    Clinical study of "withdrawal protocol" for epilepsy in children

    ease characterized by repetitive, paroxysmal and transient central nervous system dysfunction caused by over-discharge of brain neurons. The treatment for epilepsy currently includes Chinese medicine, western medicine and the combination of the two. There are few clinical reports on the withdrawal of antiepileptic drugs after the seizures have stopped, especially the withdrawal methods for children with epilepsy using Chinese medicine or Chinese and Western medicine. The author has been engaged in the clinical research of pediatric epilepsy for more than 35 years. In the longterm practice, he has explored a set of withdrawal protocols for the treatment of epilepsy in children treated with TCM or Chinese and western medicine. It has been clinically proven to have fewer side effects and low recurrence rate.

    Objective
    Based on the literature, to study the highfrequency traditional Chinese medicines and medicinal types for pediatric epilepsy in the recent 20 years. On this basis, to summarize the medication rules in pediatric epilepsy, which provides the basis for clinical treatment.MethodsThe related literature of traditional Chinese medicine in the treatment of childhood epilepsy published in China from 2000 to 2019 was searched. SPSS 19.0 software was used to analyze the frequency.
    Results
    This study involved 241 flavors and 19 kinds of traditional Chinese medicines. Among them, Acorus tatarinowii was the most frequently used(72.28%),and the flavors were mainly sweet, spicy and bitter, with a cumulative frequency of 82.08%. The medicinal properties of Acorus tatarinowii were mainly warm, cold and moderate, with a cumulative frequency of 90.67%. The main meridians were liver, heart, spleen and stomach, with a total of 65.40%. The main types of Chinese medicines used were those tonifying deficiency, calming liver and wind, resolving phlegm, relieving cough and asthma, accounting for 46.90% of the total drug use.
    Conclusion
    Clinical treatment for pediatric epilepsy mainly focuses on tonifying the heart and spleen, nourishing the liver and kidney, eliminating phlegm and resuscitating Qiao, relieving wind, dredging collaterals and stopping spasm. Drugs are mostly sweet, spicy and bitter.

    The three-dimensional teaching mode of TCM pediatrics is to adapt to the new form of teaching reform in order to meet the demand for inter-disciplinary  talents in TCM pediatrics. Take pediatric epilepsy as an example. Based on the characteristics of the disease, this mode gives full play to the advantages of clinical and scientific research, makes use of modern technology, optimizes teaching resources, reforms teaching forms, and enriches teaching methods, which stimulates students' interest in learning, so that their improve self-learning ability, clinical practice ability and innovative consciousness are improved.

    Infantil spasms is an age-dependent epileptic encephalopathy in early infancy. Early diagnosis and treatment are beneficial to the termination of seizures and the longterm cognitive protection. This paper reviewed the recent advances in the treatment of infantile spasms from drug therapy, the ketogenic diet and surgical treatment.

    Study on the change of seizure frequency and the related risk factors during adolescence in women with epilepsy

    Objective
    To study whether there is aggravation during adolescence, the proportion of aggravation and related risk factors in epileptic women.
    Methods
    With the method of retrospective study, a total of 101 female epilepsy patients who met the inclusion-exclusion criteria were screened and divided into aggravation group and no-aggravation group. General data, seizure information, drug use data and seizure control evaluation data were collected.
    Results
    In the 101 women 84 had no change in the seizure frequency, accounting for 83.17%(84/101). In 17 cases the frequency of seizures increased, accounting for 16.83%(17/101). About 69.31% of the patients had no seizures when they grew into puberty. There was statistically significant difference in EEG after treatment, seizure type and general medication between the group of aggravation and the group of no-aggravation(P<0.05). The results of multivariate Logistic regression analysis showed that the factors affecting the seizure frequency were EEG after treatment, seizure type and general medication.
    Conclusion
    In most adolescent women with epilepsy,the onset of the disease becomes less frequent or they are seizure free. The patients with abnormal EEG after treatment and focal seizures are more prone to aggravate. The patients receiving traditional Chinese medicine combined with western medicine are more likely to have aggravation.

