    Study on Dingchuan decoction based on "Lung-Intestine Theory" and bile acid metabolism network in the regulation of intestinal flora in rats with RSV infection

    Objective
    By observing the changes of bile acid metabolites in serum of RSV-infected rats, we observed indirectly the changes of intestinal flora caused by RSV. Based on the "Lung-Intestine Theory", the regulation mechanism of Dingchuan decoction on bile acid metabolism and intestinal microflora after RSV infection was further elucidated.
    Methods
    Wistar rats were randomly divided into normal control group, RSV infection model group and Dingchuan decoction group. Except the normal control group, the other groups were intranasally inoculated with RSV Long strain virus solution. The general state and pathological changes of each group of rats were evaluated, and the related metabolites were detected by liquid chromatography-mass spectrometry(LC-MS).
    Results
    (1)Dingchuan decoction could regulate the general state of rats after RSV infection, reduce the lung index after RSV infection and improve pathological changes. (2)Compared with normal group, the levels of serum CA(citrate), TCA(taurocholic acid), TUDCA(tauroursodeoxycholic acid), CDCA(chenodeoxycholic acid), 7-ketodeoxycholic acid and phenylacetic acid in RSVinfected rats were significantly increased, while GCDCA(glycine chenodeoxycholic acid) was significantly decreased, and the difference was statistically significant(P<0.05);compared with the RSV model group, the serum levels of CA(citric acid), TCA(taurocholic acid), TUDCA(tauroursodeoxycholic acid), CDCA(chenodeoxycholic acid), GCDCA(glycine chenodeoxycholic acid), 7-ketodeoxycholic acid and phenylacetic acid of Dingchun decoction group were all downregulated, the difference being statistically significant.
    ConclusionDingchuan decoction can effectively regulate the imbalance of bile acid metabolism in serum of rats after RSV infection, and thus maintain the stability of intestinal microflora.

    Objective
    To evaluate the clinical efficacy and safety of Shang Shi Fang and Yi Qi Yang Yin Fang in preventing recurrent respiratory tract infection(RRTI) in children.
    Methods
    From July 2015 to July 2016, 90 children with RRTI were treated in the Outpatient Department of Xiamen Hospital of Traditional Chinese Medicine(gastrointestinal heating or Qi-Yin-deficiency syndrome),who  were randomly divided into observation group(n=45) and control group(n=45). According to the physical constitution of the children, the observation group was given timely treatment with Shang Shi Fang and Yi Qi Yang Yin Fang, with continuous intervention for 1 year and follow-up for 1 year, while the control group was not intervened. The types, frequency, course and curative effect of respiratory tract infection were observed before and after treatment.
    Results
    After treatment, both groups got improved in the frequency, course and types of disease, and the observation group was significantly superior to the control group, with statistically significant differences(P<0.05). After 12 months of follow-up, the observation group was significantly better than the control group in the reduction of frequency of upper respiratory tract infection and tracheitis(P<0.05). The incidence of pneumonia decreased in both groups after treatment, but the difference was not statistically significant(P>0.05). The effect of the observation group was better than that of the control group, and the difference was statistically significant(P<0.05). No adverse events or adverse reactions were reported in both groups.
    ConclusionIt can reduce the incidence of recurrent respiratory tract infection in children to use Shang Shi Fang and Yi Qi Yang Yin Fang at a proper time.

    Clinical characteristics and prognosis of invasive pneumonia Streptococcosis in children in intensive care unit

    Objective
    To analyze the clinical characteristics of invasive pneumonia Streptococcosis in children in intensive care unit(ICU) and observe its prognosis.
    Methods
    From February 2016 to February 2018, 60 children with invasive pneumococcal Streptococcosis were treated in intensive care unit of Hunan Children's Hospital, including 30 cases of bacterial meningitis, 20 cases of bacterial pleurisy, 8 cases of sepsis and 2 cases of necrotizing fasciitis. The clinical characteristics and prognosis of invasive pneumonia Streptococcosis were retrospectively analyzed. Blood, pleural effusion and cerebrospinal fluid samples were collected for bacterial isolation, culture and drug sensitivity test, and antibiotics were used for treatment.
    Results
    The incidence of Haemophilus syndrome was 8.33%(5 children), septic shock was found in 4 children, the incidence being 6.67%, and acute respiratory distress syndrome was found in 8 children, the incidence being 13.33%. The coverage rate of Streptococcus pneumoniae conjugate vaccine reached 97.6%. Only one strain of Streptococcus pneumoniae showed significant resistance to all antibiotics tested. Among them, antibiotic-insensitive Streptococcus pneumoniae accounted for 76.3%, and penicillin-resistant Streptococcus pneumoniae accounted for 54.8%. A total of 50 children survived, with a survival rate of 83.33%. Of the 8 children with sepsis, 4 were complicated with septic shock, and 3 died; 4 without septic shock survived. In addition, 2 of 5 children with hemophilia syndrome died and all the 5 children with respiratory distress syndrome died.
    ConclusionThe clinical characteristics of children with invasive pneumonia Streptococcosis in intensive care unit are remarkable. Targeted antibiotic therapy based on pathogenic bacteria can reduce the incidence of prognostic complications.

