    Pharmacokinetics of loteprednol etabonate in beagle dogs through different routes of administration

    Objective
    To investigate the pharmacokinetics of loteprednol etabonate in beagle dogs through different routes of administration,and to provide reference for clinical medication safety.
    Methods
    Beagle dogs were given loteprednol etabonate eye drops(test preparation) or oral administration of loteprednol etabonate capsules(reference preparation). Concentration of PJ90 and PJ91, which were non active metabolite of loteprednol etabonate, was determined by HPLC method.
    Results
    PJ90 and PJ91 were not detected after administration of eye drops. After oral administration,the main pharmcokinetic parameters of PJ90 and PJ91 in the plasma were as follows: Tmax was (1.38±0.89)h and (1.17±0.90)h,ρmax was (0.35±0.05) mg/L and (0.24±0.13) mg/L,t1/2 was (7.11±3.76)h and (14.95±9.30)h,AUC0-t was (2.12±1.42) mg/(h·L) and (2.02±1.29)mg/(h·L),and AUC0-∞  was (2.81±1.83)mg/(h·L) and (3.97±3.23)mg/(h·L).
    Conclusion
    Loteprednol etabonate eye drops are safe.

    Clinical characteristics and endoscopic features of peptic ulcer in 320 children

    Objective
    To study the clinical characteristics and endoscopic features of peptic ulcer in 320 children.
    Methods
    A total of 4 541 children with digestive tract symptoms underwent electrogastroscopy in Shenzhen Children's Hospital from January 2014 to January 2017; 320 of them were diagnosed with peptic ulcer, 254 boys and 66 girls. Their age ranged from one to 14 years old, among whom 29 were under 3, 75 between 3 and 6, 162 between 6 and 12, and 54 aged 12 to 14 years. There were 218 cases of duodenal ulcer, 78 cases of gastric ulcer, and 24 cases of other ulcer. Totally 207 cases were H. pylori-positive and 113 were negative. The ulcer area was less than 1 cm in 143 cases, 1 to 2 cm in 145 cases, and >2 cm in 32 cases. The ulcer was complicated with hemorrhage in 107 cases, with pyloric obstruction in 33 cases, and with perforation in one case. The following items were analyzed in the children at different ages: the incidence of ulcer, the gender difference, the site of disease, the area and the situation of H. pylori infection.
    Results
    (1) The incidence rate in the children of school age(>6 to 12 years) was the highest(80.2%,130/162); duodenal ulcer occurred mainly in school-age children; gastric ulcer occurred mainly in preschool children. There was statistical difference in the incidence of peptic ulcer among children of different ages(P<0.05).(2)The ratio of boys to girls in children with peptic ulcer was 3.85∶1, which was 3.19∶1 in duodenal ulcer and 5∶1 in gastric ulcer. The incidence rate in boys was significantly higher than that in girls in the children with peptic ulcer(P<0.05).(3)The detection rate of H. pylori in children with peptic ulcer was the highest(64.7%,207/320), which was different in different types of peptic ulcer. The detection rate of H. pylori was the highest in children with duodenal ulcer(74.3%,162/218); there was statistical difference in the rate in different types of peptic ulcer(P<0.05).(4)There was statistical difference in the rate of H. pylori infection in the children of different ages with peptic ulcer(P<0.05).(5)The ulcer area in childhood peptic ulcer was mostly less than 2 cm, which was obviously larger in H. pyloripositive children than in H. pylorinegative ones, the difference being statistical(P<0.05).(6)The rate of complications in children with peptic ulcer was significantly increased; the incidence rate of hemorrhage and pyloric obstruction had statistical difference compared with H. pylorinegative children(P<0.05).(7)Duodenal ulcer tended to occur on the front wall, followed by the greater curvature; gastric ulcer was more likely to occur in gastric antrum followed by gastric angle.
    Conclusion
    Most of peptic ulcer in children is duodenal ulcer. Age and gender are obviously related to its incidence, which is the highest in schoolage and preschool children, and higher in boys than in girls. Peptic ulcer is closely related to H. pylori, which is one of the main causes of peptic ulcer in children. Clinically for the children with digestive tract hemorrhage or with anemia of unknown cause, electrogastroscopy should be performed to exclude peptic ulcer so as to reduce missed diagnosis and misdiagnosis.