    Effects of Yiqi Jiedu Huayu Xiao Fufang and its single drug on proliferation and apoptosis of glomerular mesangial cells in rats

    Objective
    To observe the effect of Yiqi Jiedu Huayu Xiao Fufang and its single drug on proliferation and apoptosis of glomerular mesangial cells in rats and explore its mechanism.
    Methods
    A total of 60 Wistar rats were randomly divided into normal group, Xiaofufang group, Huangqi group, Danshen group, Baihuasheshecao group and Telmisartan group, with 10 rats in each group. The normal group was given 4 mL of distilled water daily, and each drug group was given 2 mL of drug in the morning and evening for 3 consecutive days. Rat mesangial cells were routinely cultured, and the cultured mesangial cells were randomly divided into normal group, Xiaofufang group, Huangqi group, Danshen group, Baihuasheshecao group, Telmisartan group, and Angiotensin Ⅱ group. Angiotensin Ⅱ 1×10-6mol/L was added to each group. The proliferation of rat mesangial cells was detected by MTT colorimetric assay. The apoptosis of rat mesangial cells was determined by flow cytometry.
    Results
    Compared with Angiotensin II group, other drug intervention groups had effect of inhibiting the proliferation and increasing apoptosis rate of mesangial cells(P<0.01 or P<0.05); there was no significant difference among the other groups(P>0.05). Compared with other drug intervention groups, the Xiaofufang group had the strongest inhibition and apoptosis of mesangial cells(P<0.01 or P<0.05); there was no significant difference among the other groups(P>0.05).
    Conclusion
    Yiqi Jiedu Huayu Xiao Fufang and its single drug can inhibit the proliferation and apoptosis of rat mesangial cells, which can slow the development of glomerular sclerosis, and the effect of Yiqi Jiedu Huayu Xiao Fufang is the best.

    Observation on the efficacy of Yiqi Huayu Qingre prescription in the treatment of children with refractory nephritic syndrome

    Objective
    To observe the clinical effects of Yiqi Huayu Qingre prescription on refractory nephrotic syndrome(RNS) in children.
    Methods
    A total of 60 cases of RNS enrolled from July 2013 to July 2014 in the First Affiliated Hospital of Henan University of Traditional Chinese Medicine and Children's Hospital of Zhengzhou city were randomly divided into control group(30 cases) and observation group(30 cases). The children were routinely symptomatic treatment in the control group, such as anti-infection, anticoagulation and diuresis, and sufficient prednisone was given(2 mg/kg) to induce remission. The observation group was treated with Yiqi Huayu Qingre prescription on the basis of the treatment for control group, 4 weeks as a course of treatment, for a total of 9 courses. Observe the clinical efficacy and recurrence rate; efficacy indicators:24hUTP,ALB,CHOL,FIB,PT,APT;safety indicators: symptoms and signs of adverse reactions, blood routine examination and hepatorenal function.
    Results
    The total clinical effective rate of the observation group was 86.7%(26/30), which was better than that of the control group(70.0%,21/30), the difference being statistically significant(P<0.05).After treatment, the recurrence rate in the observation group was 36.7%(11/30), which was significantly lower than that in the control group(70.0%,21/30), the difference being statistically significant(P<0.05). After treatment, the serum albumin, activated partial thrombin time and prothrombin time in the observation group were higher than those before treatment, while 24h protein quantity, total cholesterol and fibrinogen in the observation group were significantly lower, the difference being statistically significant(P<0.05); the observation group was better than the control group in the improvement of the above indexes, the difference being statistically significant(P<0.05). There was no significant difference in such indexes as blood routine, glutamicpyruvic transaminase, glutamicoxaloacetic transaminase, urea nitrogen or serum creatinine between the two groups before and after treatment(P>0.05), and no serious adverse reactions were found.
    Conclusion
    Yiqi Huayu Qingre prescription can improve the indexes including 24h protein quantity, plasma albumin, blood lipid and hypercoagulability in children, reduce the recurrence rate of refractory nephrotic syndrome in children, and improve the total clinical effective rate. There are no adverse reactions in the clinical observations.

