    Brief discussion about pediatric chronic tonsillitis base on theory of latent pathogenic

    Chronic tonsillitis is one of common diseases in paediatrics.In normal conditions the disease is hard to cure and effect normal healthy life and study of Children.Therefore,clinical pediatric workers often pay attention to the effective prevention and treatment of this disease. Professor Chen Zhu proposes that base on the theory of latent pathogenic discuss about disease cause and mechanism of pediatric chronic tonsillitis,and considers that pediatric chronic tonsillitis and constitution of children are closely related.Latent pathogenic is an important pathogenesis of the disease. Latent evil make pediatric chronic tonsillitis happened which phlegm, heart and stasis remain dormant in the tonsil biosocially on the premise of deficiency body. The attack of this disease are "spontaneouse attack by latent pathogen"and"the disease induced by the pathogen once again".General therapeutic principle is opening onto pleurodiaphragmatic interspace and strengthening vital qi.However,flexibly grasp the degree between dispelling evil and reinforcing healthy qi that is the key to cure the disease, in the clinical treatment. The therapeutic principle of latent phlegm pathogen is expectorant by opening onto pleurodiaphragmatic interspace and dispersing swelling by strengthening spleen. The therapeutic principle of latent heat pathogen is the excess syndrome by clearing heattoxicity and the cases of deficiency by nourishing yinclearing heat. The therapeutic principle of latent stasis pathogen is activating blood removing and dispersing swelling by dredging collaterals.

    Application of acupuncture in the treatment of salivation in children with cerebral palsy

    Salivation is one of the serious complications in children with cerebral palsy, which western medicine believes is mainly caused by oropharyngeal motor dysfunction. At present, the treatment of this disease mainly includes drugs, rehabilitation and surgery, and the clinical efficacy is not satisfactory; Acupuncture treatment is simple, inexpensive, effective and has no side effects. However, there are various selection of acupoints and lack of normative standards at present. This paper analyzes and summarizes the different mechanism of acupoint selection and acupoint matching methods in acupuncture treatment for this disease.

    Randomized, doubleblind, parallelcontrolled, multicenter clinical study of Xiaoer Jingxing Zhike granules in the treatment of children with acute bronchitis with coldinduced fever syndrome

    Objective  To further evaluate the effectiveness and safety of Xiaoer Jingxing Zhike granules in the treatment of pediatric acute bronchitis with coldinduced fever syndrome.
    Methods  A randomized, parallel-controlled, double-blind, multicenter clinical trial design was used. A total of 479 patients with pediatric acute bronchitis with coldinduced fever syndrome were selected from 7 research centers. They were divided into experimental group and control group. The duration of treatment was 5 days. Symptoms and signs and comprehensive curative effect evaluation were taken as the main evaluation indicators for effectiveness, and adverse event/adverse reaction incidence and safety detection were taken as the main evaluation indicators for safety.
    Results In terms of the total effective rate of disease curative effect, the experimental group(87.71%,307/350) was better than the control group(27.12%,32/118).In terms of the total effective rate of TCM symptoms, the experimental group (87.71%,307/350) was superior to the control group (28.81%,34/118).For the evaluation of the efficacy of disease and the total effectiveness of TCM symptoms, the difference between the two groups was statistically significant(P<0.05). In terms of safety, Adverse reactions(mild allergy) occurred in 1 case in the experimental group, the incidence rate being  0.28%.
    Conclusion The results of the phase III clinical trial further prove that Xiaoer Jingxing Zhike granules is safe and effective in the treatment of pediatric acute bronchitis with coldinduced fever syndrome.

    Progress in diagnosis and treatment of adenoid hypertrophy in children by TCM and western medicine

    Adenoid hypertrophy in children refers to the pathological enlargement of children's adenoids, which is a common and frequently-occurring disease among children. In traditional western medicine oral drugs and surgery are the main treatment, and the therapeutic effect is not ideal. In recent years, TCM symptomatic therapy and TCM external therapy for adenoid hypertrophy in children have achieved good efficacy, which is accepted by more children and their parents for its low risk and few adverse reactions. Based on the current clinical situation, this paper summarizes the progress in the diagnosis and treatment of adenoid hypertrophy in children by western medicine and traditional Chinese medicine, in order to provide theoretical basis for the diagnosis and treatment of adenoid hypertrophy in children by combining traditional Chinese with western medicine.