    Objective:
    To analyze the medication experience of Prof. Xu Youjia in treating children with asthma at remitting stage based on Traditional Chinese Medicine Inheritance Support System V2.5(TCMISS V2.5).
    Methods:
    Collect Prof. Xu Youjia's 95 prescriptions of treating children with asthma at remitting stage. Analyze the data by unsupervised data-mining methods such as association rules and complex system entropy clustering, thus discovering the frequency of drugs used in prescription, and the association rules of those drugs.
    Results:
    A total of 95 samples of prescriptions were analyzed to determine the frequency of drugs used in prescriptions,and the characteristics of four natures and five flavors of drugs and the association rules of those drugs were determined.
    Conclusion:Prof. Xu Youjia's treatment for childhood asthma at remitting stage is base on lung, spleen and stomach, as well as the symptoms of phlegm and stagnation, and emphasized heavily on strengthening the spleen and keeping it well-functioning. As for the medication, herbal medicine with warm and mild character is preferred,and Liquorice,White atractylodes rhizome,Acorus gramineus soland,Perilla fruit,Fructus citri sarcodactylis,and Schisandra chinensis are mainly selected with reasonable modification according to the difference of syndromes.

    Objective
    To observe the effect on the ratio of sIgA in saliva and serum albumin(sIgA/ALB)when Fugan decoction combined with chiropractic therapy were applied to the children with recurrent respiratory tract infections(RRTIs) with spleen deficiency and liver prosperous syndrome.
    Methods
    From December 2015 to August 2016,200 cases of children with RRTIs in the Department of Pediatrics admitted to Zhongshan Hospital of Traditional Chinese Medicine were selected as the subjects,and they were randomly divided into the observation group and the control group, with 100 cases in each. Nineteen cases withdrew or stopped because of missing visits, including 8 cases in the observation group and 11 cases in the control group. The control group was treated with pidotimod, while the treatment group was treated with Fugan decoction and the chiropractic therapy. Both of them were treated for one month. The sIgA and ALB in saliva were detected before treatment and 3 months after treatment,and the clinical effects of the two groups were observed.
    Results
    The total effective rate of the observation group was 97.8%(90/92), which was significantly higher than that of the control group(80.9%,72/89), and the difference was statistically significant(P<0.05).The level of sIgA/ALB in the two groups was significantly lower before treatment than that after treatment and the difference was statistically significant(P<0.05).The level of sIgA/ALB in the observation group was significantly higher than that of the control group after the treatment, and the difference was statistically significant(P<0.05).
    Conclusion
    The Fugan decoction combined with chiropractic therapy can significantly improve the ratio of sIgA/ALB and the mucosal immune function in children.

    Objective
    To determine the changes of exhaled nitric oxide(FeNO) in children with cough variant asthma(CVA), and to explore the diagnostic value of FeNO in children's CVA.
    Methods
    Thirty cases of children with CVA admitted to Pediatric Department of Fushun Mining Administration General Hospital from January 2015 to October 2016 were set as CVA group; 30 cases of children with cough(PIC) after infection were selected during the same period and they were set as PIC group. The levels of FeNO and IgE were measured separately in two groups.
    Results
    The levels of FeNO and IgE were increased in both groups, The levels in CVA group were (33.23±16.29)×10-9mol/L, significantly higher than those of the PIC group[(19.30±9.87)×10-9mol/L], the difference being statistically significant(P<0.01). There was no significant difference in IgE levels between the two groups(P>0.05).
    Conclusion
    In the diagnosis of chronic cough in children, FeNO can be used as a diagnostic method for the diagnosis of CVA.

    Objective
    To observe the clinical curative effect of Chinese medicine fumigation formula in the treatment of children with HenochSchonlein purpura.
    Methods
    From January 2016 to January 2017, 60 children with HenochSchonlein purpura were treated at the Children's Medical Center of the Affiliated Hospital of Changchun University of Traditional Chinese Medicine, and they were randomly divided into observation group and control group with 30 cases each. The control group was given conventional treatment, and the observation group was given Chinese medicine fumigation treatment on the basis of the treatment for the control group: once a day, 30 to 40 minutes each time, 3 days as a course of treatment, and treatment lasting for 3 courses. The clinical effect of the two groups was observed; the relief time of skin purpura and joint swelling; and the number of relapses within 24 hours after treatment were also recorded.
    Results
    Ridit analysis showed that the effect in the observation group was better than in the control group, and the relief of joint pain was also better. There was fewer relapses within 24 hours in the observation group, and the difference was statistically significant(P<0.05).The time for purpura to disappear in observation group was (4.23±1.04)d, which was shorter than the control group,(7.87±1.80)d, and there was statistical difference(P<0.05).
    Conclusion:Traditional Chinese medicine fumigation formula has significant curative effect on children with HenochSchonlein purpura, especially in the rash subsidence and relief time of joint swelling and pain, and the method is simple, safe and noninvasive, which is worthy of clinical application.

