    Individualized intervention strategies in the treatment of childhood and adolescent obesity based on precision medicine

    Childhood and adolescent obesity has become a global challenge in public health.Its high weight regain rate and risks of metabolic complications highlight the limitations of traditional intervention strategies.This review systematically explores individualized intervention strategies for obesity in children and adolescents in the context of precision medicine,breaking the traditional "one-size-fits-all" model.It integrates multi-omics technologies with intelligent management methods,proposing a multidimensional framework for precise intervention.First,a precise diagnosis and evaluation system is constructed based on multi-modal data,including genomics,radiomics,and gut microbiomics,to identify genetic susceptibility,metabolic phenotypes and microbiome characteristics,laying the foundation for stratified interventions.Secondly,individualized intervention plans are developed based on genetic risk scores,epigenetic regulatory mechanisms,and gut microbiome-targeted therapies:gene-guided strategies aim to optimize dietary structure through multi-gene risk assessments; epigenetic interventions aim to regulate DNA methylation and histone modification by taking advantage of nutrients (such as methyl donors and short-chain fatty acids),reversing metabolic dysregulation; microbiome remodeling aims to improve energy metabolism balance through probiotics and synbiotics.Furthermore,artificial intelligence and wearable devices are employed for dynamic monitoring in multiple scenarios.By collaborating with multidisciplinary teams in nutrition,sports medicine,and psychology,a smart management model of "hospital-family-community" network is established.While precision medicine offers groundbreaking directions for childhood obesity treatment,challenges such as biomarker validation,data integration,and long-term efficacy evaluation remain to be addressed.This review provides theoretical foundations and practical pathways for the improvement in the effectiveness and sustainability of interventions,emphasizing the need to integrate interdisciplinary innovative technologies in the future in order to promote the transformation of childhood obesity management toward precision and intelligence.

    Pediatric cough variant asthma (CVA) is a common chronic respiratory disease primarily characterized by chronic cough. The exact etiology of CVA is not yet fully understood, but it is widely believed that the progression of the disease is influenced by various pathological factors, and is closely related to the patient's individual constitution, seasonal changes, and regional environmental characteristics. Traditional Chinese medicine (TCM) categorizes CVA under "wind cough", "wheezing cough", and "spasmodic cough", and pediatric CVA is managed according to the principle of "San Yin Zhi Yi"(three-cause adaptation), which involves personalized adjustments of the treatment plan based on the patient's constitution, seasonal changes and regional environment, reflecting the holistic treatment concept and syndrome-differentiation treatment of TCM. Therefore, this article discusses the etiology and pathogenesis of pediatric CVA from the perspectives of both TCM and modern medicine, based on the TCM principle of "San Yin Zhi Yi". It comprehensively considers the impact of factors such as constitution, season and environment on the onset and progression of the disease, exploring the application of San Yin Zhi Yi treatment for pediatric CVA from the three aspects of adapting to the season, region and individual. The aim is to provide a better theoretical foundation and practical guidance for the future TCM treatment of pediatric CVA.

    This paper mainly makes a retrospective analysis and summary of TCM literature related to dwarfism in the past 10 years,aiming to provide a theoretical basis for TCM diagnosis and treatment of dwarfism.The treatment for dwarfism in TCM includes oral administration of Chinese medicine,acupuncture,massage,acupoint application,etc The curative effect is good,the price is relatively low and there are fewer adverse reactions,so it is worthy of clinical promotion and further study.

    The function of the spleen in children is often insufficient, and improper feeding, or disease damage may cause the fact that food can't be digested, which is food accumulation。Although it is a disease of the spleen, Professor Sun Liping believes that long time of food accumulation can form internal heat syndrome, thereby inducing a variety of pediatric diseases. Attention should be paid to the physical changes caused by congenital abnormality and acquired dystrophy. Based on the physiological and pathological characteristics of children, Yin and Yang theory, five-element theory and constitution theory, and through combination with more than 30 years of clinical experience of Professor Sun Liping, this paper explores the etiology and pathogenesis of children's diseases caused by "food", the disease outcome transformed by "heat", the diagnosis experience from "observation", and the treatment method by "elimination". A new line of thinking of "same treatment for different diseases" is provided for the diagnosis and treatment of abdominal pain, cough, asthma and tic disorders caused by food accumulation.

