)      PDF(pc) (831KB)(612)       Save
Tourette's syndrome, also called tic disorder, is a common pediatric behavioral disorder in clinical practice, which is characterized by involuntary muscle twitching of the face, limbs and trunk, accompanied by abnormal laryngeal articulation and foul language. There is no clear record of Tourette syndrome in traditional Chinese medicine, and it is classified into the category of slow and shocked wind and liver wind according to its clinical manifestations. As one of the first national famous Chinese physicians,Professor Chen Baogui is the third generation of successor to the integrated traditional Chinese and western medicine school, which was established by Zhang Xichun. Professor Chen is very knowledgeable and experienced with more than 50 years of medical experience, and has unique views on the treatment of Tourette's syndrome. Prof. Chen thinks that its etiology can be summarized as "inherent weaknesses and acquired dysplasia","pathogenic factor harming lung and stimulating liver wind","spleen deficiency generating phlegm and inducing liver wind","blazing of heart-liver fires resulting in spiritual timidness". The lesion is mainly in the liver and spleen, and is closely related to the heart, lung and kidney. The treatment is generally based on the principle of "protecting the spleen and stomach and regulating the five viscera",including nourishing the liver, kidney and spleen, tonifying lung and softening liver wind, invigorating spleen and soothing liver, clearing the fire from liver and heart, and clearing heat and sedating. Acupuncture treatment is combined with medication to relieve depression at same time.

Objective To explore the impact of applying behavioral training (ABA) combined with the "60-point mother theory" on the rehabilitation treatment of children with autism. Methods Totally 62 children with autism admitted to the Rehabilitation Center from June 2020 to January 2022 were selected as the research subjects. According to the principle of matching basic features between groups, they were divided into a control group and an observation group, with 31 cases in each group. The control group received comprehensive rehabilitation treatment, with ABA training as the main intervention. The observation group received ABA training combined with the "60-point mother theory" as an auxiliary guide. The autism behavior of the two groups of children before treatment and three months after treatment were compared by Autism Behavior Checklist(ABC). The Psychological Education Scale (PEP-3) for autism spectrum and related developmental disorders was used to make a comprehensive evaluation of the children with autism. Results After 3 months of intervention, the improvement in PEP-3 scores in the observation group was significantly better than that in the control group(P<0.05).The total ABC scores of the two groups were lower on discharge than on admission, and the observation group was lower than the control group(P<0.05).Conclusion The use of ABA training combined with the "60-point mother theory" is of great significance in children with autism, as it can effectively improve the overall developmental functional level and alleviate clinical symptoms of children with autism.

    Abstract96)      PDF(pc) (832KB)(573)       Save
Cerebral palsy is a group of syndromes characterized by movement disorders, abnormal posture, musclestrength and muscle tension changes. The main function of the vestibular system is to maintain the stability of the head and trunk, maintain body balance and motor coordination. The vestibular function damage of children with cerebral palsy can lead to impaired balance function, poor posture control, difficulty in eye movement, poor head-eye coordination and delayed motor development. Vestibular rehabilitation training is an individualized non-drug rehabilitation treatment, which, mainly through vestibular adaptation, vestibular acclimatization, vestibular compensation and other mechanisms and through repeated strengthening of vestibular receptors, improves the excitability of nerve cells and promotes the injury of brain tissue area to produce new neural pathways,so as to effectively improve the patient′s motor function, posture control, balance ability, muscle strength and muscle tension, eye movement and so on. At present, vestibular rehabilitation training has been widely used in adults with Parkinson′s disease, stroke, spinal cord injury,Alzheimer′s disease. It has also been used in children with autism spectrum disorder, attention deficit hyperactivity disorder, and global developmental delay. There are few reports on the effects of vestibular rehabilitation training on children with cerebral palsy. Based on the above background, this paper summarizes the vestibular rehabilitation training mechanism of vestibular rehabilitation training in the treatment of children with cerebral palsy at home and abroad,in order to provide help for better improving the quality of life of children with cerebral palsy.