    Progress in diagnosis and treatment of transient tachypnea of the newborn

    Transient tachypnea of the newborn(TTN) is the most common cause of neonatal dyspnea, and its clinical manifestations are non-specific, which is not easy to distinguish from respiratory distress syndrome, pneumonia, and meconium aspiration  syndrome, etc., and has self-limited characteristics, so missed diagnosis or delayed treatment is commonly seen clinically. In recent years, the number of severe TTN has increased, and ventilator-assisted respiration, even extracorporeal membrane oxygenation, is often used. This paper reviews the pathogenesis, diagnosis and treatment of TTN.

    Autism spectrum disorder(ASD) is a widespread developmental disorder of the brain characterized by social interaction and communication disorders, abnormal repetitive stereotyped behavior, narrow range of interest, and limited and obstructed daily activities. Because of its unknown pathogenesis, there is no effective treatment. Vagus nerve stimulation, with autonomic nerve function regulation as its core, opens up a new principle of peripheral nerve-brain network-body function regulation, and has achieved good therapeutic effects in the treatment of epilepsy complicated with autism spectrum disorder, depression and sleep disorder. This article summarizes the current situation and feasibility of vagus nerve stimulation (VNS) in the treatment of autistic spectrum disorder by consulting the related literatures at home and abroad in recent years, in order to provide a new line of thinking about the treatment for autistic spectrum disorder in children.

    Research progress of OSA in the pathogenesis of HDP and maternal hypoxemia

    Obesity is increasingly common among pregnant women in China; 15%-20% of obese pregnant women have obstructive sleep apnea(OSA). Untreated OSA will result in a range of cardiovascular, pulmonary and metabolic complications. Hypertensive disorder complicating pregnancy (HDP) is the leading cause of maternal and neonatal morbidity and mortality. OSA during pregnancy may be at risk of pre-eclampsia or eclampsia, leading to a poor outcome in the fetus. The relationship between OSA and HDP and its fetal hypoxemia and neonatal related complications has not been clearly elucidated. In this review, we review the relationship between obstructive sleep apnea and hypertensive disorders of pregnancy, its possible mechanisms, and the effectiveness of alleviating these adverse consequences.

    Diagnosis and treatment of bleeding and Meckel's diverticulum in 60 children

    Objective
    To evaluate the clinical and pathologic charateristics of children with bleeding and Meckel's diverticulum,so as to improve the level of diagnosis and treatment.
    Methods
    The clinical data of 60 patients diagnosed with bleeding and Meckel's diverticulum who were admitted to our hospital from January 2015 to December 2017 were collected. The diagnosis was confirmed by surgery and pathology. Clinical characteristics, auxiliary examinations and pathological results were analyzed.
    Results
    (1) In total, 60 children were diagnosed with bleeding MD. Of these,49 were male and 11 were female, with a male to female ratio of 4.45∶1.(2)There are many different degree of anemia, with 49(81.7%) to be moderate or severe anemia.(3)The associated symptoms were different among the patients:17(28.3%) patients had abdominal pain, 11(18.3%) had vomiting, 2(3.3%) had fever, and 38(63.3%) had no associated symptoms.(4)Totally 56 patients were examined by technetium-99m pertechnetate imaging. Among these patients, 54(96.4%) had positive results,2(3.6%) had negative results. (5)The postoperative histology revealed that ectopic gastric mucosa existed in 59 patients(98.3%), and both gastric and pancreatic ectopic tissue in 1 patient(1.7%).(6)The positive detection rate was significantly lower by ultrasonography than by technetium99m pertechnetate scan(P<0.05).(7)All the 60 patients underwent partial ileal resection. In the surgery, it was found that the distance from the diverticulum to the ileocecal valve was 10 to 80 cm (average 46.3±15.7 cm). Diverticulum length ranged from 2.0 to 6.0 cm.
    Conclusion
    Preoperative diagnosis of bleeding and Meckel diverticulum is difficult. Meckel diverticulum patients with bleeding usually lack specific clinical symptoms and typical signs. The disease develops fast. Technetium99m pertechnetate imaging may give higher diagnostic rate of bleeding and Meckel's diverticulum. Surgical resection is necessary once a diagnosis of bleeding MD is made.