    Current research on application of micro-implant anchorage in orthodontic treatment for children

    Orthodontic micro-implants, as an absolute anchorage, has the advantages of smaller trauma, better stability, higher comfort, as well as less dependence on the patients, which promotes its rapid development. This paper introduced the current situation and research progress in clinical application of micro-implants anchorage in orthodontic treatment for children and the concept of safe zone. At last, the parameters that affect the success rate and initial stability had been analyzed, aiming to comb the lines of thinking and methods for the researches on microimplant anchorage in orthodontics and provide reference for its further research and clinical application.

    Advances in diagnosis and treatment of Kawasaki disease

    Kawasaki disease is a systemic vasculitis involving small and medium-sized blood vessels, also known as mucocutaneous lymph node syndrome. Kawasaki disease is an acute vascular disease in children. Cardiovascular damage is the most serious complication of Kawasaki disease. The etiology of Kawasaki Disease is still unclear, and the pathogenesis is difficult to explain with a theory. So far, the diagnosis of KD is still an excluded clinical diagnosis. The application of Z value of coronary artery diameter is very important for the diagnosis, management and prognosis assessment of KD. Early high-dose intravenous immunoglobulin(IVIG) combined with aspirin therapy can effectively inhibit the immune inflammatory response, but some children present with IVIG resistance. As Kawasaki disease continues to be studied, new treatments are emerging, including TNF antagonists, methotrexate, clopidogrel, dipyridamole, rituximab, plasmapheresis, and combination of traditional Chinese medicine. This article summarizes the progress in diagnosis and treatment of Kawasaki disease in children.

    Research progress in mild gastroenteritis with benign convulsions in infants and young children

    Mild gastroenteritis with benign convulsions in infants and young children is a common clinical disease and most of the children recover well. At present, there is no clear conclusion on the etiology and pathogenesis of the disease, and the researches on viral infection and immune dysfunction are now more popular. There is a unified opinion on the clinical characteristics and diagnosis of the disease, and clinical treatment mainly includes symptomatic treatment and anticonvulsant treatment.

    Analysis of vitamin D and water-soluble vitamin levels in children with autism spectrum disorder

    Objective To investigate the relationship between autism spectrum disorders and blood levels of Vitamin D and water-soluble vitamins.
    Methods A total of 71 children with autism spectrum disorder who attended the outpatient clinic of the Children's Rehabilitation Department of our hospital and received rehabilitation training from January to December 2019 were recruited as ASD group. Age-and gender matched children undergoing routine physical examination in our hospital were recruited as control group. Their serum 25(OH)D3 level was determined by chemiluminescence method. The concentration of water-soluble vitamins in blood was determined by ultrahigh performance liquid chromatography-tandem mass spectrometry.
    Results The blood levels of 25(OH)D3, Vitamin B1, Vitamin B5, Vitamin B6, and Vitamin B9 in children with ASD were lower than those in the control group(P<0.05). No statistical difference was observed in Vitamin B2, Vitamin B3, Vitamin B7, Vitamin B12 or Vitamin C between the two groups(P>0.05). There were statistical differences in Vitamin B5, Vitamin B6, and Vitamin B9 in the blood of ASD children with different Vitamin D levels(P<0.05), but no statistical difference in Vitamin B1 levels(P>0.05). The level of Vitamin D was significantly positively correlated with Vitamin B9(P<0.05). No statistical correlation was observed between Vitamin D and Vitamin B1, Vitamin B5 or Vitamin B6(P>0.05).
    Conclusion The blood levels of Vitamin D, Vitamin B1, Vitamin B5, Vitamin B6 and Vitamin B9 in children with autism spectrum disorder are reduced. The levels of Vitamin B5, Vitamin B6, and Vitamin B9 are different in children with autism spectrum disorder with different levels of Vitamin D, and there is a significant positive correlation between the level of Vitamin D in the blood and the level of Vitamin B9. These results suggest that the children with autism spectrum disorder should have their levels of Vitamin D and water-soluble Vitamins monitored and take some vitamins according to the condition.