    Application research of Kinesio taping in improving the respiratory function of children with cerebral palsy

    Objective
    To observe the effect of Kinesio taping combined with conventional breathing training on the respiratory function of children with cerebral palsy.
    Methods
    A total of 30 children with cerebral palsy were included, who were treated in Harbin Children's Hospital between Jan. 2016 and Jan. 2017,and they were divided into observation group(15 cases) and control group(15 cases) by random table method. Both groups received conventional breathing training, and the observation group was also treated by Kiesio taping. Before treatment and 8 weeks after treatment, the children of both groups were evaluated in terms of respiratory frequency and maximum phonation time(MPT).
    Results
    After 8 weeks of treatment, the respiratory frequency of the children in both groups was reduced compared with that before treatment, and it was reduced even more in the observation group, the difference being statistical(P<0.05).The MPT of both groups of children was higher than before treatment, and it was even higher in the observation group, there being statistical difference(P<0.05).
    Conclusion
    The Kinesio taping combined with breathing training can further improve the respiratory function of children with cerebral palsy and increase their speech ability.

    Risk factors of acute respiratory dysfunction in children with respiratory foreign bodies before operation

    Objective
    To investigate the risk factors of acute respiratory dysfunction in children with respiratory foreign bodies before operation.
    Methods
    A total of 100 children with respiratory foreign bodies and respiratory dysfunction were included as the research subjects(the observation group), who received treatment in Hunan Children's Hospital from Jan. 2011 to Dec. 2016. Another 100 children with respiratory foreign bodies but without respiratory dysfunction receiving treatment during the same period of time were chosen as the control group. Their clinical data were collected, and the risk factors of acute respiratory dysfunction were analyzed by single-factor and Logistic multi-factor regression analysis.
    Results
    Pneumonia, fever and mediastinal subcutaneous pneumatosis were the main risk factors based on the singlefactor and Logistic multi-factor regression analysis.
    Conclusion
    Pneumonia, fever and mediastinal subcutaneous pneumatosis are the main risk factors of acute respiratory dysfunction in children with respiratory foreign bodies before operation. Therefore, in the clinical treatment, the preventive strategies should be made according to the disease state and the risk factors of the children in order to reduce incidence of acute respiratory dysfunction before operation in children with respiratory foreign bodies.

    Research in the gastrointestinal symptoms of hand-foot-mouth disease treated with Saccharomyces boulardii and compound clove appetizing paste

    Objective
    To observe the clinical effect of Saccharomyces boulardii and compound clove appetizing paste in the treatment of gastrointestinal symptoms of hand-foot-mouth disease.
    Methods
    Totally 220 children with hand-foot-mouth disease hospitalized in Dalian Children's Hospital Affiliated to Dalian Medical University from June to September in 2016 were selected as the subjects. They were randomly divided into the observation group and the control group, 110 cases in each group. Both groups were treated with antiviral or combined antibiotics. The observation group was also treated with Saccharomyces boulardii and compound clove appetizing paste for 6 days. The incidence of gastrointestinal symptoms and adverse reactions were compared between the two groups.
    Results:Twelve cases of the control group and 14 cases of the observation group had severe complications of hand-foot-mouth disease(HFMD) and withdrew from the study. The incidence of inappetence, nausea, vomiting, abdominal pain, diarrhea, abdominal distension and dry stool was 65.3%(64/98), 42.9%(42/98), 46.9%(46/98), 34.7%(34/98), 36.7%(36/98)and 22.4%(22/98)respectively in control group, which was higher than that of the observation group[(29.2%,28/96),(18.8%,18/96),(12.5%,12/96), (14.6%,14/96),(13.5%,13/96) and (8.3%,8/96)].The difference was statistically significant(P<0.05).In the observation group, only 6 cases had redness of skin around the navel due to the compound clove appetizing paste, no rash or blister. Skin redness was relieved within 48 hours of termination of drug use, and no other adverse reactions occurred.
    Conclusion:Saccharomyces boulardii and compound clove appetizing paste can effectively reduce the incidence of gastrointestinal symptoms in children with hand-foot-mouth disease. The therapy is worth clinically recommending and applying.