    Clinical observation of internal and external Treatment by traditional Chinese treatment combined with western medicine for enuresis in children

    Objective
    To explore the effectiveness of internal and external treatment by traditional Chinese treatment combined with western medicine for enuresis in children.
    Methods
    A total of 67 enuresis children(aged 7 to 17) with spleen and kidney Yang-insufficiency were chosen as the research subjects, who were treated in Beijing Children's Hospital from Nov. 2016 to Dec. 2017. They were divided into two groups according the random number table method: 35 in the western medicine treatment group and 32 in the integrated Chinese and western medicine treatment group. Both groups were treated with desmopressin(orally taken) according to the recommendations of International Pediatric Urinary Control Association and Chinese expert consensus on the management of children's single-symptom night enuresis disease. In addition, the integrated treatment group was treated with Chinese medicine(orally taken) assisted by acupuncture point paste. The treatment lasted for 4 weeks as one course. Observe the clinical effect, improvement in TCM syndromes and the adverse events during treatment in the two groups.
    Results
    Of the 63 children who were enrolled in the study and completed the treatment and follow-up, there were 32 in the western medicine treatment group and 31 patients in the integrated Chinese and western medicine group. The total effective rate was 90.3%(28/31) in the integrated Chinese and Western medicine group and 84.4%(27/32) in the western medicine treatment group. There was no significant difference in the effective rate between the two groups(P>0.05). The improvement rate of TCM syndromes in the integrated group was 96.8%(30/31), which was significantly better than that in the western medicine group(62.5%,20/32), and the difference was statistically significant(P<0.05). In the two groups, no significant drug-related adverse reactions were observed during the treatment.
    ConclusionDesmopressin combined with TCM syndrome differentiation treatment(traditional Chinese medicine treatment combined with acupuncture point paste) has significant effect on the improvement of TCM syndromes of spleen and kidney Yang-insufficiency in children with enuresis, and can effectively improve the TCM symptoms and physique of children with enuresis.

    Maple syrup urine disease of DBT gene mutation type in children

    Objective
    To investigate the clinical characteristics, diagnosis and treatment of maple syrup urine disease of DBT gene mutation type in children.
    Methods
    The clinical manifestations, genotype, diagnosis and treatment of a child genetically diagnosed with maple syrup urine disease were analyzed retrospectively.
    Results
    Being male, 11 days, the child mainly showed anorexia and his urine had a smell similar to maple sugar. The blood tandem mass spectrometry, urine gas chromatography and head MRI showed the charges consistent with the related changes of maple syrup urine disease. Gene analysis showed that his parents were of the heterozygous mutation of c.1132C>T(p.378X), and the child was of the homozygous mutation of c.1132C>T(p.378X). Special formula milk powder and vitamin B1 were given in time and he became better.
    Conclusion
    Maple syrup urine disease of DBT gene mutation type in children is characterized by anorexia and special smell in urine. Blood tandem mass spectrometry, urinary organic acid, head MRI and gene analysis are helpful to make a definite diagnosis. The treatment with special formula milk powder and vitamin B1 can improve prognosis.