    Correlation analysis of bone metabolism indexes in early neonates

    Objective To explore the correlation of bone metabolism indexes in early neonates to reduce the occurrence of metabolic osteopathy.
    Methods A total of 121 early newborns were enrolled for the study, who were hospitalized in the Department of Neonatology of Zhuhai Hospital of Itegrated Traditional Chinese and Western Medicine from July 2018 to December 2019. The analysis of the correlation of the bone metabolism indexes were done, by detecting the serum concentrations of indexes of bone formation(ALP, PINP), indexes of bone absorption(β- CTX), indexes of bone nutrition(Ca2+, P, 25-(OH) D), and indexes of bone metabolism regulating hormone(PTH, CT).
    Results The results of PINP, which exceeded the maximum range of detection reagent, were all greater than1 200.00 μg/L,so it was not included in the analysis. The results of ALP, β-CTX, Ca2+, P, 25-(OH)D, PTH and CT were (190.08±59.99)U/L, (0.85±0.33)μg/L, (2.41±0.17)mmol/L, (2.35±0.90)mmol/L, (12.76±4.98)μg/L, (32.02±27.20)ng/L, and (5.35±5.05)ng/L, respectively. Correlation analysis showed that ALP was positively correlated with PTH; β-CTX was positively correlated with CT, while negatively correlated with P; Ca2+ was positively correlated with 25-(OH)D, and P was positively correlated with PTH, while negatively correlated with CT; PTH was negatively correlated with CT.
    Conclusion Bone metabolism is very active in neonates, and there is a complex correlation among bone metabolism indexes. Early newborns need to be supplemented with vitamin D and take enough calcium and phosphorus in order to increase bone mass reserve and reduce the occurrence of metabolic osteopathy.

    Study on the advantage of modified blue light lamp combined with enema with traditional Chinese medicine boiled powder

    Objective To explore the curative effect of modified blue light combined with enema with Chinese herbal medicine boiled powder in the treatment of neonatal fetal yellow(dampheat accumulating type).
    Methods From September 2013 to February 2020, a total of 120 children with neonatal pathological jaundice were admitted to the Zuoan Fengqiao Community Health Service Station of Handan Development Zone. They were randomly divided into observation group and control group with 60 cases in each group. The control group was treated with a singlesided jaundice treatment box and Yinzhihuang Oral Liquid was administered at the same time; the observation group was treated with a modified blue light box, and at the same time enema treatment with selfmade Chinese medicine prescriptions(yinchen, scutellaria, coptis, phellodendron, and Zhizi as the main prescription) was performed. After 3 and 5 days of treatment, the bilirubin levels and side effects of the two groups were compared.
    Results There was no significant difference in serum bilirubin decline between the two groups after 3 or 5 days of treatment(P>0.05). There were significant differences in side effects(P<0.05), such as conjunctivitis, diarrhea, insufficient breast milk, crying, etc. between the two groups after 3 and 5 days of treatment.
    Conclusion The modified blue light combined with enema with traditional Chinese medicine boiled powder and the traditional singlesided jaundice treatment box combined with oral Yinzhihuang oral liquid have the same curative effect in the treatment of fetal yellow(dampheat accumulating type), and it is easy to carry, which ensures that mothers and babies are in the same room and breastfeeding can be given, so it is conducive to the physical and mental health of mothers and babies with high safety.

    Effect of continuous blood purification combined with gamma globulin on clearance of inflammatory mediators and immune status in children with severe sepsis

    Objective To evaluate the effect and value of clinical treatment for severe sepsis children with gamma globulin and continuous blood purification(CBP). Methods The clinical data of 35 children with severe sepsis were collected from June 2018 to August 2019,and the patients were divided into groups according to the treatment method; control group (15 cases) was treated with CBP, while observation group(20 cases) was given continuous blood purification + gamma globulin treatment. The control of inflammatory reaction, improvement in immune function and prognosis in children were analyzed retrospectively. Results After treatment the levels of TNF-α, CRP and PCT in the observation group were lower than those in the control group(P<0.05);CD3+, CD4+, CD8+ and CD4+/CD8+ were significantly higher and NK cells were significantly lower in the observation group than in the control group(P<0.05).After treatment, APACHE Ⅱ score and SOFA score were lower in observation group than in the control group, and the average length of ICU stay was shorter than in the control group(P<0.05). Conclusion Treatment with CBP and gamma globulin for children with severe sepsis can effectively improve the effect of inflammation control, enhance the immune function of children, increase the total curative effect, and help improve the prognosis of children.

