    Efficacy and safety of Reduning injection in the treatment of viral pneumonia in children

    ObjectiveTo explore whether the use of Reduning injection can improve the effectiveness and safety of the treatment for pediatric viral pneumonia by Meta-analysis.MethodsLiteratures on Reduning injection and pediatric viral pneumonia were searched in CNKI,VIP, Wanfang, CBM, PubMed and Embase databases, with the time limit from database construction to September 2021.Duplicate literature was excluded by Endnote X9 software; literature was screened according to the exclusion criteria,and data were extracted and quality evaluated.Data were recorded and Meta-analyzed using Stata 15.1 software.Finally, a sensitivity analysis was used to assess the outcome reliability.Funnel plots were used to evaluate the publication bias of the literature.ResultsA total of 21 literature articles were included, and the total sample size of the study was 2 632 cases.The results by Meta-analysis showed that the effective rate of test group was significantly higher than that of the control group with RR=1.13,95%CI(1.05,1.21).The incidence of adverse reactions in the test group was significantly lower than that in the control group, with RR=0.43,95%CI(0.29,0.64).Cough disappearance time[SMD=-2.73, 95%CI(-3.45, -2.00)], lung rale disappearance time[SMD=-1.28, 95%CI(-1.40, -1.13)],sore throat disappearance time[SMD=-1.26, 95%CI(-2.00, -0.52)], X-ray chest ray shadow disappearance time[SMD=-2.38, 95%CI(-3.38, -0.88)], time of fever relief[SMD=-2.14, 95%CI(-2.85, -1.43)],and time of hospital stay[SMD=-1.97, 95%CI(-2.79, -1.14)], in the test group were all better than those in the control group.ConclusionReduning injection is very effective in the treatment of viral pneumonia in children,and can increase the effective rate and reduce the incidence of adverse reactions.

    Efficacy and safety of traditional Chinese herbal compound in the treatment of children with influenza

    ObjectiveTo systematically evaluate the efficacy and safety of traditional Chinese herbal compound in the treatment of children with influenza.MethodsAll randomized controlled trials on TCM compound in the treatment of pediatric influenza were searched for in CNKI, Wanfang database, VIP, PubMed, Embase and Cochrane Library databases,the retrieval time being from January 2012 to June 2022. Literature screening and data extraction were carried out by two researchers independently, and the quality of the included studies was evaluated. Meta-analysis was conducted by RevMan 5.3 software, and trial sequential analysis was conducted by TSA 0.9.5.10 software.ResultsA total of 11 RCTS were included,involving a total of 3 232 patients. Meta-analysis showed that the total effective rate [RR=1.25, 95%CI(1.22, 1.30), Z=13.83, P<0.000 01] was better than the control group,the main symptom relief time was significantly reduced [MD=-2.62,95%CI(-3.88,-1.36),Z=4.08,P<0.000 1], and the incidence of adverse reactions was lower than that of control group [RR=0.66, 95%CI(0.47,0.94), Z=2.29, P=0.02], the differences being statistically significant. TSA results showed that Chinese herbal compound was effective in the treatment of children with influenza, and has reached the expected information value.ConclusionTraditional Chinese medicine compound is effective and safe in the treatment of children influenza, but it needs to be further verified by high-quality studies.

    Efficacy and safety of Yupingfeng granules combined with conventional western medicine in management of childhood asthma