    Functional dyspepsia,as one of the most common functional diseases of upper digestive tract,seriously affects the quality of life and growth and development of children in China.Due to the changing definition and symptom spectrum of functional dyspepsia in children,its diagnosis and treatment are controversial.In order to standardize the diagnosis and treatment of children with functional dyspepsia with integrated traditional Chinese and western medicine,and effectively solve the clinical problems in the diagnosis and treatment of children with functional dyspepsia,this guideline is formulated to guide the standardized diagnosis and treatment of children with functional dyspepsia with integrated traditional Chinese and western medicine.

    In this paper, we analyzed the evolution of TCM name of ADHD, clinical and experimental research, characteristics and advantages,and deficiencies and prospects. Although there are many clinical reports, most of them lack strict design, without multicentric, randomized, controlled and large-sample clinical research. In the recent 10 years, the research projects supported by National Natural Science Foundation of China haven't obtained ground-breaking progress. The advantages of TCM treatment for ADHD are individualized diagnosis and therapy, syndrome differentiation treatment, integral regulation and restoration of the function of the internal organs. In order to make greater innovative achievements, it is necessary to conduct further research and standardize experiment through cooperation.

    In the autumn and winter of 2023, the number of children with Mycoplasma pneumoniae respiratory tract infections in Northeast China has increased significantly. In order to further standardize the clinical prevention and treatment and give full play to the advantages of integrated traditional Chinese and western medicine treatment, the construction project team of pediatric pneumonia(a key disease) of the National Center for Inheritance and Innovation of Traditional Chinese Medicine, and the pediatric experts of traditional Chinese and western medicine in Northeast China, by referring to the National Health Commission's Guidelines for the Diagnosis and Treatment of Mycoplasma Pneumoniae Pneumonia in Children(2023 Edition) and on the basis of the clinical characteristics of children in the cold regions of Northeast China, have reached a consensus on the classification of Mycoplasma pneumoniae respiratory tract infection, the pathogenesis of traditional Chinese and western medicine, clinical manifestations, imaging and bronchoscopic manifestations, laboratory examination, early warning of important complications, and the treatment of traditional Chinese and western medicine in the infection period, pneumonia period and recovery period. The aim is to effectively solve the clinical problems of Mycoplasma pneumoniae respiratory tract infection and improve the comprehensive abilities of pediatricians in the diagnosis and treatment of Mycoplasma pneumoniae respiratory tract infection, and then promote the development of integrated traditional Chinese and western medicine research in Mycoplasma pneumoniae respiratory tract infection.

    In Xu's pediatrics of Shanghai traditional Chinese medicine, it is believed that the pathogenesis of children's insomnia lies in the rise of yang and the weakness of yin, so the treatment is as follows: on the basis of warming the lower and suppressing yang, Huanglian Ejiao decoction is used to warm yang and nourish yin and to control water and fire and yin and yang to restore its normal state, thus forming a treatment with the characteristics of Shanghai traditional Chinese medicine. Based on the warming and suppressing method of Xu's pediatrics of Shanghai traditional Chinese medicine, this paper explores Xu's understanding of etiology and pathogenesis of insomnia in children, and analyzes the characteristics of prescription and medication of tranquilizing method by suppressing yang through the combination with the development characteristics of etiology and pathogenesis in children. A case is included here to provide some references for clinical practice.