    Professor Qin Yanhong's experience in treating constipation in children

    During the diagnosis and treatment of constipation in children, Professor Qin Yanhong adhere to the etiology and pathogenesis, who combines the clinical observation results with the experience of experts throughout in the differentiated treatment. The differentiation is clear, comprehensive and flexible. Professor Qin Yanhong thinks that constipation in children is mainly in the type of intestinal dryness and fluid deficiency, so she has make Zengshui Xingzhou decoction. Meanwhile, she deals with the disease based on the root causes and manages both the symptoms and causes, paying attention to nursing and regulation. The method of relaxing bowel has been used throughout the treatment process. All of these have achieved significant effect in the treatment of children with constipation and in the prevention of its relapse

    Clinical analysis of 5 cases of StevensJohnson syndrome complicated with bronchiolitis obliterans and literature review

    The clinical features of 5 cases of StevensJohnson syndrome complicated with bronchiolitis obliterans(BO), chest CT, treatment and prognosis were analyzed.Three cases had a clear history of medication; 5 cases had cough and wheezing symptoms,4 of which had wet and dry murmurs before discharge, and had been followed up in the clinic after discharge. One patient developed cough and wheezing symptoms one week after discharge,and was diagnosed with BO 3 months after discharge. All 5 patients had patchy shadows,2 patients had uneven inflation combined with thickened tracheal wall; 2 patients had increased pulmonary transparency,and 2 patients had typical mosaic signs. Treatment and prognosis: 5 patients were treated with hormones and immunoglobulins in the acute phase,and 2 patients received plasma exchange. All 5 cases underwent fiber bronchoscopic lavage,1 case had obvious mucosal erosion,and 1 case was a little blocked in the lower trachea. Oral hormones were used in 5 cases after the diagnosis of BO,while nebulized hormones was used and azithromycin and montelukast were taken orally. One of the family members stopped the drug for 1 month. Four patients were followed up for 0.5~3 years. One patient's wheezing had completely improved,two patients had wheezing (successful deoxygenation) while moving,and one patient had some wheezing when being quiet. CT was significantly improved in 1 case. Combining BO in SJS patients affected longterm prognosis,and the clinical manifestations were cough and wheezing,and the improvement in clinical symptoms was earlier than CT

    Clinical observation on traditional Chinese herbal fumigationin in the treatment of children with influenza A

    Objective  To observe the effect of foot bathing with traditional Chinese herbal fumigation in the treatment of children with influenza A.
    Methods  A total of 100 children with influenza A who were treated in our hospital from January 2018 to January 2019 were randomly divided into experimental group(n=50) and control group(n=50). The control group was treated with conventional western medicine, mainly oseltamivir for symptomatic treatment. In addition to the conventional western medicine treatment, traditional Chinese medicine fumigation for foot bathing was given to the experimental group. On the 1st,3rd,5th and 7th day of treatment, record the changes in body temperature, the interval time between taking antipyretic drugs, and the relief in symptoms. On the 3rd,5th and 7th day of treatment, record the seroconversion rate of virus nucleic acid. The length of hospital stay and the total expenses were calculated.
    Results  1)In curative effect: after 7 days of treatment, the total effective rate of experimental group was 96.0%(48/50), higher than that of the control group(88.0%,44/50)(P<0.05).(2)In the change of body temperature: the interval time of experimental group was higher(P<0.05).(3)In the relief of symptoms: On TCM symptom reduction rate(fever, chills, stuffy nose, runny nose, and red and swollen throat) was higher in the experimental group after treatment than in the control group(0.05) , but there was no significant difference in the reduction rate in cough between the two groups(P>0.05).(4)In seroconversion rate of influenza A virus nucleic acid: there was no statistical difference between the two groups on 3rd,5th or 7th day of treatment(P>0.05).(5)The length of hospital stay was shorter in experimental group than in control group, and the hospitalization expenses were also lower(P<0.05).
    Conclusion  The foot bathing with traditional Chinese herbal fumigation is effective in the treatment of influenza A. Compared with the treatment with oseltamivir alone, it can reduce the time of fever more effectively,improve the symptoms of influenza such as feeling cold, fever, stuffy nose and runny nose.