    Objective
    To analyze the effect of montelukast combined with tanshinone on children with Henoch Schonlein purpura and its influence on serum inflammatory factors.
    Methods
    A total of 117 children with Henoch Schonlein purpura treated from January 2015 to February 2016 in Qi County Maternal and Child Health Planning and Child Care Service Center of Henan Province were selected as the study subjects, and they were randomly divided into the control group(58 cases) and observation group(59 cases). Both groups were treated with montelukast, and the observation group was given tanshinone intravenous drip, with 30 d as 1 course. The clinical efficacy, the improvement of serum inflammatory factors(interleukin 4, 6 and tumor necrosis factor alpha) and the incidence of adverse reactions were observed in both groups.
    Results
    The total effective rate of the observation group after treatment was 96.6%(57/59), which was significantly higher than that of the control group(82.8%,48/58), the difference being statistically significant(P<0.05). The levels of interleukin 4, 6 and TNF-alpha in both groups were significantly lower than those before treatment, and the difference was statistically significant(P<0.05). After treatment of 30d, the levels of interleukin 4, 6 and tumor necrosis factor alpha in the observation group were significantly lower than those in the control group(P<0.05). There was no significant difference in the incidence of adverse reactions between the 2 groups during the treatment period(P>0.05).
    Conclusion
    Montelukast combined with tanshinone can significantly improve the level of serum inflammatory factors in children with Henoch Schonlein purpura and the efficacy of treatment and it is safe.

    Objective
    To investigate and analyze the clinical efficacy and safety of integrated traditional Chinese and Western medicine in the treatment of allergic purpura in children.
    Methods
    A total of 58 children with allergic purpura were selected in our hospital from June 2012 to December 2016, and randomly divided into two groups. The 29 cases simply treated with western medicine were as control group, and 29 cases treated with integrated traditional Chinese and western medicine were as observation group. The control group received intravenous drip of Cimetidine injection at 40 mg/(kg.d), once a day; oral Loratadine Syrup was also used at 5 mL each time, once a day. The course of treatment was 1 to 2 weeks. Besides the treatment for control group, the observation group was given Sanhuang Qingxue Yin, one dose a day, to be taken separately in the morning and evening, which lasted for 1 to 2 courses, one week as a course. The clinical efficacy, time for symptoms to disappear and the complications during treatment of the two groups were compared.
    Results
    The total effective rate of the observation group was 96.5%(28/29), which was significantly higher than that of the control group(69.0%,20/29)(P<0.05). The time for swelling pain, rash and digestive tract symptoms to disappear in the observation  group was shorter than that in the control group(P<0.05).Kidney damage occurred in both groups, but the incidence of complications in both groups was not statistically difference(P>0.05).
    Conclusion
    The treatment for children with allergic purpura by combination of Sanhuang Qingxue Yin can effectively improve the clinical efficacy.

    Effect of low molecular weight heparin in different doses on the levels of serum CysC, MMP-9 and β2-MG and the coagulation function in children with Henoch Schonlein purpura nephritis

    Objective
    To explore the effect of low molecular weight heparin in different doses on the levels of serum CysC, MMP-9 and β2-MG and the coagulation function in children with Henoch Schonlein purpura nephritis.
    Methods
    Totally 94 children with Henoch Schonlein purpura nephritis treated in our hospital from November 2015 to April 2017 were selected, and were divided into group A (small dose) and group B(large dose) randomly, 47 cases in each group. Both groups were given basic treatment, and on the basis of that, group A were given subcutaneous injection of 50~100 U/(kg·d) low molecular heparin calcium, and group B were given subcutaneous injection of 150 U/(kg·d) low molecular heparin calcium. Clinical efficacy, the levels of serum CysC, MMP-9 and β2-MG, inflammatory factors, blood coagulation indexes and adverse reactions were compared between the two groups.
    Results
    Total effective rate of group A was 93.62%(43/47), and there was no significant difference when compared with that of control group(95.74%,44/47)(P>0.05). Serum CysC, MMP-9, and β2-MG levels were significantly lower than those before treatment in the two groups(P<0.05), but there was no significant difference between the two groups(P>0.05). IL-4, IL-6 and IL-8 levels were significantly lower than those before treatment in the two groups(P<0.05), with little difference between the two groups(P>0.05). APTT, Fig and D-D levels were significantly lower than those before treatment in the two groups(P>0.05), and PT level was significantly higher than that before treatment, but there was no significant difference between the two groups(P>0.05). The incidence of adverse reactions in group A (25.5%,12/47) was significantly lower than that in group B (4.3%,2/47)(P<0.05).
    Conclusion
    Satisfactory results can be achieved in treatment of children with Henoch Schonlein purpura nephritis with different doses of low molecular weight heparin, and subcutaneous injection of low molecular weight heparin is of higher safety.