    Objective
    To analyze the clinical features of human rhinovirus(HRV) infection in infants with bronchiolitis, and to explore the relationship between allergic factors and HRV infection.
    Methods
    The clinical data of 174 infants with bronchiolitis were collected, who were admitted to Dalian Municipal Women and Children's Medical Center from January 2015 to December 2016, and they were divided into HRV group and respiratory syncytial virus(RSV) group according to etiological test. The clinical characteristics of the two groups were retrospectively compared.
    Results
    In HRV group of 29 children, the ratio of male to female was 3.1∶1, and the average age was (9.0±3.0) months. In RSV group of 108 children, the ratio of male to female was 1.5∶1, and the average age was (5.6±2.3) months. The clinical features of infants with bronchiolitis in HRV group(n=29) were similar to those of RSV group(n=108), but HRV group was older than RSV group(P=0.000) and had a higher temperature(P=0.003). The incidence of eczema(P=0.006) was higher, and family history of asthma and allergic rhinitis in parents or secondary relatives in HRV group were more common than RSV group; the eosinophil counts(P=0.006) were higher, but there was no significant difference in basophil counts.
    ConclusionThe clinical features of bronchiolitis with HRV and RSV infection are similar, but infants with HRV infection are older than those with RSV infection with higher temperature. Individual atopy, family history of asthma and allergic rhinitis increase the incidence of bronchiolitis with HRV infection.

    Objective
    To observe the antipyretic effect of the medicated foot bath therapy on 43 cases of infantile fever caused by exogenous pathogens.
    Methods
    We included 85 eligible patients from the outpatient department of pediatrics in our hospital from May 2017 to May 2018. The patients were randomly divided into treatment group and control group. When the temperature was between 37.5-39.0 ℃, the children in treatment group were given the medicated foot therapy, while those in control group were given warm water foot therapy. Record the temperature of the two groups 5 minutes before the treatment, and 0.5h,1h,2h after the treatment, as well as the onset time of antipyretic effect and the use of the drugs.
    Results
    After treatment, the temperature at 0.5h had no significant difference between two groups(P>0.05). The temperature in 1h and 2h in the treatment group was significantly lower than that in the control group(P<0.05). The onset time of antipyretic drugs in treatment group was (1.08±1.62)h, significantly shorter than that in the control group[(1.65±1.86)h](P<0.05). No significant difference was found between two groups in the frequency of using antipyretic drugs(P>0.05).
    ConclusionThe medicated foot bath therapy has a definite curative effect on infantile fever caused by exogenous pathogens. The therapy can be performed simply. It can improve the patients' compliance, reduce the sideeffect of the medicine and relieve the anxiety of the family. Thus, it is worthy of clinical application and popularization.

    Objective
    To observe the clinical effect of Xiyanping injection in the treatment of pediatric bacterial lower respiratory tract infection.
    Methods
    A total of 212 children treated for lower respiratory tract infection(positive sputum culture) in General Hospital of Ningxia Medical University from Jan. 2015 to Jan. 2016 were chosen as the research subjects, who were randomly divided into observation group and control group(106 cases in each group).Conventional treatment with antibiotics and atomization inhalation was given to both groups, and observation group was also given venous dripping of Xiyanping injection. The treatment lasted for 7 days.The improvement in the following aspects were observed in the two groups: clinical effect, fever-relief time, cough-relief time, time for pulmonary signs to disappear, hospitalization time, C-reactive protein, erythrocyte sedimentation rate(ESR),white blood cell count and lung function(tidal expiratory peak velocity).
    Results
    The clinical total effective rate in observation group was 94.3%(100/106),which was significantly higher than that in control group(76.4%,81/106)(P<0.05).After 5 days of treatment, the positive rate of sputum culture in observation group was 7.3%(15/106),significantly lower than that in control group(28.3%,30/106)(P<0.05).The fever-relief time and cough-relief time, the time for pulmonary signs to disappear and the length of hospital stay were all significantly shorter in observation group than in control group(P<0.05). The Creactive protein, erythrocyte sedimentation rate and WBC counts in observation group were lower than those in control group after treatment, while the tidal expiratory peak velocity was higher, the difference being statistically  significantly(P<0.05).
    ConclusionXiyanping injection has good effect in the treatment of bacterial lower respiratory tract infection in children.