    Clinical analysis of 19 children with idiopathic pulmonary hemosiderosis

    Objective  To increase the diagnosis and treatment level of idiopathic pulmonary hemosiderosis.
    Methods  The clinical data of 19 children diagnosed with idiopathic pulmonary hemosiderosis from January 2013 to October 2019 were analyzed.
    Results  Among the seven cases treated with hormone alone, three cases had no relapse, and the other four cases relapsed during hormone reduction. Seven cases of children treated with hormone combined with azathioprine relapsed repeatedly during the period of hormone treatment alone, five of which had no relapse after adding azathioprine (in 1 case azathioprine was used for only one month, and the parents stopped using azathioprine by themselves), and two cases relapsed once after adding azathioprine. In addition, there were three cases of repeated attacks during hormone therapy alone, and then azathioprine was used, but in two cases it was stopped because of the obvious decrease of granulocytes, and in one case it was stopped because of the serious vomiting and diarrhea reaction; the three cases were all treated with hormone combined with hydroxychloroquine, and the number of relapses and side effects were reduced after adding hydroxychloroquine. In the remaining two cases, one patient was diagnosed with juvenile idiopathic arthritis and Sjogren's syndrome one year after the diagnosis of IPH; in the treatment, hormone combined with methotrexate was used to control the disease and reduce the recurrence; the other patient was diagnosed with nephritis syndrome because of hematuria and proteinuria. He was treated with hormone combined with tacrolimus for one year. He still had hemoptysis and hematuria and proteinuria. After that, he was treated with hormone combined with leflunomide for more than half a year in another hospital. He had no attack of hemoptysis or hematuria or proteinuria.
    Conclusion  For children diagnosed with IPH, if hormone therapy alone or hormone combined with azathioprine is not effective or if the patient 〖JP2〗can not tolerate side effects, hormone combined with hydroxychloroquine can be considered for treatment

    Effects of Intensive Therapy Program on motor function of children with spastic diplegic cerebral palsy

    Objective To observe the effect of Intensive Therapy Program(ITP) on balance function and gross motor function in children with spastic diplegic cerebral palsy. Methods A total of 30 children with spastic diplegic cerebral palsy admitted to the Third Hospital of the Jiamusi University from December 2018 to December 2019 were selected and stratified according to age and gross motor function classification system (GMFCS) level, the patients in each level were randomly divided into control group and observation group with 15 cases. The Control Group was treated with routine physical therapy combined with omnidirectional dynamic correction clothing. ITP was applied in the observation group, including: omnidirectional dynamic correction clothing and omni directional dynamic sports equipment. Both groups were treated for 3 months. Peabody Developmental Motor Scale(gesture part), Berg Balance Scale (BBS) and Gross Motor Function Measurement(GMFM-88) were used to assess posture control ability, balance function and gross motor function before and after treatment. Results Peabody Posture Score, BBS score and GMFM88 score of the two groups were significantly higher after treatment, and they were higher in the observation group than in the control group(P<0.05). Conclusion The application of Intensive Therapy Program is helpful, to some extent, to improve the gross motor function of children with spastic diplegic cerebral palsy.

    Clinical value of ultrasonography in the early diagnosis of idiopathic precocious puberty in girls

    Objective  To explore the clinical value of ultrasonography in the early diagnosis of idiopathic precocious puberty in girls.
    Methods  Fiftyseven girls suspected with idiopathic central precocious puberty admitted to our hospital from March 2018 to October 2019 were selected as the observation group, and 57 healthy girls at the same age range were selected as the control group. Both groups were given breast and pelvic highfrequency ultrasonography and relevant hormone level examination.
    Results (1) Breast highfrequency ultrasonography showed: the length and thickness of the observation group in the peripheral gland echo area and the central hypoecho area of the breast were higher than those of the control group, and the difference was statistically significant (P<0.05). (2) Pelvic highfrequency ultrasonography showed: the observation group was higher in ovarian volume, anterior and posterior diameter of the ovary, transverse diameter of the ovary, long diameter of the ovary, and follicle diameter than the control group, and the difference was statistically significant (P<0.05). (3) Examination of relevant hormone levels showed: the levels of estradiol, follicle stimulating hormone, and luteinizing hormone in the observation group were higher than those in the control group, and the difference was statistically significant (P<0.05). Conclusion  Color Doppler ultrasonography has high clinical application value in the diagnosis of girls with idiopathic central precocious puberty, with clear diagnostic criteria and relatively mature and stable technology.