    Clinical effect observation on auxiliary treatment with massage for 66 cases of pediatric diarrheal disease

    Objective
    To discuss the clinical effect of western medicine combined with the traditional Chinese massage in treating pediatric diarrheal disease.
    Methods
    Select 132 cases of children with diarrhea as the research objects; the children were randomly divided into control group(n=66) and observation group(n=66). The patients of both groups were treated with conventional western medicine, and the traditional Chinese massage as an assistant treatment was applied to the children of observation group, with 3 days as a course of treatment. The two groups were compared in the recovery time of stool features, antidiarrheal time and clinical curative effect.
    Results
    The total effective rate of the observation group was 95.5%(63/66), which was higher than the 81.8%(54/66) of the control group, and the difference was statistically significant(P<0.05). The antidiarrheal time and the recovery time of stool features of observation group was significantly shorter than the control group, and the difference was statistically significant(P<0.05).
    Conclusion
    The combination of western medicine treatment with the traditional Chinese massage can obviously improve the clinical effect in treating pediatric diarrheal disease, reduce side effects and adverse reactions, and effectively improve the disease cure rate, which is worth wide application clinically.

    Change of Th1/Th2 in children with recurrent respiratory infection and its relationship with IgA

    Objective
    To study the serum IgA in the effect of Th1/Th2 migration in children with recurrent respiratory infection.
    Methods
    A total of 45 children with recurrent respiratory infection treated in Hexian Memorial Hospital from Jan. 2014 to Jan. 2017 were selected as the observation group, and 60 children with upper respiratory infection treated in our hospital during the same period were included as the control group(with one-year follow up and only one to 4 times of infection in one year).Flow cytometry was used to determine the ratio of Th1(CD3+CD8-IFN-γ+IL-4-)and Th2(CD3+CD8-IL-4+IFN-γ-);ELISA was adopted to determine serum level of IgA, and Pearson linear correlation was used to analyze their correlation.
    Results
    Blood routine indexes were of no statistical difference between the two groups(P>0.05).The children with recurrent respiratory infection at acute stage had Th1/Th2 migration, characterized by Th1 decline and Th2 increase, and at the same time IgA levels increased too(P<0.05).With the increase of Th2, IgA levels increased too. Correlation analysis showed that IgA and Th2 changes were positively correlated(r=0.745,P=0.001).
    Conclusion
    Children with recurrent respiratory tract infections at acute stage have Th1/Th2 migration, and Th2 is predominant, which is positively related to the serum IgA levels. The immune imbalance is an important factor of the onset of the disease, which should be attached importance to clinically.

    Application of ARIMA model in predicting the case number of severe acute respiratory tract infection in Pediatric Department of Chengguan District Hospital, Lanzhou

    Objective
    To establish an ARIMA model for inpatients with severe acute respiratory infection in Pediatric Department of Chengguan District People's Hospital of Lanzhou, and to judge the modeling result and make predictions.
    Methods
    The number of cases of severe acute respiratory infections from January 2010 to June 2016 in the People's Hospital of Chengguan District of Lanzhou City was selected as the modeling part of the ARIMA model. From July to December 2016, the data from July to December 2016 was used as a model validation part to obtain the model's fitting and prediction effect.
    Results
    From January 2010 to June 2016, the number of severe acute respiratory infections admitted to the Department of Pediatrics of Chengguan District People's Hospital of Lanzhou City was lower in January and February, and higher in June and July, showing a seasonable trend. ARIMA(1,1,2)×(2,1,0)12 model fitting effect was good. The average absolute percentage error of the model verified on the test set was 31.30;the root-mean-square error was 11.49.Taking the data from July to December 2016 as the verification set, the relative error of the verification result was 22.19%.
    Conclusion
    The established ARIMA(1,1,2)×(2,1,0)12 model has a good fitting effect on the test set. The prediction accuracy on the validation set is high. This model has been applied in the Pediatric Department of our hospital to predict the case number of sever acute respiratory tract infection and to send alarm, which has achieved good results.