Treatment for childhood short stature based on "Yang transforming qi and yin shaping stature"
    TONG Tingting, ZHANG Guiju
Chinese Pediatrics of Integrated Traditional and Western Medicine    2024, 16 (5): 392-394.   DOI: 10.3969/j.issn.1674-3865.2024.05.007
    Abstract150)      PDF(pc) (1048KB)(558)       Save
Childhood short stature is mostly caused by insufficient cong enital endowment and improper nutrition later on, and the key pathogenesis is the spleen and kidney insufficiency. At present, recombinant human growth hormones are used most often for the treatment of short stature in western medicine, and the treatment plan is relatively simple. Traditional Chinese medicine(TCM) has obvious effects in the treatment of short stature based on syndrome differentiation, which can effectively improve children's growth rate and regulate physical fitness. Based on the clinical experience of Professor Zhang Guiju and the theory of "Yang transforming qi and yin shaping stature" it is believed that childhood short stature is closely related to the insufficiency of "Yin shaping stature". Yin essence plays an important role in the growth and development of the human body. Children can gradually grow taller and stronger if the function of "Yin shaping stature" is normal. On the contrary, insufficiency in "Yin shaping stature" will lead to delayed growth and short stature in children. Therefore, in the treatment of childhood short stature, it is necessary to add yin-nourishing Chinese medicines based on syndrome differentiation, such as Radix rehmanniae praeparata, Cornus officina, yam, Fructus lycii, Polygonatum kingianum, Polygonatum odoratum, lily, etc. Such medicines contain a variety of effective ingredients, which can not only supplement the nutrients needed by the human body, but also have a variety of effects such as regulating immunity and protecting the internal organs. In addition, spleen-strengthening and digestion-assisting drugs should be added to the prescription to protect the spleen and stomach and prevent getting too greasy.

One case of neuroblastoma complicated with heart failure in children is reported, and the possible cause was studied. The child patient is 3 years and 7 months old, the onset is occult and anorexia is the first symptom. After abdominal imaging and postoperative histopathological examination, it is diagnosed as neuroblastoma. In the 4 months of treatment, the following chemotherapy regimens have been used in turn: cyclophosphamide combined with irinotecan, etoposide combined with cisplatin, and then etoposide combined with nedaplatin. After 5 cycles of chemotherapy, the child has some symptoms of acute heart failure, including dyspnea, forced sitting position, pallor, cyanosis and irritation. Clinically there have been few reports on heart failure after chemotherapy for neuroblastoma, so we should be alert and pay attention to the toxic effect of chemotherapy medicines on heart, performing related examinations at an early stage in order to improve heart function and the prognosis.

Because children with short stature are shorter than children of the same age, they may suffer from low self-esteem,which seriously affects their mental health and has different degrees of impact on their future learning and lives.Traditional Chinese medicine has some advantages in the treatment of this disease because its syndrome differentiation and treatment can improve the growth and development of children. This paper first sorts out the historical development of short stature, which,as an independent name of the disease, was firstly seen in the textbook of Integrated Traditional Chinese and Western Medicine Pediatrics edited by Prof.Wang Xuefeng. Then, the etiology and mechanism of short stature are summarized. Its etiology is the deficiency of innate endowment and the lack of nutrition in later life. The disease is located in the spleen and kidneys, and it is closely related to the heart, liver and lungs. The nature of the disease is mostly deficiency. The primary mechanism of the disease is the deficiency of the five organs and the deficiency of essence and qi.The basic treatment principle is determined again, which is to support the spleen and nourish kidneys and also to take into account other organs.Among the treatment methods, the method of tonifying the kidneys and nourishing the liver is suitable to those with deficiency of yin in the liver and kidneys and loss of nourishment of the muscles and bones;the method of supporting the spleen and strengthening the stomach is suitable to those with weakness of the spleen and stomach and deficiency of qi and blood;the method of nourishing the kidneys and clearing the heart is suitable to those with deficiency of the heart and kidneys and imbalance of water and fire;the method of strengthening the spleen and nourishing the lungs is suitable to those with deficiency in the lungs and spleen and loss of qi of the mother;the method of warming and tonifying the kidney-yang is suitable to those with insufficiency of the kidney-yang and dysfunction of warming. Finally, the strategy of multi-dimensional care and prevention before the onset of disease is put forward. For children with short stature, attention should be paid to balanced nutrition, moderate increase in exercise, reasonable arrangement of sleep, and adjustment of the body in line with the four seasons.

Functional constipation (FC) is a common gastrointestinal disease in children. The global prevalence of functional constipation in children is about 0.5% to 32.2%,which seriously affects the quality of life of children.Wan Quan is a famous pediatric doctor in ancient China, who innovatively put forward the idea of "spleen insufficiency" in children, and paid attention to regulating the spleen and stomach, which has an important guiding role in the diagnosis and treatment of modern pediatric diseases.Wan Quan put forward the general principle of the treatment for pediatric constipation in "Infant Family Secrets:Volume Ⅳ:Urination and Defecation Diseases":"The stool induration should be attacked, but sometimes is attacking method useless in spite of the condition".Based on Wan Quans theory of "spleen insufficiency",and "The stool induration should be attacked, but sometimes attacking method is useless in spite of the condition",and combined with modern research progress,it is proposed that the causes of FC in children are mostly attributed to dryness-heat accumulation, stagnation of milk and food, spleen deficiency leading to transportation failure,qi stagnation, and blood deficiency resulting in intestinal dryness.Spleen deficiency leading to food injury is considered to be the key pathogenesis;therefore,it is recommended that the treatment for FC in children should differentiate deficiency from excess, and carry out the principle of activating spleen to assist the transportation throughout the treatment process. This study explores the differentiation and treatment strategies for FC in children from the aspects of acute purgation, digestion and elimination of accumulated stagnation, slow purgation, regulating qi,and moistening dryness, etc.,in order to provide a reference for the clinical diagnosis, treatment, and care of FC in children.