    Clinical observation on Yiqi Jianpi Fanggan decoction in auxiliary treatment of recurrent respiratory tract infection in children

    Objective To evaluate the clinical effect of routine western medicine combined with Yiqi Jianpi Fanggan decoction in the treatment of repeated respiratory tract infection in children. Methods The 82 children with recurrent respiratory tract infection who were treated in our hospital from February 2018 to June 2019 were divided into two groups based on treatment methods,40 children treated with conventional western medicine were included as control group, and 42 children treated with western medicine plus Yiqi Jianpi Fanggan decoction as observation group. The TCM syndrome score, time of symptom relief and improvement in immune function were analyzed retrospectively. Results The TCM syndrome scores of the observation group after treatment were significantly lower than those of the control group(P<0.05).The total effective rate was 92.86%(39/42) in observation group, which was significantly higher than that in control group(75.00%,30/40)(P<0.05).The time in the relief of fever, cough and red eyes was significantly shorter in the observation group than in the control group(P<0.05).CD8+ in the observation group was significantly lower than in the control group, and CD3+ and CD4+ in the observation group were significantly higher than in the control group(P<0.05). Conclusion On the basis of conventional western medicine, Yiqi Jianpi Fanggan decoction can obviously improve the effect of relieving clinical symptoms and the total curative effect, and effectively enhance the immunity function of the body.

    Effects of cognitive function training combined with core muscle stability training on improving cognitive function and limb function in children with severe craniocerebral injury

    Objective To analyze the effects of cognitive function training combined with core muscle stability training on improving cognitive function and limb function in children with severe craniocerebral injury. Methods According to the desire of the family members, 102 children with severe craniocranial injury admitted to our department from May 2018 to February 2019 were divided into groups:50 children in the control group received cognitive training, and 52 children in the experimental group received core muscle stability training. After 24 weeks of intervention, the cognitive function and upper and lower limb function of the two groups of children were compared. Results The scores in orientation, visual perception, spatial perception, motion application, visual and motor organization and thinking operation in cognitive functions of the experimental group were higher than those of the control group, and the differences were statistically significant (P<0.05); the upper limb function score and lower limb function score were higher than the control group, and the difference was statistically significant (P<0.05). Conclusion Cognitive function training combined with core muscle stability training can improve the cognitive function and limb function of children with severe craniocerebral injury, which is of great significance in improving children's autonomous living ability.

This paper reports a child diagnosed with this disease, which was treated with integrated traditional Chinese and western medicine. The disease was controlled and drug was stopped successfully, and the treatment achieved good results.

    Objective To study the medication laws of Chinese medicine in the treatment of allergic rhinitis in infants in the past 20 years. Methods The clinical research literatures on Chinese medicine treatment for allergic rhinitis in infants were collected from China Academic Journal Fulltext Database, Chinese Science and Technology Journal Database and Wanfang Data Knowledge Service Platform. The data were extracted from the literatures that meet the screening criteria. Descriptive statistics was made on the frequency of drug use, and the effect, classification, nature and taste of the drugs and the meridians affected, and then cluster analysis and association analysis were performed. Results The most commonly used drugs in the treatment of infants with allergic rhinitis were exteriorreleasing drugs and tonic medicines, mainly those dispersing windcold and tonifying Qi. The medicines most commonly used were warm in nature and pungent, sweet and bitter in taste, and the meridians that were affected were those of lungs, spleen, stomach, liver and heart. Four cluster prescription were formulated and they were C1(Atractylodes macrocephala, Astragalus, Fangfeng),C2(Xanthium sibiricum, Radix Angelicae dahuricae, Xinyi, Chuanxiong, Asarum),C3(Wumei, Schisandra, Guizhi, ephedra),C4(Poria, Codonopsis,Licorice). Conclusion The summary of the medication laws in the treatment of infants with allergic rhinitis shows that in clinical treatment Yupingfeng powder combined with Xanthium powder(modified) is commonly used, and differentiation medication is performed based on the relevant clinical symptoms.