    ObjectiveTo systematically evaluate the efficacy and safety of Yupingfeng granules combined with conventional western medicine for acute exacerbation and remission of childhood asthma.MethodsThe databases of CNKI, Wanfang, VIP, CMB, Chinese Medical Journal Network, PubMed, Cochrane Library and Embase were searched for randomized controlled trials on the effect of Yupingfeng granules in management of childhood asthma. RevMan 5.4.1 was used to perform a Meta analysis.ResultsA total of 23 studies were included, with 1 147 subjects in acute exacerbation stage and 1 897 subjects in remission stage. The Meta analysis showed that compared with the control group, Yupingfeng granules significantly improved the effective rate in acute exacerbation(RR=1.24,P<0.001), and shortened duration of cough(MD=1.25,P<0.001),wheezing(MD=1.46,P<0.001) and pulmonary wheezing on auscultation(MD=1.39,P<0.001). For asthmatic children in remission stage, Yupingfeng granules significantly improved the effective rate(RR=1.22, P<0.001), reduced the number of acute exacerbation(MD=1.62,P<0.001) and respiratory infections(MD=3.19,P<0.001) and time of antibiotic use(MD=1.09,P<0.001), and significantly improved lung function, cellular immunity and humoral immunity(P<0.05). Yupingfeng granules significantly down-regulated IL-4, IL-9 and IL-17 levels(P<0.05), and up-regulated IFN-γ level(P<0.05). In addition, the incidence of adverse effect was comparable between the Yupingfeng granules group and the control group(P=0.19).ConclusionYupingfeng granules combined with conventional western medicine can improve management efficacy of childhood asthma in acute exacerbation and remission stage, relieve symptoms of acute exacerbation, and reduce exacerbations during remission stage. Yupingfeng granules can also help to improve lung function, cellular immunity and humoral immunity, reduce inflammatory factors, and has good safety.

    IgA vasculitis used to be called allergic purpura, which is a kind of leucocyte fragmentation vasculitis with unknown pathogenesis. A large number of studies have found that streptococcal infection and immune disorders may be the cause of the disease. This article mainly focuses on the structure of streptococcal biofilms that may cause IgA vasculitis, such as AgⅠ/Ⅱ family proteins, deoxyribonuclease, lipid wall phosphatase, collagen-like protein-1, cysteine protease, etc., and Th1/Th2 ratio imbalance in immune disorders.

    Study on the mechanism of Qingxuan Zhike granules in the treatment of child bronchitis based on network pharmacology and molecular docking technology

    ObjectiveTo explore the mechanism of Qingxuan Zhike granules in the treatment of bronchitis based on network pharmacology and molecular docking technology.MethodsTCMSP was used to search for the active components of Qingxuan Zhike granules.The Gene Cards database was used to screen for the action targets of bronchitis,and the STRING database and Cytoscape were used to construct the action target network and protein-protein interaction network(PPI) of the components of Qingxuan Zhike granules and bronchitis to find the key action targets.Perl software was used to conduct gene ontology(GO) and genome database(KEGG) pathway enrichment analysis on key action targets.The molecular operation platform software MOE(molecule operating environment) was used for molecular docking.ResultsThe main active components of Qingxuan Zhike granules were stigmasterol,quercetin,locust element,kaempferol,coumarin,licorice flavone,licorice chalcone B,isorhamnetin,luteolin,naringen.The key targets acting on bronchitis were AKT1,PPARG,STAT3,ABCA1,AHR,AKR1B1,CD163,CHUK,MAPK14,ALOX5.The results of molecular docking verification showed that the core components could fully combine with key targets to play a role.The candidate targets mainly enrich Th17 cell differentiation,Lipid and aerosclerosis,Toxoplastosis,Acute myeloid leukemia,and Adipocytokine signaling pathway.ConclusionThe main components of Qingxuan Zhike granules are quercetin,isorhamnetin,acaricin,kaempferol,etc.By regulating Th17 signal pathway and inflammation signal pathways such as NF-κB signal pathway and AGE-RAGE,they act on MAPK14,STAT3 and CHUK targets,thus playing a role in anti-inflammation,protecting bronchial smooth muscle and improving the clinical symptoms of bronchitis.