With the change of dietary structure and lifestyle, the rate of pediatric obesity is increasing year by year, which not only affects the growth and development, but also leads to the occurrence of metabolic diseases. Therefore, effective prevention and treatment measures should be taken regarding the pathogenesis of pediatric obesity. In recent years, with the development of high-throughput sequencing technology, many studies have pointed out that the structural characteristics of gut microbiota in obese children are different from those in normal children, which is manifested by the increase of relative abundance of Firmicutes and conditioned pathogen as well as the decrease of Bacteroidetes and probiotics. Modern medical science has shown that gut microbiota and its metabolites interact with the host, which can regulate the inflammation response, intestinal homeostasis, energy metabolism, eating behavior and expression of circadian rhythm genes, thus affecting the occurrence and development of obesity. Traditional Chinese medicine believes that the function of spleen is to govern transportation and transformation and is closely related to the gut microbiota in digestion and absorption as well as resistance to evil. When the spleen fails to transport and transform water, the water gathers into dampness and generates phlegm, blocking qi movement, thus affecting the intestinal function and immunity and disturbing the gut microbiota, which promotes the occurrence of obesity. This article reviews the current research progress in pathogenesis of gut microbiota and pediatric obesity, in order to provide new targets for the prevention and treatment of pediatric obesity and improve the clinical effectiveness.

Pharyngitis and tonsillitis are common respiratory infectious diseases in childhood, and are highly prevalent in spring and fall. They can be categorized into acute and chronic stages. The acute stage is characterized by rapid onset and progression of the disease, and in severe cases, hoarseness and even respiratory difficulties may occur. In the chronic stage, the disease is prolonged due to the lack of proper treatment.Repeated pharyngitis and tonsillitis can lead to voice changes in children. At present, there are not many western medicines that can effectively treat childhood pharyngitis and tonsillitis, which are mainly antibiotic drugs, and although there are many types of traditional Chinese medicines used to clear the throat, western pediatricians are not standardized in the selection, syndrome differentiation and application of traditional Chinese medicines. Therefore,it is particularly important to explore the treatment for pharyngitis and tonsillitis in children with integrated Chinese and western medicine. This consensus firstly systematizes the Chinese and western medical understanding of pharyngitis and tonsillitis in children;secondly,it discusses its staging, etiology and main risk factors;thirdly,it clarifies its diagnostic criteria;finally,it gives a detailed elaboration on the principle of integrated Chinese and western medicine treatment for pharyngitis and tonsillitis in children and the Chinese and western medicine treatment methods. This consensus provides Chinese and western pediatricians, especially western physicians, with ideas on integrated Chinese and western medicine treatment, and further standardizes the rational selection and application of Chinese and western medicines.

This paper discusses the etiology, pathogenesis, course characteristics and clinical symptoms of Henoch-Schonlein purpura nephritis based on the theory of 'pathogen lodging in Shaoyin'. The deficiency of kidney yin is the primary condition of hidden pathogen storing in the body. The inducing factors were exposure to six excesses, diet irritation and contact with allergic substances.External evil is from the surface into the inside, gathering in the throat. It turns into heat after along period of time and becomes a latent fire. The latent fire consumes the fluid and blood and becomes stagnated. The stagnated fire is interconnected and becomes disease of the nest, and stays at kidney collaterals. Latent fire goes outside, and the conflict occurs between pathogenic qi and vital qi. The heat damages the blood collaterals, blood overflows to the skin, and blood seeps to urine, then skin ecchymosis or hematuria occurs. The latent fire is hidden and thepathogenic qi and vital qi are not in conflict, then there is no hematuria or skin ecchymosis. The latent and recurrent process of pathogenic qi is the basic feature of insidious pathogenic diseases. The treatment often depends on its stages, including acute stage and remission stage. In the acute stage, the treatment methods are clearing latent fire, activating blood circulation and dispelling blood stasis. In the remission stage, the treatment methods are nourishing Yin and blood and repairing kidney veins. Fumai decoction, Didang decoction and Liuwei Dihuang pill can be used flexibly in a comprehensive treatment for Henoch-Schonlein purpura nephritis. Eliminating the disease nest and removing the insidious pathogens can stop and reverse the persistent and refractory disease of nephritis.