    Effect of guided education combined with core muscle stability training on fine motor function and balance ability of children with spastic cerebral palsy

    Objective To analyze the influence of guided education combined with core muscle stability training on fine motor function and balance ability of children with spastic cerebral palsy(SCP). Methods Totally 125 children with SCP admitted to our department from October 2017 to October 2019 were selected as study subjects, and were randomly divided into control group (62 cases) and observation group (63 cases). The control group was given core muscle stability training, while the observation group was given additional guided education. After 3 months, the children in the two groups were compared in terms of foot dorsal flexion angle, fine motor function and balance ability before and after training. Results After treatment, the observation group had lower dorsal foot flexion angle than the control group, and the difference was statistically significant(P<0.05). The scores in fine motor function and balance ability of SCP children in the observation group were higher than those in the control group after treatment, and the difference was statistically significant(P<0.05). Conclusion Stability training of core muscle can correct abnormal posture and muscular tension of children with SCP, improve limb movement ability and balance ability. Combined with guided education, it can improve the awareness of families of children with SCP on rehabilitation training, and improve the fine motor ability of children with SCP, which is of high value in promoting children's early integration into the society.

    Evaluation of efficacy and safety of Jianpi Lishi decoction in treating children with diabetes complicated with acute renal failure

    Objective To investigate the clinical effect and safety of Jianpi Lishi decoction in children with diabetes and acute renal failure. Methods A total of 100 children with diabetes mellitus and acute renal failure admitted to our hospital from February 2016 to March 2019 were randomly divided into 2 groups: 50 cases in the control group and 50 in observation group. Both groups were given oral hypoglycemic or subcutaneous injection of insulin, irbesartan, and angiotensinconverting enzyme inhibitors for treatment, and the observation group was additionally given Jianpi Lishi decoction, both being treated for 8 weeks. The clinical effects, TCM symptom scores, the fasting plasma glucose(FPG), 2h plasma glucose(2hPG) and glycated hemoglobin(HbAlc) levels, and renal function indexes were compared, and the safety of medication was observed. Results The total effective rate of the observation group was 94.00%(47/50), which was significantly higher than that of the control group(78.00%,39/50), the difference being statistically significant(P<0.05).After 4 weeks and 8 weeks of treatment, the TCM symptom scores of the observation group and the control group were significantly reduced, and the observation group was lower than the control group, the difference being statistically significant(P<0.05); after 4 weeks and 8 weeks of treatment, the levels of FPG, 2hPG and HbAlc in the two groups were significantly reduced, and the observation group was lower than the control group, and the difference was statistically significant(P<0.05). At the 4th and 8th week of treatment, the Cr and BUN levels of the two groups decreased significantly, and the observation group was significantly lower than the control group; the GFR level of the two groups increased, and the observation group was significantly higher than the control group, the difference being statistically significant(P<0.05). No obvious adverse drug reactions were seen in both groups. Conclusion Jianpi Lishi decoction combined with western medicine is more effective in treating children with diabetes and acute renal failure, and blood sugar control better, which can improve the clinical symptoms of traditional Chinese medicine in children and protect the kidney function of children. It is safe and reliable, and worthy of promotion.

    Effect of breastfeeding support intervention donated by breast milk bank on maternal and infant prognosis of highrisk pregnant women

    Objective To explore the effect of breast milk donation (DHM) feeding support intervention on the prognosis of mothers and infants of highrisk pregnant women. Methods The clinical data of 120 cases of highrisk pregnant women admitted to our hospital from September 2017 to September 2019 were retrospectively analyzed. According to whether the family members agreed to give DHM to infants, the mothers and infants were divided into observation group (64 cases) and control group (56 cases). The observation group received DHM feeding support intervention, while the control group received formula feeding. The incidence of breast distention within 3 days after delivery was compared between the two groups. The lactation rate of postpartum 1,2 and > 3 days, the rate of pure breastfeeding within 6 weeks after delivery, and the neonatal development after 7 days were compared between the two groups. Results (1) Within 3 days after delivery, the incidence of breast distention in the observation group was 12.50%(8/64), which was lower than 28.57%(16/56) in the control group(P<0.05). (2) After using DHM, the number of lactation women in the observation group was higher than that in the control group on the first day after delivery. The number of lactating women in the observation group two days after delivery and the number of nonlactating women 3 days after delivery in the observation group were lower than those in the control group(P<0.05). (3) The rate of pure breastfeeding in the observation group was 75.00%(48/64), which was higher than 57.14%(32/56) in the control group, the difference being statistically significant(P<0.05). (4) The rate of normal defecation in the observation group was higher than that in the control group, the rate of abdominal distension was lower, and the growth of body weight was higher than that in the control group, and the difference was statistically significant (P<0.05). Conclusion DHM feeding intervention support can help highrisk pregnant women to reduce milk distension, promote early lactation, help mothers implement early exclusive breastfeeding, reduce the incidence of abnormal stool and abdominal distension, and promote the growth of neonatal body weight.

