头针结合语言训练治疗脑性瘫痪语言障碍临床疗效及相关因素分析

The analysis of the effects and related factors of head acupuncture combined with speech therapy in treating the language disorder of cerebral palsy children

摘要: 目的观察头针结合语言训练对脑性瘫痪(脑瘫)语言障碍的治疗作用,分析疗效与语言障碍分型、病情程度、智力水平之间的关系。
方法155例脑瘫患儿按随机数字表法分为常规治疗组30例,常规治疗+语言训练组76例,常规治疗+语言训练+头针组49例,常规治疗主要进行以物理疗法、作业疗法、按摩、理疗为主的综合康复治疗,不包含语言治疗。治疗3个月为1个疗程。治疗前后应用语言发育迟缓检查法和构音障碍检查法进行语言功能评定,2~7岁采用“中国比内测验指导书”第3次修订本进行智力水平测定,2岁以下采用Gesell量表测定。
结果(1)治疗效果:常规治疗+语言训练组的治疗效果优于常规治疗组,差异有统计学意义(P<0.01),常规治疗+语言训练+头针组的治疗效果优于常规治疗+语言训练组,差异有统计学意义(P<0.01)。(2)相关因素分析:语言障碍分型中语言发育迟缓患儿疗效最显著(P<0.01),而语言前阶段与语言发育迟缓合并构音障碍的治疗效果比较差异无统计学意义(P>0.05);病情程度轻、中、重度的治疗效果差异有统计学意义(P<0.01);智力水平正常与落后之间的治疗效果差异有统计学意义(P<0.01)。
结论(1)头针结合语言训练和单纯语言训练对脑瘫伴语言障碍者都有治疗效果,但头针结合语言训练的疗效优于后者;(2)语言障碍分型中语言发育迟缓患儿头针结合语言训练疗效显著;(3)病情程度越轻,智力水平越高,治疗效果越好。

Abstract: Objective〖WT5”BZ〗〖JP2〗To observe the curative effect of head acupuncture combined with speech therapy and analyze the relations among the types of speech disorder,degree of illness and level of mental retardation.〖JP〗
〖WT5”HZ〗Methods〖WT5”BZ〗Selected 155 cerebral palsy(CP) children through the examination of CRRC(ss),dysarthria scale and intelligent test before and after treatment;all patients were excluded auditory disorder.Three groups were: 30 for contrast group(group 1),76 for speech and language training group(group 2) and 49 for head acupuncture combined with speech and language training group(group 3).Three months was one treatment period.
〖WT5”HZ〗Results〖WT5”BZ〗(1)There was a significant difference on the effective rates(P<0.01) with 60% for group1,88.4% for group 2 and 95.9% for group 3;(2)It was more effective for delayed language development disorder(DLDD) than others(P<0.01).The higher the level of function was , the higher the score of intelligence was,the better the effect was achieved,with significant difference(P<0.01).
〖WT5”HZ〗Conclusions〖WT5”BZ〗(1)The therapy has curative effects in group 2 and group 3,while group 3 is more effective;(2)The DLDD is more effective than the others;(3)The better the curative effect,the higher the level of function and the higher the score of intelligence.