    Mechanism of Danmu extract syrup in the treatment of influenza based on network pharmacology and molecular docking

    ObjectiveTo explore the possible mechanism of Danmu extract syrup in the treatment of influenza by network pharmacology and molecular docking technology.MethodsThe active components of Danmu extract were searched forby literature, and the target of active components in Danmu extract syrup was predicted according to Swiss target prediction data platform. The compound-target network was constructed by using the software of Cytoscape 3.6.1.Search for influenza-related genes through GeneCard database;through the Venny diagram, the intersection of active component target of Danmu extract syrup and influenza disease target was obtained,the key target of effective active component of Danmu extract syrup in treating influenza was obtained, and the drug-active component-key target-disease network was constructed. The common targets were analyzed for protein interaction by string database. In order to screen for the core target genes,Cytoscape 3.6.1 was used for visualization,and the plug-in CytoNCA was used to analyze the parameters of degree centrality(DC), betweenness centrality(BC), compactness centrality(CC), eigenvector centrality(EC), network centrality(NC) and local average connectivity(LAC) of protein-protein interaction(PPI); R language was used to make enrichment analysis of the GO and Kyoto encyclopedia of genes and genomes(KEGG) pathway. AutoDock vina 1.1.2 software was used to conduct molecular docking between the main active components and the target of Danmu extract syrup.ResultsTotally 28 active components corresponding to the target of Danmu extract syrup were obtained, and 544 targets were predicted. There were 2 574 influenza-related genes, 189 key targets of Danmu extract syrup in the treatment of influenza.A total of 54 key targets were screened out by PPI network analysis, including threonine protein kinase 1(AKT1), signal transducer and activator of transcription(STAT3), mitogen activated protein kinase 8(MAPK8), epidermal growth factor receptor (EGFR), etc.. Enrichment analysis showed that the common biological process of Danmu extract syrup and influenza was related to inflammatory reaction, cell proliferation and apoptosis,and the main pathways of action were virus infection pathway, apoptosis pathway, Th17 pathway, tumor necrosis factor pathway and T cell receptor signaling pathway. The results of molecular docking showed that the main active components had good binding activity with influenza virus neuraminidase.ConclusionDanmu extract syrup can inhibit the activity of neuraminidase, inflammatory reaction and apoptosis-related factors and pathways, so as to reduce the inflammatory reaction caused by influenza and achieve the purpose of treating influenza.

    ObjectiveTo compare the echocardiographic absolute value method with two Z socres(Kobayashi and Dallaire) calculation methods in evaluating the results of coronary artery lesions(CAL) in infants below 6 months with Kawasaki disease(KD).MethodsThe infants below 6 months with KD who were hospitalized in the Department of Cardiology, Xuzhou Children's Hospital,Xuzhou Medical University from January 2019 to December 2020 were selected. The internal diameters of major coronary artery segments were measured by echocardiography, and the CAL detection rates by absolute value method, Kobayashi(KZ method) and Dallaire(DZ method) were compared. Two Z socres of major coronary artery segments were analyzed for consistency.ResultsThere were 72 cases(84.7%) of CAL according to KZ, 63 cases(74.1%) according to DZ method, which was 0.95 times and 0.70 times higher than 37 cases(43.5%) according to absolute value method, respectively(P<0.001). The two Z socres of girls' RCA and boys' LAD were relatively poor in consistency.ConclusionIn evaluating coronary artery of infants below 6 months with KD, the degree of CAL judgment by BSA-corrected Z score method is much greater than that by absolute value method.

    Neuropsychological development at the age of 1 year in preterm infants <34 weeks with respiratory distress syndrome:a follow-up study

    ObjectiveTo investigate the influence of respiratory distress syndrome(RDS) on the neuropsychological development of preterm infants at the age of 1 year. MethodsA total of 240 preterm infants who were born in our hospital from Jan. 2016 to Dec. 2019 hospitalized in the NICU and followed up for one year after discharge in the high-risk infant clinic were selected as the study subjects. According to the diagnosis, the preterm infants were divided into the RDS group(n=99) and the non-RDS group(n=141), and the neuropsychological development of preterm infants at corrected gestational age of 1 year were evaluated by the Gesell Scale.ResultsAt the corrected gestational age of 1 year, the DQ of adaptability, great motor ability, fine motor ability and personal social ability of RDS preterm infants was behind that of non-RDS preterm infants, and the difference was statistically significant(P<0.05), while the difference in language development DQ was not statistically significant(P>0.05).The abnormal DQ rates(DQ<85 points) in the five energy regions of RDS preterm infants were significantly higher than those of non-RDS preterm infants, and the difference was statistically significant(P<0.05).Multivariate logistic regression analysis was used to correct the relevant influencing factors, and it was found that compared with non-RDS preterm infants, RDS preterm infants had a higher risk of abnormal development of adaptive ability and language ability(OR=0.460/0.734,P<0.05).ConclusionThe neuropsychological development of RDS preterm infants falls behind that of non-RDS preterm infants at the age of 1 year, and the rate of neurodevelopmental abnormality is increased. Clinical attention should be paid to the follow-up care and early intervention of RDS preterm infants.