    Clinical and genetic analysis of XiaGibbs syndrome

    Objective To investigate the clinical symptoms and gene variation of XiaGibbs syndrome(XGS) caused by ADHC1 gene mutation. Methods The clinical data of 2 children with XGS caused by the ADHC1 gene mutation and the results of whole exome sequencing(WES) were analyzed and summarized retrospectively. The literatures were reviewed to collect XGS cases with confirmed genetic diagnosis, and the clinical phenotypes and gene mutation features were summarized. Results The male patient was 7 months and the female patient was 7 years and 11 months. Both patients had sport language retardation, muscle tension reduction, and brain MRI and EEG abnormalities. The male patient had an special facial features(binocular esotropia, low bridge of nose), skin hemangioma, patent foramen ovale, new nonsense mutation of ADHC1 gene found by WES[1-8c.4321(exon) C > T(p.Q1441X,163)]. The female had epilepsy, new frame shift mutation of ADHC1 gene found by WES[c.1122dupC (p.gly375fs)].Eight English literatures and two Chinese literatures were retrieved from the Pubmed, Wanfang and CNKI databases. There were 40 cases of AHDC1 mutation in total including the 2 cases. Most of them had comprehensive underdevelopment(97.5%), hypotonia(87.5%), special facial features(77.5%), and sleep apnea(42.5%). Dysgenesis of corpus callosum was common in craniocerebral imaging, and frame shift mutation(70%) was the most common gene mutation. Conclusion The main symptoms of XGS caused by AHDC1 gene mutation are underdevelopment, hypotonia, special facial features and sleep apnea, which can be diagnosed by gene detection based on clinical phenotypes.

    Lung is a delicate organ, and the postnatal growth relies on spleen. The relationship between lung and spleen is like mother and child, with gas and blood exchange. The children are weak in lung and spleen after birth, so they are likely to have the diseases related to lung and spleen. Their normal functions and the pathophysiological changes of them significantly affect the outcome and prognosis of the diseases in children. Professor Zhang Shiqing is a famous TCM physician in pediatric diseases, who is good at the diagnosis and treatment of diseases in lung and spleen. These diseases are characterized by much phlegm and dampness and being prone to coldness and heat. Prof. Zhang uses modified Erchen decoction to regulate lung and spleen. Erchen decoction is a basic prescription in the treatment of lung and spleen diseases, and is widely used in clinic.

    Influence of the time and method of collecting specimen in sputum culture on the quality of specimen in children with bronchopneumonia

    Objective To study the influence of different time and method of collecting specimen in sputum culture on the detection quality of specimen in children with bronchopneumonia. Methods A total of 150 children with bronchopneumonia were treated in our hospital from Feb. 2017 and Feb. 2019, and they were divided into control group(75 children) and observation group(75 children) by lottery. The specimen was taken by way of sputum aspiration in the observation group: the first time was on admission to hospital and the second time was at 2h fasting on the morning of the second day after admission; in the control group, the specimen was taken by cotton swab: the first time was right after admission to hospital and the second time was at 2h fasting on the morning of the second day after admission. The total number of strains detected out, the positive rate of strains, the return rate of specimen, and the accuracy rate were compared between the two groups. Results The total detection number and the positive rate of strains in the sputum specimens taken at different time points were higher in observation group than in control group, and the total detection number and positive rate on getting up were higher than at random time points in both groups, the difference being statistically significant(P<0.05); the return rate at different time points was significantly lower in observation group than in control group, and the rate on getting up was lower than at random time points in both groups(P<0.05); the accuracy rate was higher in observation group than in control group at different time points, and it was higher on getting up than at random time points in both groups(P<0.05).