    Effect of three-level rehabilitation program on motor function and treatment cost in children with cerebral palsy

    Objective
    To explore the effect of threelevel rehabilitation program on motor function and treatment cost in children with cerebral palsy.
    Methods
    From October 2017 to October 2018,140 children with cerebral palsy were treated in Shenyang Children's Hospital, and they were divided into observation group and control group according to random number table method, 70 in each group. The control group was given routine rehabilitation treatment. The observation group was treated with threelevel rehabilitation program according to GMFcs grading, and the clinical efficacy and health economic indicators were evaluated and compared.
    Results
    There was no significant difference in gross motor score between the two groups before treatment(P>0.05).After treatment, the scores of the two groups were higher,and the scores of the observation group were higher than those of the control group(P<0.05). The direct medical expenses, direct nonmedical expenses and indirect expenses of the observation group were lower than those of the control group, and the difference had statistical significance(P<0.05).
    Conclusion
    The construction of a standardized threelevel rehabilitation mode for children with cerebral palsy can make full use of rehabilitation resources, maximize costeffectiveness and reduce the economic burden in the patients.

    Effects of rehabilitation compliance on the efficacy of treatment for children with spasmodic cerebral palsy

    Objective
    To study the effect of rehabilitation compliance of children with spasmodic cerebral palsy on the treatment efficacy.
    Methods
    A total of 100 children with spasmodic cerebral palsy(CP),who were treated in Weifang Maternal and Child Health Care Hospital form Jan. 2015 to Oct. 2018, were chosen as the research subjects. They were divided into two groups according to their compliance in rehabilitation treatment: the observation group(50 children) in which the children were accompanied by a fixed parent during the whole process and received comprehensive rehabilitation treatment combined with rehabilitation training at home, and the control group(50 children) in which the children couldn't receive comprehensive rehabilitation treatment or rehabilitation training at home. Both groups were given motortherapy, occupational therapy and massage therapy. In addition, the observation group received guided education, sensory integration training and music therapy, and training and management at home were also performed; the parents received grouped and individualized training and psychological tutoring regularly. Both groups were assessed respectively with GMFM-88 and HK-PFMDA before treatment and 3 months after treatment. The improvement in the gross motor function and fine motor function was observed in the children.
    Results
    The total percentage of GMFM score and HK-PFMDA score were better after treatment than before treatment in the two groups, the difference bring statistically significant(P<0.05), and they were better in observation group with statistical difference after treatment(P<0.05).
    Conclusion
    The rehabilitation compliance of children affects the treatment effects; the children with good compliance have significantly greater improvement in gross and fine motor function,which can promote their physical and psychological rehabilitation to the largest extent and improve their life quality.

    Effects of gonadotropin-releasing hormone analogues on body mass index and bone metabolic markers in girls with idiopathic central precocious puberty

    Objective
    To investigate the effects of gonadotropin-releasing hormone analogue (GnRHa) on body mass index (BMI) and bone metabolism markers in girls with idiopathic central precocious puberty (ICPP).
    Methods
    Totally 100 ICPP girls admitted to our hospital from January 2015 to January 2016 were selected as the study group, and 100 healthy check-up girls in our hospital during the same period were selected as the control group. The observation group was treated with triptorelin hydrochloride for 1.5 years. Up to June 2018, the average follow-up time was (1.1±0.3) years. The age, bone age, BMI, overweight rate and obesity rate of the girls in the observation group at the beginning, end of treatment and at the time of reaching lifelong height were recorded. The levels of procollagen type I amino-terminal peptide(P1NP), osteocalcin N-terminal midmolecular fragment(N-MID) and β-collagen degradation product(β-CTX) were recorded before and 2 years after treatment in the observation group and in the control group.
    Results
    The age, bone age and BMI at the end of treatment in the observation group were higher than those at the beginning of treatment, and the difference was statistically significant (P<0.05); the age and bone age at reaching the lifelong height were higher than those at the beginning of treatment in the observation group(P<0.05), while BMI was similar(P>0.05). In the observation group, the age and bone age at the time of reaching lifelong height were higher than those at the time of discontinuation of treatment, while BMI was lower(P<0.05). Before treatment, the levels of P1NP and N-MID in the observation group were significantly higher than those in the control group(P<0.05), but there was no significant difference in the levels of β-CTX between the two groups (P>0.05). After treatment, the levels of P1NP and N-MID in the observation group were significantly lower than those before treatment and the difference was statistically significant(P<0.05); after treatment, the level of β-CTX was not significantly different from that before treatment(P>0.05).
    Conclusion
    GnRHa can effectively inhibit the hyperactivity of osteoblasts in girls with ICPP, and has no significant effect on the function of osteoclasts. Although the BMI value of the patients increases during the treatment, it will return to normal level after the treatment.