    Effect of early intervention by neurodevelopmental therapy on neurodevelopment of high-risk premature infants

    ObjectiveTo observe the effect of intervention by neurodevelopmental therapy(NDT) on neurodevelopment of high-risk preterm infants and the sensitive indicators by rehabilitation intervention in high-risk preterm infants.MethodsA total of 82 cases of preterm high-risk infants borned in our hospital from September 2019 to September 2021 were selected and randomly grouped into control group and intervention group.The family members were told about the risks of brain injury and ways of intervention in preterm high-risk infants.The control group(42 cases) was the conventional family rehabilitation group, and the intervention group(40 cases) was the NDT rehabilitation group. Both groups were intervened for 6 months to observe the degree of neurodevelopmental improvement after the intervention, including muscle tone, nervous system reflexes, growth and development(development quotient). ResultsThere was no statistical significance in the difference between the two groups before the intervention(P>0.05); there was no statistical difference in neural reflexes and development quotients between the two groups before the intervention(P>0.05), but there were statistically significant differences between the two groups after the intervention(P<0.01). The Pearson correlation analysis was made and the results showed that the normal number of neural reflexes was related to the development quotient(r=0.829,P<0.01).ConclusionAbnormal neuroreflex examination in high-risk preterm infants, combined with muscle tone and growth development assessment,calls for NDT rehabilitation intervention in the children. Normal levels of neuroreflex can be clinically used as a sensitive indicator of rehabilitation interventions in high-risk preterm infants.

    ObjectiveTo explore the application effect of Yishen Tongdu acupuncture therapy in children with spastic cerebral palsy(CP), and its influence on motor function, cognitive level and serum neurofactor level.MethodsA total of 89 children with spastic cerebral palsy admitted to our department from August 2019 to September 2021 were selected as the research subjects and randomly divided into two groups. The control group(n=44) was given exercise therapy, and the observation group(n=45) was given exercise combined Yishen Tongdu acupuncture therapy. The motor function, cognitive function and neurofactor levels of the two groups were compared. ResultsAfter treatment, the gross motor function and fine motor function scores of the observation group were higher than those of the control group (P<0.05). Gesell Developmental Diagnostic Scale scores of fine motor and language,and personal-social scores of observation group were higher than those of the control group(P<0.05), and the scores of gross motor and adaptability tended to increase, and the differences were not statistically significant(P>0.05). The levels of nerve growth factor and brain-derived neurotrophic factor in the observation group were higher than those in the control group(P<0.05).ConclusionYishen Tongdu acupuncture combined with exercise therapy can effectively improve gross and fine motor function and promote neurocognitive development by strengthening kidney and spleen, Tongdu and dredging meridian, and regulating the expression levels of serum nerve growth factor and brain-derived neurotrophic factor.

    Analysis of the curative effect of early intervention combined with shallow needling method on facial neuritis in children under the guidance of ICF-CY theory