    Application of neonatal critical case scoring method in neonatal respiratory distress syndrome

    Objective
    To explore the applicability and clinical significance of neonatal critical case scoring(NCIS) in neonatal respiratory distress syndrome (NRDS).
    Methods
    A total of 70 cases of NRDS child patients admitted to the Affiliated Hospital of Medical College of Yangzhou University from January 2015 to December 2016 were selected and divided into early preterm infant group(gestational age<34 weeks, n=46) and late preterm infant group(gestational age between 34 and 36+6 weeks, n=16) and fullterm infant group(gestational age≥37 weeks, n=8) according to the gestational age. All children were scored by the 10 items of NCIS method with a total score of 100 points. The initial scoring was performed within 24h after entering NICU; the scoring was performed every other day, and the most critical score was taken; the patients with aggravation were scored immediately and the taken most critical score was taken; the patients with increasing aggravation were scored every day until the day of death. The children with a total score>90 points were non-critical, 70 to 90 points, critical, and<70 points, extremely critical. The patients with one or more than one item of single index might also be judged as neonatal critical cases.
    Results
    The incidence rate of non-critical cases was low among the three groups (P>0.05). The incidence rate of critical cases was high in early preterm infant group and late preterm infant group, but the difference was not statistically significant compared with that in full-term preterm group(P>0.05). The incidence rate of extremely critical cases was highest in the full-term infant group, and there was a significant difference compared with early preterm infant group and late preterm infant group(P<0.05). There were no deaths in non-critical cases; the death rate in extremely critical cases was high among the three groups, and there was a significant difference compared with non-critical cases and critical cases(P<0.05).
    Conclusion
    NCIS method is helpful to assess the severity and development trend of NRDS, and it can also be used to predict the risk of death and provide a basis for continuing treatment and communication with family members.

    Objective
    To observe the clinical characteristics and treatment effects of paroxysmal supraventricular tachycardia in children.
    Methods
    A total of 100 children with paroxysmal supraventricular tachycardia were chosen as the research subjects, who were treated in Hunan Children's Hospital from May 2015 to April 2017. The children were divided into 4 groups according to different treatment:32 in physiotherapy group(A), 40 in drug therapy group(B), 12 in drug therapy+electroversion group(C), and 16 in drug therapy+radiofrequency ablation group(D).Group A was managed by stimulating nervi vagus to normalize the heart rate. Group B was treated with propafenone, amiodarone and digoxin. Group C was treated with electroversion in addition to the drug therapy, and group D was managed by radiofrequency ablation besides drug therapy. Observe the conversion time of heart rate and the success rate, and the treatment results of different therapies.
    Results
    The conversion time of heart rate was the longest in group A, followed by group B, group C and group D in turn; there was statistical difference among the four groups(P<0.05). The success rate was the highest in group D, followed by group C, group B and group A in turn, and the difference was statistically significant(P<0.05). The conversion rate by propafenone was the lowest, and amiodarone the highest(P<0.05).
    Conclusion
    The characteristics of paroxysmal supraventricular tachycardia are significant in children, and there are various treatments. Clinically, the treatment of drug+radiofrequency ablation is preferred, followed by drug+electroversion. The result of pure drug therapy or physiotherapy is not satisfactory.

    Objective
    To analyze the efficacy of Erzhu Antidiarrheal paste combined with infantile massage in the treatment of pediatric diarrhea and its influence on immune index.
    Methods
    Eighty children with diarrhea, who were hospitalized in Pediatric Department from May 2016 to February 2017 in Fangcheng County Maternal and Child Healthcare Hospital of Henan Province, were selected as study subjects. They were randomly divided into observation group and control group with 40 cases each. Both groups were given oral montmorillonite powder, and the observation group was also treated with Erzhu Antidiarrheal paste combined with massage. There were 5 days in a course of treatment, and the treatment lasted for 2 courses. Observe the efficacy and symptom relief time of the two groups; immunodiffusion method was used to detect IgG and IgA levels in children; immunofluorescence was used to detect CD8 and CD4 levels in children; calculate CD4/CD8.
    Results
    The total effective rate of observation group was 92.5%(37/40), which was significantly higher than that of control group(75.0%,30/40),the difference being statistically significant(P<0.05).The relief time of vomiting, fever and abdominal pain, and the time of bowel movements recovery were shorter in the observation group than those in the control group, the differences being  statistically significant(P<0.05). After treatment, the IgG, IgA, and CD4/CD8 in the observation group were significantly higher than those in the control group, and the differences were statistically significant(P<0.05).
    Conclusion
    The use of Erzhu Antidiarrheal paste combined with infantile massage in addition to conventional western medicine can improve the therapeutic effect on pediatric diarrhea and improve the immune function of children. It is worthy of promotion.