    Objective
    To investigate the clinical efficacy of adenosine monophosphate combined with Xiyanping in the treatment of herpetic angina in children.
    Methods
    Totally 92 children with herpes angina admitted to the Ninth People's Hospital of Shenyang from June 2017 to June 2018 were randomly divided into control group(47 cases) and observation group(45 cases). Both groups were given Xiyanping injection by intravenous dripping, and the observation group was also given intravenous dripping of adenosine monophosphate; the treatment lasted for 3 days in a row. The clinical efficacy, herpes healing time, ulcer healing time and fever relief time of the two groups were observed.
    Results
    The total effective rate of the observation group was 95.6%(43/45), which was significantly higher than that of the control group(70.2%,33/47), and the difference was statistically significant(P<0.05). The healing time of herpes and ulcer and the fever relief time in the observation group were significantly shorter than those in the control group(P<0.05).
    Conclusion
    Adenosine monophosphate combined with Xiyanping is effective in the treatment of herpetic angina in children.

    Clinical features and prognosis of late-onset GBS pyemia

    Objective
    To observe the clinical characteristics and prognosis of children with late-onset GBS pyemia, and analyze the factors influencing the prognosis of patients.
    Methods
    A total of 102 children with GBS pyemia treated in our hospital from June 2014 to October 2017 were selected as the research subjects; according to the type of disease, they divided into early-onset group(48 cases) and late-onset group(54 cases). Observe the difference between the two groups of children in the clinical manifestations and laboratory indexes. Compare the incidence of complications and prognosis between the two groups, and analyze the factors influencing the prognosis of patients.
    Results
    The incidence rate of fever or hypothermia, shortness of breath, convulsions, apnea, jaundice and pneumonia in late-onset group was  significantly higher than that in the early-onset group, the difference having statistical significance (P<0.05). The Index of WBC, CRP, IL-6 and TNF-αlevel in late onset group were significantly higher than those in the early-onset group, while PLT was lower, the difference being statistically significant(P<0.05). The complication rate of purulent meningitis, NEC, MOF septic shock and DIC in late-onset group was significantly higher than in the early-onset group, the difference being statistically significant(P<0.05). The cure rate of late-onset group(46.3%,25/54) was significantly lower than that in the early-onset group(87.5%,42/48)(P<0.05);mortality rate was significantly higher in late-onset group(31.5%,17/54) than in the early-onset group(8.3%,4/48)(P<0.05); GBS was the risk factor of the death of children(P<0.05).
    Conclusion
    The clinical manifestations of late-onset GBS pyemia is serious, with high mortality and poor prognosis.

    Objective
    To study the effect of hydrotherapy and fumigation on motor function of high-risk children with cerebral palsy (CP).
    Methods
    Totally 80 children with cerebral palsy admitted to the Department of Pediatrics of the First Affiliated Hospital of Henan University of Traditional Chinese Medicine from January 2018 to January 2019 were selected as research subjects, and they were randomly divided into observation group and control group with 40 cases in each group.  The control group was given basic treatment(massage, physical therapy). In addition to basic treatment, the observation group also received hydrotherapy and fumigation, once a day, 6 days a week, and 4 weeks as a course of treatment, for 3 consecutive courses of treatment. Gross motor function test scale(GMFM) and modified Ashworth muscular tension grading method were used to evaluate, and its safety was observed.
    Results
    The total percentage of GMFM in the observation group was higher than that in the control group, and the muscular tension score was higher than that in the control group, the difference being statistically significant(P<0.05). The two groups of children did not have abnormal vital signs during treatment, no adverse reactions such as allergy and scald occurred during fumigation with traditional Chinese medicine, and no adverse events such as choking water occurred during hydrotherapy.
    Conclusion
    Hydrotherapy and fumigation therapy can improve motor function of high-risk children with cerebral palsy.