    ObjectiveTo explore the clinical efficacy of early intervention combined with shallow needling in the treatment of facial neuritis in children under the guidance of ICF-CY theory.MethodsThe data of 80 children with facial neuritis treated in our hospital from October 2019 to October 2020 were retrospectively analyzed, and they were divided into acupuncture group(40 cases) and combination group(40 cases) according to different treatment methods. All children were treated with hormones, B vitamins and other conventional drugs. The children in the both groups were treated with shallow needling (that is, the acupoints were quickly punctured without needle manipulation or needle retention), while the combination group was also treated with early intervention under the guidance of ICF-CY theoretical framework. The total effective rate, treatment time and House-Brakmann facial nerve function score (H-B score) before and after treatment were observed in the two groups.ResultsAccording to the statistical analysis, the total effective rate of the combination group was high, and the treatment time was short. Compared with the acupuncture group, the difference between the two groups was significant (P<0.05). After treatment, the increase in H-B score in combination group was more obvious than that in acupuncture group(P<0.05), and the difference was statistically significant.ConclusionUnder the guidance of ICF-CY theory early intervention combined with shallow needling can effectively improve the effectiveness in children with facial neuritis and shorten the treatment time.

    Study on the rule of selecting auricular acupoints for the treatment of children's multiple tics by data mining technology

    ObjectiveBased on data mining, to summarize the study on the rule of selecting auricular acupoints for the treatment of children with multiple tic syndromes. MethodsSearch the Chinese Journal Full-text Database (CNKI) and Wanfang Data Knowledge Service Platform for the literature on the treatment for children's multiple tic syndrome and establish a database; conduct a research on the data through frequency statistics, distribution, cluster analysis and complex network analysis.ResultsA total of 33 auricular acupoints were included, with a cumulative frequency of 295 times, of which there were 16 acupoints with a frequency of 4 or more times, with a cumulative frequency of 92.2%,and they were regarded as high-frequency acupoints; the first 16 acupoints were liver, Shenmen, heart, kidney, spleen, subcortical, endocrine, marginal, stomach, sympathetic, mouth, adrenal gland, brain stem, eye, throat and lung. The auricular acupoints were mostly located in the concha, antitragus and tragus. Cluster analysis included 4 categories, involving the combination of lung and throat, Shenmen, subcortical and adrenal glands, etc. The top 7 nodes of acupoints in complex network analysis were heart, liver, spleen, kidney, Shenmen, subcortex, and endocrine. The strong links of ear acupoints were liver-Shenmen and heart-liver.ConclusionThe treatment with ear acupoints for children with multiple tics is mainly through such acupoints as the heart, liver, Shenmen, spleen, and kidneys, which soothes the heart and nerves, calms the liver and wind, and invigorate the kidney and spleen, providing a reference for the clinical development of new prescriptions.

    Wang Ji′an′s experience in treating infantile pulmonary diseases

    Professor Wang Ji'an emphasizes that there is difference between dispersing and clearing in pulmonary diseases, which helps lung-qi to go upward or downward. Clearing and warming must not be excessive in order that the differentiated treatment can be carried out in time. The reinforcing and reducing should be given properly to eliminate the evil and secure health. The drugs should be mild in nature and the course should be short because the organs are delicate. The organs are inter-related, so the treatment for pulmonary diseases should not be limited to lung, and spleen, stomach and liver should be considered. For patients whose lung-qi can't be dispersed, the treatment should be based on coldness or heat: for patients with wind-heat invading the lung, the lung-qi should be cleared, while for those with wind-coldness invading the lung, the lung-qi should be warmed to disperse. For the patients whose lung-qi can't be cleared, there are 4 ways to clear the lung: descending lung-qi, relieving spasm, clearing organs, and reducing lung to relieve asthma. Prof. Wang Ji'an believes that the treatment for measle pneumonia should focus on dispersing, clearing and relieving. Clinically, measles usually is a disease involving both weifen and qifen, and involving qifen and yingfen, but it rarely involves xuefen, so the treatment focuses on the causes: to remove the evil and regulate qi in order for the heat-evil of yingfen to be eliminated. The treatment for pertussis can be based on liver, and the well-known prescription——Xuanci Baibu decoction——has significant effect on spasm and cough of pertussis and pertussis-like syndrome.