    Study on the correlation between Mycoplasma pneumoniae infection and acute attack of bronchial asthma

    Objective
    To analyze the influence of Mycoplasma pneumoniae infection on the acute attack of bronchial asthma in children.
    Methods
    A total of 54 patients with acute exacerbation of bronchial asthma and Mycoplasma pneumoniae infection in our hospital were included as the experimental group. Another 54 children with respiratory tract infection were used as the control group. After the treatment, serum mycoplasma antibodies(MP-IgM), serum IgE, and peripheral eosinophilic granulocyte markers were detected in both groups.
    Results
    Compared with the control group, the positive rate of MPIgM in the experimental group was significantly increased(11.1%,6/54) with significant difference(P<0.05). The level of serum IgE and positive EOS counts were significantly higher in the MP-IgM positive group than in the negative group(P<0.05).
    Conclusion
    There is a close relationship between Mycoplasma pneumoniae infection and pediatric bronchial asthma. Mycoplasma pneumoniae infection can induce bronchial asthma and exacerbate the symptoms of children. Strengthening pediatric bronchial asthma examination and detecting related indicators can help to effectively make an early   diagnosis and has a wide range of clinical and is worth further promotion.

    Objective
    To explore the effect of Huaiqihuang granule on children with respiratory syncytial virus(RSV) bronchiolitis and its immunomodulatory effect.
    Methods
    Ninety cases of RSV bronchiolitis were treated in our hospital from July 2016 to June 2017, and they were randomly divided into the control group and the observation group,45 children in each group. The control group was treated with routine treatment,The observation group was treated with Huaiqihuang granule on the basis of the treatment for control group,6 months as 1 course. The curative effect and immunological indexes(IgA,IgG,IgM,IgE and IL-17 and IL-23) of the two groups were compared.
    Results
    The total effective rate of the observation group(93.33%,42/45) was significantly higher than that the control group(73.33%,33/45)(P<0.05).The time of clinical symptoms disappearing, antipyretic time and length of hospitalization in the observation group were significantly shorter than those of the control group(P<0.05).The level of IgA, IgG and IgM in the observation group was significantly higher than that of the control group, while the levels of IgE, IL-17 and IL-23 were significantly lower(P<0.05).
    ConclusionHuaiqihuang granule has significant effect on children with respiratory syncytial virus bronchiolitis. Immune regulation can be enhanced.

    The aim of the paper is to introduce the experience of Professor Ni Juxiu in the treatment of bronchiolitis in children by staging treatment. Based on the etiology and pathogenesis of this disease. Professor Ni thinks that phlegm retention is the key to the pathogenesis and divided the disease treatment into 3 stages: attack stage, relief stage, and stable stage. Prof. Ni combines internal medicine with external acupoint application in the treatment,paying attention to differentiating the syndromes,establishing treatment method based on syndromes and giving treatment accordingly,which achieves significant effect.

    Objective
    To investigate the influence of children's constitution on the pathogenesis of Henoch-Schonlein purpura.
    Methods
    In order to explore the influence of physical factors on pathogenesis of children with Henoch-Schonlein purpura, through the cross-sectional investigation, the constitution of 165 healthy children and 165 sick children were analyzed and summarized based on the children constitution scale of TCM published in 2015 by YangYin.
    Results
    The types of constitution were mostly balanced constitution(39.39%) and phlegmdampness constitution(26.06%) in the 165 healthy children, while in sick children, the main types were Yindeficiency(42.42%) and dampheat constitution(26.67%). The difference between the two groups was significant(P<0.05).
    ConclusionThere is significant difference in the distribution of TCM constitution types between healthy children and children with Henoch-Schonlein purpura. Children with Yin-deficiency and damp-heat are more likely to develop Henoch-Schonlein purpura than those with other types.