    Functional dyspepsia is a common disease in childhood, which belongs to the category of "anorexia","stagnation","vomitting" and "abdominal pain" in traditional Chinese medicine(TCM) according to the differentiation of main symptoms, and the TCM differential treatment has its unique advantages. Director Wang Jing is good at dealing with spleen and stomach diseases in children and performs clinical diagnosis and treatment of functional dyspepsia based on the theory of "spleen-dominating tiredness and sleepiness" from the syndrome differentiation system of Qian Yi Wu Zang. Considering the changes in the pathogenic factors of the diseases, Director Wang Jing thinks that the pathogenic factors of exogenous evil causing functional dyspepsia in children have been reduced nowadays, while the pathogenic factors such as hand-foot-mouth disease and chicken pox have increased, and such disease causes as having too much greasy and raw food and emotional diet have increased significantly. As a result, the 3 most common clinical syndromes of functional dyspepsia in modern children are dysfunction of spleen in transportation, stagnation of liver invading spleen and comorbidity of lung and spleen. The treatment principle should be to soothing liver and strengthening spleen. Based on the Director's experience, a self-made digestive mixture has been developed in the hospital to strengthen spleen and assisting the transportation. In the digestive mixture, dried tangerine peel is used to strengthen spleen and promote transportation; Poriacocos is used to strengthen spleen and removing dampness; rice sprout, and malt are used to strengthen spleen and stimulate appetite, and promote digestion and regulate stomach. For the children with liver stagnation, Shichangpu and Yujing can be added to soothe liver and relieve stagnation. If there is comorbidity of hand-foot-mouth disease, while the lung qi is being dispersed during acute stage, spleen and stomach should also be dealt with, and the empirical formula of the Director's father for measles is used in combination with the digestive mixture with modification. During the relief period, it is necessary to nourish lung and strengthen spleen, and Huangqi(Astragalus) and Baizhu(Atractylodes) are added to the mixture for treatment.

    The causes of short stature in children can be summarized as insufficient innate endowment or improper postnatal nourishment. The spleen is the source of life after birth, and when the spleen is healthy, the nutrients it generates can be distributed throughout the body, providing sufficient material basis for the internal organs and abundant qi and blood, allowing normal growth and development of children. On the other hand, if the spleen and stomach functions are impaired, the source of qi and blood generation is lacking, which affects other organs and causes dysfunction of other internal organs, leading to delayed growth and development in children. Additionally, children have the physiological state and constitutional feature of "spleen often being insufficient". Therefore, treating childhood short stature based on the spleen is of great significance. Taking "nourishing and strengthening the spleen" as the basic treatment principle and based on the theory of "the spleen governing the four internal organs", the author lists eight common Chinese medical syndrome types: spleen deficiency, spleen deficiency with food stasis, spleen deficiency with dampness, spleen deficiency with liver hyperactivity, spleen- and stomach-yin deficiency, lung- and spleen-qi deficiency, spleen and kidney deficiency, and heart and spleen deficiency. This paper summarizes the experience in the differentiated treatment of short stature in children based on the basic formula of Sijunzi soup, aiming to provide clinical references for the treatment of short stature in children with traditional Chinese medicine.

    This paper explores the application value of Traditional Chinese Medicine (TCM) theory of "lungs governing skin and hair" and the thinking method of "analogy and classification" in the treatment of pediatric pulmonary diseases.Both "lungs governing skin and hair" and "analogy and classification" are traditional TCM theoretical approaches guiding clinical differential diagnosis and treatment.By discussing the correlation between the lungs and the skin and hair,and then employing the method of analogy and classification,this paper explores the feasibility and efficacy of treating pulmonary diseases by the "skin"-cortex (the character of skin and cortex is the same in Chinese).Starting from the physiological characteristics of children,such as "lung deficiency" and loose cutaneous layers,the paper argues that "cortex-related" herbal medicines (e.g.,Baixian Pi,Hou Po,and Chan Tui) can improve the pathological features of being easily invaded by external pathogens in children.Through case studies,this paper demonstrates a formula containing cortex-related herbs in the treatment of cough (wind-phlegm obstructing the lung) in a 4-year-old child,which is very effective.The results confirm the clinical feasibility of this approach.By integrating traditional TCM theories with modern scientific validation,this paper provides new insights into the diagnosis and treatment of pediatric pulmonary diseases,highlighting the unique advantages of TCM in the field of pediatrics.