    Detection and significance of serum BDNF and S100B protein levels in autistic children

    Objective
    To detect the levels of BDNF and S100B protein in serum of autistic children, and to explore the relationship of BDNF and S100B with autism in children.
    Methods
    Totally 35 autistic children who participated in training in Luohe Disabled Rehabilitation Center from March 2016 to September 2017 were selected as the study group, including 24 children with mild autism and 11 children with severe autism. Also, 40 healthy children were selected as the control group. ELISA was used to detect the levels of BDNF and S100B protein in serum.
    Results
    The serum BNDF level in the study group was significantly higher than that in the control group(P<0.05). There was no significant difference in serum S100B protein level between the two groups(P>0.05). The level of serum BDNF in mild autism group was higher than that in control group(P<0.05), but there was no significant difference between severe autism group and control group(P>0.05),and there was no significant difference between mild and severe autism group, either(P>0.05).
    ConclusionThe level of serum BDNF in autistic children is significantly increased, but the level of S100B protein has no significant increase. These results suggest that BDNF may be related to the occurrence and development of autism, but the relationship between S100B protein and autism is not very clear.

    Objective
    To explore the effect of interventional respiratory therapy on postoperative respiratory function in infants with congenital heart disease.
    Methods
    Totally 40 children with congenital heart disease were randomly divided into control group(n=20) and observation group(n=20), who were hospitalized in Thoracic Surgery of Children's Hospital of Hunan Province from November 2017 to April 2018. The control group was treated with the routine respiratory therapy, and the observation group was treated with the interventional respiratory therapy. The auxiliary time of the ventilator, the time of staying in the CICU, the incidence rate of the secondary air tube, the blood gas analysis index, the ventilation/ blood flow ratio and the results of the chest film in both groups were compared.
    Results
    The auxiliary time of the ventilator and the time of staying in the CICU were significantly shorter in observation group than in control group(P<0.05). The incidence rate of the secondary air tube in observation group (10.0%, 2/20) was significantly lower than that in control group(65.0%, 13/20) and the difference was statistical significant(P<0.05). After treatment, the blood PCO2 and PO2 was significantly better in observation group than in control group(P<0.05). There was no significant difference in the blood oxygen saturation between the two groups(P>0.05). However, the incidence of complications in the observation group(5.0%, 1/20) were lower than that in the control group(55.0%, 11/20) and the difference was statistically significant(P<0.05). In addition, the ventilation/blood flow ratio was significantly balanced and improved in both groups after treatment.
    Conclusion
    Interventional respiratory therapy can improve the postoperative respiratory function and lung ventilation in infants with congenital heart disease, and increase the success rate of operation.

    Objective
    To observe the antipyretic effect and the change of sIgA/ALB in influenza A children with excess heat due to Qi deficiency treated by self-prescribed Tuirefang of Chinese medicine.
    Methods
    Totally 200 influenza A children with excess heat due to Qi deficiency in autumn-to-winter of 2018 were randomized into study group and control group, with 100 cases in each. Two groups were given antipyretic treatment. The study group was given Tuirefang, and the control group was given oseltamivir taken orally. Three days after treatment, the antipyretic effect and change of sIgA/ALB were compared between two groups.
    Results
    The antipyretic period of the study group was(1.85±0.26)days, and in the control group it was(2.50±0.30)days, the difference between two groups being significant(P<0.05).The sIgA/ALB in two groups were higher after treatment, and the increase of study group was more obvious than that of control group, the difference being significant(P<0.05) between the two groups.
    Conclusion
    The antipyretic period of Tuirefang is shorter than that of oseltamivir, and it can increase the immune function of respiratory mucosal in influenza A children.