    Objective
    To detect whether the serum of the rats irrigated with rhubarb and leech contain the main components of these two drugs, and to provide theoretical basis for clinical use of drugs.
    Methods
    The effective components, rhein, emodin and hypoxanthine, in serum of rhubarb group and leech group were determined by HPLC.
    Results
    The rat serum of rhubarb group contained rhein and emodin;although the detection of rhein had certain interference, emodin could be detected. Hypoxanthine could be detected in the rat serum of leech group.
    ConclusionIt is confirmed that the main chemical constituents of rhubarb and leech are found in rat serum.

    By combing clinical practices and basic researches on anti-epilepsy treatment with Chinese medicine in recent years, and by reviewing the progress, achievements and directions of the treatment in many ways, such as treating method, prescription, channels and acupoints, it is concluded that the curative effect of Chinese medicine on epilepsy is significant with few adverse reactions. However, the effect mechanism still needs to be clarified further.

    Bronchiolitis obliterans is a clinical syndrome caused by chronic airflow obstruction due to small airway stenosis or obstruction. The disease is recurrent and has long been a burden on the family and society. Traditional Chinese medicine treatment for the disease can relieve the clinical symptoms and gains more and more attention. In recent years, clinical physicians have put forward new academic views on the TCM etiology and pathogenesis of the disease in combination with their clinical symptoms and etiology and pathogenesis. This paper will summarize them in order to provide a more comprehensive theoretical basis for clinical syndrome differentiation.

    Meta-analysis of pure traditional Chinese medicine in treating cough variant asthma in children

    Objective
    To systematically evaluate the therapeutic effect and safety of traditional Chinese medicine in the treatment of children with cough variant asthma.
    Methods
    We searched The Cochrane Library,PubMed,Embase,CNKI,SinoMed,VIP and WanFang Databases to collect randomized controlled trials(RCTs) on traditional Chinese medicine in the treatment of cough variant asthma in children from inception to November 2017. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies and made a Meta-analysis by using RevMan5.3 software.
    Results
    A total of 34 RCTs involving 2 230 patients were included into the study.The Meta-analysis results showed that compared with western medicine,the traditional Chinese medicine could significantly improve the clinical efficiency[RR=1.20,95%CI(1.12,1.29),P<0.000 01],decrease the recurrence rate[RR=0.40,95%CI(0.21,0.77),P=0.005],and reduce the incidence of adverse reactions[RR=0.07,95%CI(0.04,0.13),P<0.000 01],and there were statistically significant differences between the two groups.
    ConclusionBased on the current evidence, this study shows that the traditional Chinese medicine is superior to western medicine in overall clinical efficacy and as the safety of  treatment.

    Objective
    To observe the curative efficacy of gelatin oyster oral liquid in combination with compound preparations of zinc, iron and calcium in children with vitamin D deficiency rickets and its influences on bone metabolism.
    MethodsA total of 80 children with vitamin D deficiency rickets, who received treatment in our department from June 2016 to September 2017, were collected as observational objects and were randomly divided into the control group and the observation group,with 40 cases in each group. All children were given conventional treatment of vitamin D. Besides conventional treatments, the control group was given treatment of compound preparations of zinc, iron and calcium, while the observation group was given treatment of gelatin oyster oral liquid in combination with compound preparations of zinc, iron and calcium. After 12 weeks, the curative efficacy and levels of indices of bone metabolism were compared between the two groups.
    Results
    After 12 weeks, the total effective rate of the observation group(95.0%, 38/40) was statistically higher than that of the control group(77.5%, 31/40, P<0.05). Indices of bone metabolism showed that in comparison with the control group, the observation group had higher bone density of radius and ulna, lower level of bone alkaline phosphatase, and higher level of 25-(OH)D3 with all statistical differences(P<0.05).
    ConclusionBesides conventional treatment of vitamin D, gelatin oyster oral liquid in combination with compound preparations of zinc, iron and calcium has good curative effect in children with vitamin D deficiency rickets, and it can increase curative efficacy and improve indices of bone metabolism.