    Objective To explore the distribution of traditional Chinese medicine(TCM) constitution in children with attention deficit hyperactivity disorder(ADHD) (syndrome of heart and liver fire hyperactivity),and its relationship with symptoms and social function of the children.Methods A total of 70 children aged 7 to 14 years with ADHD (syndrome of heart and liver fire hyperactivity) were included in the study,who were treated in Yixing Traditional Chinese Medicine Hospital from January 2023 to May 2024.The TCM syndrome scoring,SNAP-Ⅳ and CGAS were used to evaluate the severity of the symptoms and the impact on social function of the children,and the TCM constitution distribution was divided according to the TCM constitution scale.Results TCM syndrome score was (18.54 ± 2.84) in ADHD children (syndrome of heart and liver fire hyperactivity).The average SNAP-Ⅳ score of the boys was (41.02±8.56),the average SNAP-Ⅳ score of the girls was (41.19±8.46),the average CGAS score of the boys was (63.12±6.83),and the average CGAS score of the girls was (63.75±6.65),which significantly affected the children′s social function and normal life activities; the severity of the disease was not related to gender(P>0.05).Unbalanced constitution was the main type of TCM constitution,totaling 42 cases (60%),and 28 cases (40%) were with balanced constitution.Among the children with unbalanced constitutions,the number of children with qi-stagnation constitution,yin-deficiency constitution and qi-deficiency constitution was the highest (≥10%); the children with yin-deficiency constitution had the longest course of disease [(4.25±0.41) years],and the children with balanced constitution had the shortest course of disease [(1.74±0.33) years].Conclusion The TCM syndrome score of the 70 children with ADHD(syndrome of heart and liver fire hyperactivity) in Yixing area included in this study is high,which has a significant impact on the social function of the children,but there is no gender difference.Unbalanced constitution is the main type in the distribution of constitution,and the disease course of ADHD (syndrome of heart and liver fire hyperactivity) is different in children with different constitutions.

    Objective: Traditional Chinese medicine has obvious advantages in treating mesenteric lymphadenitis,and the external treatment is especially effective.This article summarizes the reports on TCM external therapy for mesenteric lymphadenitis in the past five years.It is found that acupuncture,massage,acupoint application and umbilical therapy,enema with traditional Chinese medicine,percutaneous treatment with traditional Chinese medicine on acupoints,ultrasonic induction of traditional Chinese medicine,microwave physiotherapy and other methods are more effective than conventional treatment with pure western medicine.This provides reference and methods for clinical treatment and broaden the thoughts on clinical treatment methods to a certain extent hoping to further promote the external treatment of TCM in clinical practice.

    Zhimu-Huangbai is a commonly used medicine pair in clinical practice,and the compatibility of the two drugs has the effects of nourishing yin and clearing heat,purging fire and detoxifying.Modern studies have found that Zhimu-Huangbai has anti-inflammatory,antioxidant,hypoglycemic,and lipidemic regulation effects because of its main active ingredients such as quercetin,kaempferol,barmatine,stigmasterol,mangiferin,berberine,etc.It is widely used in clinical practice,mainly for the treatment of precocious puberty,diabetes,diabetic nephropathy and other diseases in children.From the perspectives of chemical composition,pharmacological effects and clinical application,this paper expounds the application research of Zhimu-Huangbai in precocious puberty in recent years,in order to provide a theoretical basis for the scientific application of the drug in precocious puberty and for the research and development of new drugs.

    Objective To analyze the direct medical cost and composition ratio of children with acute lymphocytic leukemia,in order to provide basis for the reform of medical system and guarantee the standard treatment of children with leukemia.Methods A total of 110 children with acute lymphocytic leukemia were included in the study,who were treated in the Department of Hematology Oncology of Children′s Hospital Affiliated to Chongqing Medical University from Jan.2015 to Apr.2018.Descriptive analysis was used to analyze the direct medical costs of these children and investigate the main compositions.Results During the standard treatment of children with acute lymphocytic leukemia,the direct medical costs were mainly composed of outpatient costs (averaged 23 597 CNY),one-day chemotherapy costs averaged 23 216 CNY,hospitalization costs averaged 158 152 CNY.In the total cost of children with ALL,drug costs,treatment costs and laboratory test costs accounted for a large proportion.The average reimbursement rate was 59.1%(93 415/158 152).The average hospitalization time of children with acute lymphoblastic leukemia was 131 days,and the average daily hospitalization cost was 1 207 CNY(158 152/131).Conclusion Reducing drug costs and standardized treatment and testing can reduce medical costs.Comprehensive and multi-angle support should be provided for the families of children with leukemia,in order to guarantee the treatment for these children.
