Chronic cough is a common symptom of respiratory diseases in children, protracted bacterial bronchitis (PBB) is the common cause of chronic cough in children worldwide.PBB refers to a persistent infection of the endobronchial membrane caused by bacteria, and is characterised by an antibiotic-responsive wet cough persisting for longer than 4 weeks.Bacterial culture of bronchoalveolar lavage fluid has shown Haemophilus influenza,Streptococcus pneumonia and Moraxella catarrhalis to be the main pathogens. The occurrence of PBB is related to bacterial biofilm formation in the airway, impaired mucociliary clearance, systemic immune disorder, and airway anomalies Currently, the mainstay of PBB treatment is a 2-week therapy with antibiotics, such as amoxicillin-clavulanate,to eradicate the infection and restore epithelial integrity.PBB is considered as forerunner of other chronic respiratory diseases like bronchiectasis and chronic suppurative pneumonia.Currently,PBB hasn′t been fully recognized by most pediatricians in China. This article reviews the protracted bacterial bronchitis in children concerning the etiology, pathogenesis, clinical manifestations, diagnostic criteria, and treatment.

HSP is one of the common autoimmune diseases in children. In recent years, the incidence of the disease has been increasing year by year, which may be related to the changes of children's constitution, diet, exercise and environment. There have been obvious pathogenesis and treatment progress in the prevention and treatment of HSP in TCM and western medicine. However, the treatment results and the control of the recurrence rate of HSP need further discussion. Studies have shown that the therapeutic effect and recurrence rate of allergic purpura are closely related to clinical care after illness. Therefore, this article, based on analyzing the etiology of the disease and summing up the pediatrician's experience in the routine treatment and clinical care of allergic purpura in the Affiliated Hospital of Chengdu University of Chinese Medicine, aims to reduce the recurrence of children with HSP and complications and shorten the course of disease, in order to provide clinical guidance for the diagnosis, treatment and recovery of children with HSP, and reduce the economic and psychological burden of children and their families.

    Abstract ( 239 )   PDF (569KB) ( 73 )   Save
The purpose of this paper is to introduce Professor Ding Ying's experience in the treatment of pediatric refractory nephrotic syndrome. Professor Ding Ying believes that deficiency in origin and excess in superficiality are the main pathogenesis of pediatric refractory nephrotic syndrome, and the accumulation of evil qi is the main factor that distinguishes refractory nephrotic syndrome from general nephrotic syndrome. Therefore, under the guidance of Zhang Zihe's theory of "removing pathogenic factors for the reinforcement of vital qi", we should pay attention to the elimination of evil qi on the basis of supporting healthy qi, in order to achieve the therapeutic effect of "treating disease without evil, and removing pathogenic factors for the reinforcement of vital qi",and the clinical effect is quite significant.

Preoperative interest inducement for reducing stress reaction of preschool and school-age children during peri-anesthesia period
    2020, 12 (2):  172-175.  doi: 10.3969/j.issn.1674-3865.2020.02.023
    Abstract ( 139 )   PDF (384KB) ( 52 )   Save
Objective To explore the effect of preoperative interest induction to reduce the stress reaction of pre-school and school-age children during peri-anesthesia period.
Methods A total of 106 children undergoing elective surgery in our hospital from February 1, 2017 to January 31, 2019 were included in the study, and they were randomly divided into observation group and control group, 53 cases each. The control group received routine nursing, and the observation group received combined preoperative interest induction. (1) Observe the degree of anesthesia induction coordination and anxiety self-rating scale(SAS) score; (2) observe heart rate and blood pressure in different periods;(3) observe anesthesia induction cooperation score(ICC) and modified Yale preoperative anxiety scoring(mYPAS) Score; (4) compare plasma cortisol, norepinephrine and epinephrine content in different periods.
Results The degree of anesthesia induction coordination was higher in the observation group than in the control group, while the SAS score was lower, the difference being statistical (P<0.05). Before induction, there was no statistical difference in heart rate or blood pressure between the two groups (P>0.05). After induction, the heart rate and blood pressure during operation were lower in the observation group than in the control group (P<0.05). Before induction, there was no statistical difference in ICC or mYPAS score between the two groups (P>0.05). After induction, the ICC and mYPAS score were lower in the observation than in the control group (P<0.05). At 10min before induction, there was no statistical difference in plasma cortisol, norepinephrine or epinephrine between the two groups (P>0.05). At 5min after induction and during the operation, the level of plasma cortisol, norepinephrine and epinephrine was lower in the observation than in the control group (P<0.05).
Conclusion Preoperative interest induction has a good effect on improving the psychological anxiety and anesthesia induction coordination of children. It can improve the psychological anxiety of children, improve the anesthesia induction coordination of children, and reduce the influence on heart rate, blood pressure, cortisol, epinephrine and norepinephrine. It decreases ICC and mYPAS scores, improves the stress status of children, and has high application value.