    Objective
    To observe the clinical effect of recombinant human growth hormone (rhGH) in the treatment of growth hormone deficiency(GHD) and idiopathic short stature (ISS) in children, and to explore the value and safety of IGF-1.
    Methods
    A total of 12 GHD children and 7 ISS children diagnosed in Yixing People Hospital from January 2016 to December 2017 were chosen as the research subjects and were treated with rhGH. They were divided into GHD group and ISS group according to the diagnosis. Subcutaneous injection of rhGH at 0.1-0.2 U/(kg·d) was given 30 min before going to bed for 6-7 times each week. The growth rate and IGF-1 were measured before treatment, and at 3 months and 6 months after treatment. The standard deviation integral was calculated, too.
    Results
    (1) The average height increase and growth rate in GHD group were significantly higher than those in ISS group, and the difference was statistically significant.(2) The standard deviation integral of IGF-1 in both groups increased significantly, with statistical significance, and the increase in GHD group was significantly higher than that in ISS group.
    ConclusionRhGH has obvious effect of promoting growth on children with short stature, especially on children with GHD, and IGF-1 is helpful for the evaluation of efficacy and safety.

    Clinical significance of PTX3 in children with Henoch-Schonlein purpura

    Objective
    To detect the expression of PTX3 in plasma and urine of children with HSP and HSPN, and to explore its clinical significance.
    Methods
    A total of 50 HSP children were selected, who were treated in the First Affiliated Hospital of Xinxiang Medical College from Mar. 2016 to Sep. 2017, and they were divided into two groups based on whether kidney was involved: HSP group(32 cases) and HSPN group(18 cases). Twenty healthy children receiving physical examination during the same period were included as the control group. The expression level of PTX3 in plasma and urine was determined in the 3 groups; the expression level of PTX3 of HSP group and HSPN group in the plasma and urine at acute stage and recovery stage was also determined.
    Results
    (1)The expression level of PTX3 in plasma and urine was significantly higher in HSP and HSPN group than in control group(P<0.05); the level was significantly higher in HSPN group than in HSP group(P<0.05).(2)The PTX3 expression level of HSP and HSPN group was significantly lower at recovery stage than at the acute stage(P<0.05).
    Conclusion

    Impact of family training and parents' cooperation on early cognitive ability of children with developmental delay

    Objective
    To investigate the effects of family training and parents' cooperation on early cognitive ability of children with developmental delay.
    Methods
    Totally 75 children with developmental delay were chosen as the research subjects, who were treated in Hunan Children's Hospital from Jan. 2017 to Jan. 2018, and they were divided into two groups based on whether they received family training or not: control group(n=13) and observation group(n=62). Based on the parents' cooperation, the observation group was again divided into 4 group: 15 cases in observation group 1(cooperation degree<30%), 18 cases in observation group 2(cooperation degree between 30% and 60%), 16 cases in group 3(61%-90%) and 13 cases in group 4(>90%). Both control and observation groups received rehabilitation treatment, including exercise therapy, cognition training, electrical stimulation of cerebellum, hydrotherapy and acupuncture. Family training was carried out by the parents every day in the observation groups. Before treatment and 3 month after treatment, Gesell development scale was used to make evaluation in the children.
    Results
    After treatment, the developmental quotient of each group was significantly improved(P<0.01). The improvement of the developmental quotient of observation group 3 and 4 was significantly higher than the control group(P<0.05).
    ConclusionFamily training by the parents plays an important role in the improvement of developmental quotient in children with developmental delay.

    Objective
    To explore the clinical effects and complications of Ya-iodine minimally invasive gel in the treatment of dental caries in children and make evaluation of the masticatory function.
    Methods
    Totally 128 children with dental caries admitted to Department of Stomatology, Hunan Children's Hospital, from January to December 2017 were selected as the research subjects, and they were randomly divided into observation group and control group with 64 cases in each group. The control group was treated with the conventional dental drill molar method, while the observation group was treated with the method of Ya-iodine minimally invasive gel. The pain degree, clinical effect, quality of life, complications and masticatory function of the two groups were compared.
    Results
    The pain degree in the observation group was lower than that in the control group, and the difference was statistically significant(P<0.05). After treatment, the total effective rate of the observation group was 95.31%(61/64), which was significantly higher than that of the control group(67.19%,43/64), the difference being statistically significant(P<0.05). After treatment and nursing, the function score of the quality of life (society, cognition, role, body) of the observation group was significantly better than that of the control group, with statistical significance(P<0.05). After 6 months of followup and re-examination, the incidence of complications in the observation group was 4.69%(3/64), significantly lower than that in the control group(18.75%,12/64), the difference being statistically significant(P<0.05). After treatment, the masticatory function of the two groups was improved, and the improvement of the observation group was significantly better than that of the control group, and the difference was statistically significant(P<0.05).
    ConclusionThe treatment for children with dental caries with Ya-iodine minimally invasive gel is effective, with little trauma, less pain and low complication rate, which can improve the quality of life and improve the masticatory function of children.