    Discussion on relationship between lung and spleen by Professor Jia Liujin and his experience in the treatment of pediatric diseases based on co-regulation of lung and spleen

    Reinforcing earth to generate metal is a way of treatment commonly used in traditional Chinese medicine(TCM).It is also one of the important clinical ideas to treat spleen and stomach diseases based on lung,so the co-regulation of lung and spleen is of practical clinical significance.Based on the TCM basic theories and the pathophysiological characteristics of "usual insufficiency of lung and spleen" in children,this article reviews the theoretical evidence on "co-regulation of lung and spleen",taking the nine factors suggested by Prof.Jia Liujin as the points of discussion,including connection among meridians,the common pathophysiological features,relation among five elements,generation and transportation of qi,the relationship with fluid retention,and the relationship of modern anatomy.Based on the theory of "co-regulation of lung and spleen",Prof.Jia Liujin′s clinical experience was summarized in three aspects:prevention of diseases,treatment for exogenous diseases and treatment for internal diseases,with one case as an example.Finally,in combination with modern medicine,the relationship between lung and spleen is explored from neurological and immunological points to find the common aspects in clinical manifestation and pathogenesis between the theory of "co-regulation of lung and spleen" and the respiratory and gastrosplenal diseases in modern medicine.The correct understanding of the theory of "co-regulation of lung and spleen" and attention to the co-regulation of lung and spleen in pediatric clinical practice will play a very important role in improving the diagnosis and treatment of pediatric diseases.

    The paper aims to summarize Professor Jia Liujin's experience in the treatment of child tonsillitis.According to the onset of disease and the length of disease course,the disease is divided into acute and chronic tonsillitis. Professor Jia Liujin thinks that acute tonsillitis is mostly caused by solid heat while chronic tonsillitis is mostly due to fire of deficiency.Because the disease is in the throat and the cause is mostly fire,the primary principle of treatment is to clear throat.Clinically differentiated treatment is performed based on different onset of child tonsillitis.For acute tonsillitis of wind-heat invasion,the treatment principle is to relieve wind,clear heat,detoxify and clear throat,and the prescription of Yinchai Tuire decoction is used; for this type, the treatment focuses on clearing heat and detoxifying so as to stop high fever.For acute tonsillitis of excessive lung-stomach heat,the treatment principle is to clear lung and stomach heat,resolve phlegm and disperse stagnation,ventilate nose and manage adenoidal hypertrophy at the same time,and such medicines as Scutellaria baicalensis, Rhizoma cimicifugae, plaster, ageratum and Arisaema cum bile.The points in the treatment of this type of disease lie in clearing lung and stomach heat and solving the problem of snoring in children.Chronic tonsillitis is mostly due to Yin-deficiency of lung and kidney,and the treatment focuses on nourishing Yin and eliminating fire and clearing throat,and the prescription of Zhibai Dihuang decoction with modification is used.This type of tonsillitis is likely to recur,the remained evil is hard to be eliminated,and lingering evil heat damages Yin of lung and kidney,so in the treatment,attention should be paid to the balance between strengthening resistance and eliminating pathogenic factors and to the prevention of recurrence of tonsillitis.Currently there has been no significantly effective therapy for tonsillitis,and in contrast differentiated TCM treatment has its unique advantages. Professor Jia Liujin has his own features in clinical prescriptions and medication,which is proved to be effective in the treatment of this disease.A case report is attached for verification.

    ObjectiveTo screen and evaluate the opinions of pediatricians of integrated traditional Chinese and Western medicine on the diagnosis and treatment of precocious puberty, providing basis for formulating the integrated traditional Chinese and Western medicine guildline for diagnosis and treatment of child precocious puberty.MethodsThe Delphi queChinese and western medicine among 52 pediatric experts of integrated traditional Chinese and Western medicine in China, the results were statistically analyzed, and the indicators of the integrated traditional Chinese and western medicine guildline for diagnosis and treatment of children's precocious puberty were formulated.ResultsTotally 52 and 51 questionnaires were received in the first and second rounds of survey respectively, and the positive indexes of experts were 100% and 98.09%, respectively. The  of all items in the two rounds of questionnaires ranged from 0.33 to 2.00, Sum ranged from 28 to 104, R ranged from 0% to 69%, and CV ranged from 0% to 160%.ConclusionEnthusiasm and professional level of the experts in this survey are high and the geographical coverage is wide.