    Objective
    To analyze the effect of sildenafil combined with Calsurf in the treatment of neonatal with persistent pulmonary hypertension.
    Methods
    A total of 200 PPHN neonates were selected as the research subjects, who were treated in Linxia State People's Hospital from April 2011 to February 2016; the 100 neonates treated from April 2011 to April 2014 were in the control group, while the 100 from May 2011 to February 2016 were included in the observation group. Both groups received positive treatment for primary diseases and support of mechanical ventilation, and intravenous dripping of 20% magnesium sulfate; in addition, the observation group was treated by sildenafil combined with Calsurf. The treatment lasted for 3 days; observe the clinical effect, measure the pulmonary pressure by Doppler ultrasound, and detect the related laboratory indexes in the two groups.
    Results
    The total effective rate of observation group was 95.0%(95/100), significantly higher than that of control group(65.0%,65/100). The pulmonary pressure after treatment was significantly lower in the observation group, and there was statistical difference(P<0.05).The related blood gas indexes after treatment in the observation group were significantly better than those in the control group(P<0.05).
    Conclusion:Calsurf combined with sildenafil for neonates with persistent pulmonary hypertension can obviously improve the clinical symptoms of the neonates; after treatment, the blood-gas-related indexes are decreased, and the Doppler ultrasonography shows a decrease in pulmonary pressure, which promotes the improvement of the disease.

    Febrile seizures in children: analysis of risk factors and prognosis

    Objective
    To analyze the relationship between risk factors and prognosis of febrile seizures in children.
    Methods
    Totally 120 cases of children with febrile seizures who were admitted to Hunan Children's Hospital from August 2013 to August 2015 were studied. The relationship of prognosis with first-onset age, initial body temperature, family history and disease history was analyzed.
    Results
    The followup time was 3 to 6 years, averaged (4.8±1.1)years. Among 120 children with febrile seizures, the prognosis was poor in 14 cases(11.7%). The electroencephalogram confirmed that 10 cases were converted to epilepsy and 4 cases were determined to have intelligence disorder. The risk factors of febrile seizures included first-onset age, initial temperature, family history and disease history, and the differences were statistically significant among groups(P<0.05). Logistic regression analysis suggested that the independent risk factors affecting the prognosis of children with febrile seizures were firstonset age <1 year, initial body temperature <40 ℃, disease history, and family history.
    Conclusion
    The risk factors such as first-onset age, initial temperature, family history and history of febrile seizures were closely related to prognosis.

    Objective
    To investigate the clinical effect of pulmonary recruitment maneuvers combined with pressure adjustment volume control in premature infants with severe respiratory distress syndrome.
    Methods
    Sixty premature infants with moderate to severe(grade ⅢⅣ) respiratory distress syndrome who were admitted to the People's Hospital of Ruyang County from October 2015 to December 2016 were selected. They were divided into observation group(30 cases) and control group(38 cases) according to the treatment regimen. The control group received mechanical ventilation, and the mode was pressure adjustment volume control mode. The observation group received pulmonary re extension in addition to the treatment for the control group. The clinical efficacy of the two groups was observed. Ventilatory parameters were compared before and after pulmonary recruitment maneuvers. The time of taking up the machine, the time of oxygen therapy, the use of second surfactant and the complications of the two groups were recorded.
    Results
    The arterial oxygen pressure (PaO2), carbon dioxide partial pressure (PaCO2) and oxygenation index (PaO2/FiO2) of the observation group were (87.7±12.4) mm Hg, (48.6±9.2) mm Hg and (266.8±63.5) mm Hg, respectively, which were significantly better than those in the control group[(73.6±9.5) mm Hg, (57.4±8.6) mm Hg, (218.5±57.3) mm Hg](P<0.05). In the observation group, the time of taking up the machine, oxygen therapy time, the use of second pulmonary surfactant and the incidence of complications were all lower than those in the control group(P<0.05).
    Conclusion
    For children with moderate to severe respiratory distress syndrome, pulmonary recruitment maneuvers and pressure modulated volume control can help relieve or eliminate clinical symptoms, improve the clinical efficacy and reduce the incidence of complications.

    Objective
    Based on the traditional Chinese medicine inheritance support system V2.5(TCMISS V2.5), to study the rules of drug use by Professor Xu Youjia in the treatment of childhood asthma in remission stage.
    Methods
    A total of 124 prescriptions on childhood asthma in remission stage by Professor Xu Youjia were selected in Guangdong Provincial Hospital of TCM from June 2012 to December 2014. The following items were analyzed: frequency of drug use, four properties and five tastes, drug combination, drug pair and core combination.
    Results
    The prescriptions of Professor Xu mainly consist of the drugs tonifying deficiency, relieving exterior syndrome, regulating Qi, helping digestion and inducing astringency. The drugs most frequently used in the 124 prescriptions were: radix sileris, Atractylodes, licorice, orange peel, Schisandra, fried rice shoots, betel nuts, Shidi, heterophylla and bergamot like. The drugs used by Professor Xu were mostly with warm nature, followed by mild-natured drugs; the drugs with cold and hot nature were less used.
    Conclusion
    Professor Xu's treatment for childhood asthma in remission stage is guided by the principle of "the diseases being deficient and cold in nature. The medicinal materials tonifying deficiency, regulating Qi and helping digestion are often used, and the drugs mainly used are warm in nature.