    Objective
    To observe the relationship between arterial blood lactate, early lactate clearance rate, amplitude-integrated electronencephalogram(aEEG) and neonatal asphyxia, and to investigate the clinical values of arterial blood lactate, early lactate clearance rate and aEEG in neonatal asphyxia.
    Methods
    A total of 60 asphyxia neonates admitted to Affiliated Hospital of Yangzhou University from July 2015 to July 2017 were divided into mild asphyxia group(22 cases) and severe asphyxia group(38 cases); they were divided into resuscitation 5-minute correction group(n=26), resuscitation 10-minute correction group(n=24) and resuscitation more-than-10-minute correction group(n=10);they were divided into low-clearance group(n=15) and high-clearance group(n=45);they were divided into survival group(n=51) and death group(n=9). The levels of arterial blood lactate were detected at the time of admission and 6 hours after treatment, and aEEG was evaluated. The statistical analysis on the relationship between arterial blood lactate, early lactate clearance rate, aEEG and neonatal asphyxia was done respectively.
    Results
    The levels of arterial blood lactate in severe asphyxia group were higher than mild asphyxia group(P<0.05). The pH value, alkali residual value and Apgar score in severe asphyxia group were significantly lower than those in mild asphyxia group, while aEEG abnormal rate was significantly higher(P<0.05).Compared with resuscitation 10-minute correction group and resuscitation 5minute correction group, the arterial blood lactate levels of resuscitation more-than-10-minute correction group were significantly higher on admission and 6h after treatment(P<0.05). The pH value, alkali residual value and Apgar score in resuscitation more-than-10-minute correction group were lower than those in resuscitation 10-minute and 5-minute correction group(P<0.05).Compared with survival group, the arterial blood lactate levels of death group were significantly higher on admission and 6h after treatment(P<0.01). The pH value, alkali residual value and Apgar score in the death group were significantly lower than those in survival group(P<0.01). There was no difference in lactate level on admission between low-lactate-clearance-rate group and the high group(P<0.05).The mortality in low-lactate-clearance-rate group was higher than high-lactate-clearance-rate group(P<0.05). The levels of arterial blood lactate in abnormal aEEG group were higher than normal aEEG group(P<0.05).
    Conclusion
    Arterial serum lactate level, early lactate clearance rate and aEEG are reliable evidences for neonatal asphyxia diagnosis, severity judgment and prognosis evaluation.

    Analysis of related factors for recurrence of ALL in child patients after treatment with SCCLG-2016 regimen

    Objective
    To analyze the related factors for recurrence acute lymphoblastic leukemia(ALL) in child patients after treatment with 2016 regimen of South China Children's Acute Lymphocytic Leukemia Treatment Cooperative Group(SCCLG-2016).
    Methods
    Totally 364 child patients with ALL who underwent SCCLG2016 regimen in Department of Hematology and Oncology of Shenzhen Children's Hospital were selected for the study. The treatment results of 364 patients who underwent SCCLG-2016 regimen were analyzed, and the recurrence and related pathological data were followed up. Logistics regression analysis was used to analyze the independent risk factors affecting recurrence after treatment.
    Results
    The recurrence rate was 18.68% (68/364) in 364 children patients with ALL, and there were significant differences in the age, white blood cell count, fusion gene positive rate, minimal residual disease and risk grading between recurrence group and no-recurrence group(P<0.05). The age ≥10 years old, white blood cell count ≥100×109/L and positive BCR/ABL gene and high risk grade were independent risk factors for recurrence in child patients with ALL after treatment.
    ConclusionAge ≥10 years old, white blood cell count ≥100×109/L, positive fusion gene and high risk grade are independent risk factors for recurrence in child patients with ALL after SCCLG-2016 regimen. Paying close attention to the above factors during treatment is helpful to identify the patients with higher probability of recurrence so as to give early intervention.