Objective To analyze the clinical features and etiology features of children with acute infectious diarrhea, and to provide basis for the clinical treatment and prevention strategy of the disease.
Methods The clinical data (demographic data, related living habits, disease data records, etc.) of 360 children with acute infectious diarrhea who were diagnosed and treated in pediatric department of our hospital from January 2017 to December 2018 were retrospectively collected. Fecal specimens were collected for pathogen detection. The main detection contents included 8 kinds of common bacteria and 6 kinds of viruses causing diarrhea in children. Summary and analysis were performed.
Results There was no significant difference in age distribution among children with different genders(P>0.05). The onset age was concentrated within 5 years(89.17%). The diarrhea frequency of 3 to 5 times/d was the most common, complicated with fever, nausea, vomiting and abdominal pain. The proportion of people living in rural areas was higher than that in urban areas. The proportion of people with habits such as washing hands before meals, washing hands after using bathroom, no drinking of raw water and regular cutting of nails was less than 50%. Among the 360 fecal specimens, the total detection rate for common pathogens was 49.17%(177/360), including detection rate for pathogenic bacteria (15.56%,56/360) and detection rate for viral nucleic acid(33.61%,121/360). There was no significant difference in detection rate for pathogens between 2017 and 2018(P>0.05). The pathogens that were detected out were mainly Salmonella and enterodiarrheal Escherichia coli, and the viruses were mainly rotavirus and norovirus. There was no difference in the detection rate of common bacteria or viruses between 2017 and 2018(P>0.05). The proportion of acute bacterial diarrhea in autumn and summer was significantly higher than that in spring and winter(P<0.05), but the proportion of pathogenic bacteria in different seasons was not statistically different(P>0.05); the proportion of acute viral diarrhea in autumn and winter was significantly higher than that in spring and summer(P<0.05), and the proportion of rotavirus in winter was significantly higher than that in other seasons, and norovirus in autumn was significantly higher than that in other seasons(P<0.05).
Conclusion From 2017 to 2018, the children with acute infectious diarrhea in the hospital are characterized by low age, poor self or family health, frequent onset in autumn and winter, bacterial and viral infections. The viruses are mainly rotavirus and norovirus. The pathogens are mainly Salmonella and entero-diarrheal Escherichia coli. Accordingly, the relevant departments should take corresponding measures to control incidence of acute infectious diarrhea in children.

Analysis of influencing factors and clinical characteristics of intrauterine infection in preterm infants
    2020, 12 (2):  180-182.  doi: 10.3969/j.issn.1674-3865.2020.02.025
    Abstract ( 193 )   PDF (323KB) ( 62 )   Save
Objective To study the influencing factors of intrauterine infection and analyze the clinical features of intrauterine infection in premature infants caused by placenta previa and premature rupture of membranes.
Methods A retrospective analysis was made in 262 cases of premature infants with intrauterine infection treated in neonatal wards from January 2017 to January 2019. The general data such as gender, gestational age, birth weight, and asphyxia were recorded, and the perinatal factors related to preterm birth were recorded. The clinical characteristics of infection caused by placenta previa were compared with those caused by premature rupture of membranes.
Results There are many factors leading to intrauterine infection in premature infants, and the top two most important perinatal factors were premature rupture of membranes and placenta previa (accounting for 51.2%), and the premature infants with intrauterine infection due to premature rupture of membranes mainly presented symptoms of non-specific infection, accounting for 52.4%. A small part of the infants had pneumonia related to intrauterine infection, and the ratio of infants needing ventilator support was very low (2.4%); however, pulmonary infection was the main infection in the premature infants with intrauterine infection due to placenta previa (62.0%), and the ratio of infants needing mechanical ventilation was significantly increased, nearly one-third of them(32.0%), the difference being statistically significant (P=0.033).
Conclusion Premature infants born with placenta previa need to be specially alert to intrauterine infectious pneumonia and need active respiratory support.