    Objective
    To observe the effect and safety of Xiyanping injection in the treatment of children with influenza.
    Methods
    A total of 64 children with influenza were chosen, who were treated in the Pediatric Department of Chaozhou Central Hospital from Aug. 2016 to Aug. 2017,and they were divided randomly into two groups: the control group(32 children) and the observation group(32 children).Both groups received conventional treatment, including oseltamivir against virus, removing sputum, supplementing fluid and maintaining the electrolyte and acid-base balance. In addition, the observation group was given Xiyanping injection for intravenous dripping, once a day. Both groups were treated until the body temperature returned to normal and maintained for 48 hours. Observe the following items: the clinical effect, length of hospital stay, the relief time of catarrh symptoms, the time of body temperature returning to normal, the relief time of cough, and the adverse reactions.
    Results
    The rate of being significantly effective was 62.5%(20/32) in the observation group, which was significantly higher than that of the control group(28.1%,9/32),there being statistical difference(P<0.01).The effect in observation group was better, and the difference is statistical(z=-4.05,P<0.01).The relief time of catarrh symptoms and cough, the time of body temperature returning to normal, and length of hospital stay were all greatly shorter in the observation group, the difference being statistical(P<0.05). Only one child in the observation group had a little rash, which disappeared after withdrawal of medicine; there was nothing abnormal in the control group.
    Conclusion
    Xiyanping injection can greatly shorten the relief time of clinical signs and symptoms as well as the length of hospital stay in the treatment of children with influenza; there is no obvious adverse reaction, so it is worth promoting clinically.

    Objective
    To observe the clinical value of the mini-incision outside the elbow joint for children with severe humerus supracondylar fracture.
    Methods
    Totally 38 cases of severe humerus supracondylar fracture were chosen at the Hunan Children's Hospital between January 2012 and January 2015. The children underwent surgical treatment with miniincision outside the elbow and fixation with Kirschner wire. The efficacy was assessed by the Flynn standard during the 1 to 2-year follow-up.
    Results
    The surgeries for children were all successful and the incision wound healed smoothly. The elbow function recovered well with an average bone healing time of (2.7±0.4)months. The total percentage of patients rated as excellent(26 cases) and good(8 cases) was 89.47%(34/38), and the other patients were rated as not good(3 cases) and bad(1 case). During the perioperative period and follow-up, no patient had elbow loosening, neurological vascular damage or cubitus varus.
    Conclusion
    The mini-incision outside the elbow joint should be recommended as a surgical procedure for children with severe humerus supracondylar fracture, due to its good safety, minor wound, simple operation and low expense.

    Ru'e, which is a Chinese name for acute tonsillitis in western medicine, is a disease in the throat, which is caused by the rotting of blood and flesh due to evil guest throat nuclear. It is characterized by sore throat and reddingness and swelling of throat nuclear. Because it is very likely to transform into edema, palpitation and arthralgia, additional care should be given to it. Prof. Sun Liping thinks that acute tonsillitis is clinically common and the causes are complicated. Nowadays the children love to have greasy and heavy food, which can't be digested easily, and it brings about damage to the throat, resulting in acute tonsillitis. Therefore, the lung-stomach-heat type is more common in acute tonsillitis. In the treatment at acute stage, some medicines with cooling effect should be used, followed by heat-relieving drugs in order to assist the digestion and keep heat from developing. Some assisting medicine can also be used to enhance the effect. Later, pay attention to dieting, eat less greasy and spicy food and more vegetables to keep heat away. In addition, try to maintain the bowel movement, and put on clothes according to the weather so as to stop the cold from inducing the disease.

Because of the changes in environment, especially the increase of haze days, the incidence rate in children is increasing year by year, and the repeated attack has seriously affected the physical and psychological health of children. Prof. Song Guihua is experienced in its clinical treatment, who believes that the disease is caused by the incapability of freeing lung Qi and insufficiency of Yang Qi,and suggests the idea of combining syndrome-differentiated treatment with differentiation of constitution. The treatment should focus on Shufeng, Tongqiao, Wenyang and constitution regulation; meanwhile, the effect of immune regulation of probiotics can be utilized. By the integration of Chinese with western medicine, a better clinical results will be achieved.