    Objective
    To investigate the efficacy and safety of Mucopolysaccharide polysulfate cream combined with Desonide cream in the treatment of infantile eczema.
    Methods
    From September 2017 to March 2018, 200 cases of infantile eczema were treated in the dermatology clinic of Shengjing Hospital Affiliated to China Medical University. They were randomly divided into a control group and an observation group, each with 100 cases. The observation group was treated with Desonide cream and Mucopolysaccharide polysulfate cream for external use; the control group was treated with topical Desonide cream alone. The rash and adverse reactions were recorded at the first week, the second week, and the fourth week of treatment.
    Results
    After two weeks of treatment, the total effective rate of the observation group was 88.0%(88/100), which was significantly higher than that of the control group, 77.0%(77/100), the difference being statistically significant(P<0.05). After four weeks of treatment, the recurrence rate of the observation group was 16.0%(16/100), which was significantly lower than that of the control group(48.0%,48/100), and the difference was statistically significant(P<0.05). No adverse reactions were found during the treatment in both groups of children.
    ConclusionMucopolysaccharide polysulfate cream combined with Desonide cream is effective in treating infantile eczema, with lower recurrence rate and fewer adverse reactions, and is relatively safe. It is worthy of promotion in clinical practice.

    Observation on the long-term predictive value of the assessment of general movement in infants

    Objective
    To observe the development of infants receiving the assessment of general movement for 2 years.
    Methods
    From March 2015 to March 2016, 203 high-risk children admitted to the Rehabilitation Outpatient Department of Shenyang Children's Hospital were evaluated by the assessment of general movement. The overall motor development of the children was observed, and a total of 54 children were able to be followed up to the age of 2 years.
    Results
    Among the 203 cases, the poor repertoire movement was 39.2%(49/125), the cramped-synchromised movement was 1.6%(2/125), and 24.4%(33/135) had abnormal fidgety movement. Among the 54 patients who were followed up, the poor repertoire movement was 44.0%(11/25), the cramped-synchromised movement was 4.0%(1/25), and the abnormal fidgety movement was 25.7%(9/35). The early stage results of the assessment of general movement showed that, a total of 10 cases had problems, which conformed to the original diagnosis that it would develop into cerebral palsy in the later follow-up. The patients whose movement were poor repertoire in the twist stage had certain language and social behavior problems.
    ConclusionFor infants with assessment of general movements as non-typical cerebral palsy, it is also necessary to regularly monitor the changes in their growth and development. At the same time, standardized management should be carried out for high-risk infants to find problems in time and provide targeted intervention or treatment.

    Objective
    To observe the curative effect of traditional Chinese medicine-Tiaowei decoction combined with Jinshi plaster on acupoint Shenque, in treating anorexia of children with spleen dysfunction.
    Methods
    Select 80 cases from Pediatrics Outpatient of Tongling Traditional Chinese Medicine Hospital between August 2016 to October 2017.The patients who met the inclusion criteria were divided into observation group(40 cases, treated with Tiaowei decoction combined with Jinshi plaster) and control group(40 cases, treated with Zinc gluconate oral solution) through the random method. Both groups were treated for four courses(4 weeks,7 days as a course).
    Results
    The syndrome scores were lower than before treatment in both groups, and the difference was statistically significant(P<0.05). After treatment, the syndrome scores of the observation group were significantly lower than those of the control group, and the difference was statistically significant(P<0.05). The total effective rate of the observation group was 95%(38/40), which was significantly higher than that of the control group(77.5%,31/40), and the difference was statistically significant(P<0.05).
    ConclusionTiaowei decoction combined with Jinshi plaster on acupoint Shenque is an effective method to treat childhood anorexia with syndrome of dysfunction of spleen, and it is worthy of clinical popularization.