    In the diagnosis and treatment of children with allergic purpura, Prof. Qin Yanhong explores the cause of the disease and sticks to the pathogenesis. She summarizes the clinical observation results and takes into account the experience of experts throughout the history to perform differentiated treatment. In the differentiation, the primary syndrome is emphasized, and the pathogen and pathological products are shown in the form of standard syndromes, which is clear, comprehensive and flexible. The cause and manifestations are both considered in the treatment. Prof. Qin thinks that in the early stage,the treatment should focus on clearing wind and heat, detoxifying and cooling blood. In the later stage, the focus of treatment should be nourishing Qi and Yin and tonifying spleen. Meanwhile, promoting blood circulation and removing and removing stasis should be performed throughout the whole process of treatment. These principles of treatment have achieved significant effect in the prevention and treatment of purpura at acute stage as well as its relapse.

Professor Wang Youpeng, the instructor of Zexie Wendan soup, has a series of prescriptions for Wendan Tang.     Investigation and analysis of serum 25-hydroxy vitamin D level in children in Ordos area

    Objective
    To investigate the nutritional status of vitamin D in children in Ordos area in order to provide basis for rational clinical supplementation of vitamin D.
    Methods
    Totally 1 572 healthy children aged 29 days to 18 years living in Ordos were included in this study, who took routine physical examination in our hospital from August 2016 to August 2018. Serum 25-hydroxy vitamin D(25-(OH)D) level was measured by electrochemiluminescence.
    Results
    Vitamin D deficiency existed in 779 cases(49.56%), vitamin D insufficiency in 576 cases(36.64%), and vitamin D sufficiency in 217 cases(13.80%) in the 1 572 subjects. Vitamin D deficiency and insufficiency were more obvious with the increase of age. Before 9 months of age, the vitamin D level increased with the increase of month age, and reached the peak between 9 and 12 months; and after 1 year of age the vitamin D level decreased continuously with the increase of age. There was no significant difference in 25-(OH)D level or vitamin D deficiency and insufficiency rate between boys and girls(P>0.05). The serum 25-(OH)D level was the highest in autumn and the lowest in winter(P<0.05), but there was no significant difference in other seasons(P>0.05).
    Conclusion
    The levels of 25-(OH)D in children aged 29 days to 18 years old in Ordos area are low, and vitamin D deficiency and insufficiency in children is common.

    Analysis of the factors influencing neuropsychological development of 26 230 infants and children aged 0 to 24 months

    Objective
    To learn about the level and the rules of neuropsychological development of infants and children aged 0 to 24 months.
    Methods
    The Neuropsychological Development Scale for Children Aged 0 to 6 Years was used to make development quotient(DQ) testing(gross motor, fine motor, adaptability, language and social behavior), in order to study the main risk factors that influence the neuropsychological development of infants and children.
    Results
    The total growth retardation rate was 2.15% in the infants and children aged 0 to 24 months. The multiple linear regression analysis showed that the following factors influenced the DQ, which included gender, premature delivery, body mass at birth, body length at birth, age(months) at physical examination and increase in body mass and body length(P<0.05). The interaction between body mass at birth and premature delivery, body mass at birth and body length at birth, age(month) at physical examination and increase in body mass, and age(month) at examination and increase in body length also had influence on DQ(P<0.05). The analysis of the influencing degree of independent variables revealed that age(month), increase in height, age (month) and height, gender and increase in body mass had greater influence on DQ.
    ConclusionThe early stage nutrition in infants and children has significant influence on the development quotient. The healthcare work for children must pay great attention to the nutritional intervention at the early development in infants and children. When analyzing the factors influencing DQ, the interactions between the child's growth and development and those factors must be taken into account.