    Objective
    To study the effect of Kangaroo mother care (KMC) on the growth and development of preterm infants with birth weight less than 2 000 g and clinical efficacy during hospitalization.
    Methods
    A total of 136 preterm infants with birth weight less than 2 000 g from January 2016 to June 2017 in Yueyang Second People's Hospital were randomly divided into KMC group(68 cases) and control group(68 cases). The KMC group was given kangaroo mother care as early as possible, while the control group was given routine care until discharge. The growth and development condition and NBNA scores were recorded. The clinical efficacy of the two groups was evaluated.
    Results
    The weight and head circumference at discharge, average weight gain in hospital in KMC group were significantly higher than those in the control group(P<0.05). There was no significant difference between the two groups in body length at discharge or the incidence of extrauterine growth retardation(P>0.05). The NBNA score in KMC group (35.37±0.96) was higher than that in control group (35.00±0.86). The incidence of apnea and bronchopulmonary dysplasia in KMC group was significantly lower than that in the control group(P<0.05). There was no significant difference in the incidence of patent ductus arteriosus, intracranial hemorrhage, feeding intolerance, gastrointestinal bleeding, necrotizing enterocolitis or nosocomial infection between the two groups (P>0.05). Compared with the control group, the KMC group had shorter duration of fasting and parenteral nutrition, earlier total enteral nutrition and faster rate of adding milk(P<0.05). Moreover, the duration of oxygen therapy, mechanical ventilation and hospitalization in the KMC group were significantly shorter than those in the control group(P<0.05).
    ConclusionKangaroo mother care can promote the growth and neurobehavioral development of preterm infants, reduce the incidence of complications related to premature birth, and promote the implementation of the nutritional support. KMC is beneficial to respiratory stability and early discharge and has good clinical effect on preterm infants.

    Objective
    To investigate the clinical effect of kindred-like nursing in perioperative period of pediatric hernia.
    Methods
    Totally 124 pediatric hernia patients admitted to Jinzhou Maternal and Infant Hospital from April 2017 to April 2018 during the perioperative period of pediatric hernia were selected as the study subjects, and were randomly divided into the control group and the observation group (62 cases each). Routine nursing was used in the control group and kindred-like nursing was used in the observation group. Clinical efficacy, abnormal heart rate, pain, nursing satisfaction rate, operation time, incision pain time, postoperative time to get out of bed, and intraoperative blood loss were compared between the two groups.
    Results
    The excellence rate, total effective rate and nursing satisfaction rate of the observation group were higher than those of the control group, while the abnormal heart rate and pain rate in the observation group were lower than those in the control group (P<0.05). Compared with the control group, the operation time, intraoperative bleeding, incision pain time and postoperative time to get out of bed in the observation group were all relatively better (P<0.05).
    ConclusionKindred-like nursing intervention in perioperative nursing of children with hernia can not only effectively improve the clinical efficacy, but also effectively lower the clinical indicators and improve the clinical efficacy of children with hernia.

    Objective
    To analyze the deaths of children under 5 years old in Shenyang City from 2013 to 2017, and explore intervention measures.
    Methods
    Choose all living births and dead children under 5 years old in the 13 districts and counties in Shenyang from October 1, 2012 to September 30, 2017 (according to the national mortality monitoring program for children under 5 years old; the annual monitoring and statistical period was: from October first of the previous year to September 30th of the present year) (with Shenyang household registration and nonShenyang household registration for more than one year; those who had been away from Shenyang for more than one year were not included in the monitoring), and their death monitoring data were statistically analyzed.
    Results
    Underfive mortality, infant mortality and neonatal mortality had shown a downward trend in Shenyang City from 2013 to 2017. The underfive mortality rate dropped from 5.42% in 2013 to 3.70% in 2017. The infant mortality rate dropped from 4.57% in 2013 to 3.18% in 2017, and the neonatal mortality rate dropped from 3.07% in 2013 to 2.15% in 2017. The death of children under 5 years old was mainly infant death, and the death of infants was mainly neonatal death. The change was not obvious in the 5 years.  From 2013 to 2017, the cause of death of major diseases in children under 5 years old changed significantly. The premature birth and low body weight was the first cause of death in 2013-2016. In 2015, other congenital anomalies were the top three causes of death and became the first cause of death in 2017.
    ConclusionIt is necessary to strengthen the health care during pregnancy and perinatal period, and take various measures to improve the ability of prenatal screening and diagnosis. At the same time, strengthening the quality of basic pregnancy and child health care services, reducing the incidence of preterm birth, and promoting neonatal asphyxia resuscitation techniques are also keys to reducing the mortality rate of children under 5 years old.

    The training of clinical thinking in interns needs to be carried out in stages and steps. The pediatric undergraduate internship teaching in our hospital is divided into four stages: (1) getting familiar with the rules and regulations of the department and the daily work and the review stage of the theoretical basic knowledge; (2) the improvement stage of clinical diagnosis thinking; (3) the improvement stage of clinical thinking--medical record discussion; (4) training students' observation ability and human communication ability. The training period for each stage is about 1 week. Through training, the clinical work ability of the interns in pediatrics has been improved comprehensively.
