    CHEN Haixia,SHAN Yuxia,CUI Zhenze,I Lei
    2021, 13 (1):  5.
    Abstract ( 172 )   PDF (651KB) ( 63 )   Save
In the global scope, the morbidity and mortality of bronchial asthma in children is increasing. The main causes include both the genetic factors and the environmental factors, especially the interaction between them. Epigenetics brings about the gene mutation through the environmental factors, which can be passed on to the next generation. It is an important factor of the higher incidence of bronchial asthma in children. Epigenetic modification not only affects the prevalence of asthma but may also affect the control level of asthma. Understanding the characteristics of interaction between genetic factors and environmental factors, especially the epigenetic characteristics, can provide new ideas for the early diagnosis and early intervention of childhood bronchial asthma.

    Abstract ( 246 )   PDF (468KB) ( 56 )   Save
Henoch-Schnlein purpura(HSP) is an allergic disease mainly involving capillaries and small arteries and veins. Professor Zhu Jinshan has unique clinical opinions on the treatment of allergic purpura and other pediatric difficult and miscellaneous diseases with pure Chinese medicine, which has achieved great effect. It is suggested that the etiology and pathogenesis of this disease are the exogenous six evils and heat underlying evils. At the early stage of differentiated treatment the first thing is to cool and disperse blood heat, and pay attention to the discharge of evils; at the middle stage, when clearing heat and cooling blood,we should take care of the Yin fluid, and at the later stage,Both attack and complement should be carried out,and we should not forget to clear the residual heat. Based on case analysis, this paper summarizes Professor Zhu's experience in the treatment of children with Hencch-Schonlein purpura.

    LI Xiao,ZHANG Lei,TANG Yu,XU Chunna
    2021, 13 (1):  33.
    Abstract ( 451 )   PDF (366KB) ( 51 )   Save
Objective:To investigate the T2 locus polymorphism of A Disintegrin and metalloproteinase 33 (ADAM33) gene in the analysis of the susceptibility and severity of bronchial asthma (BA) in children.
Methods:Forty-seven child patients with BA admitted to the hospital from January 2017 to March 2019 were selected as the study group, and 47 healthy children who had physical examination during the same period were selected as the control group. There were 15 cases of severe asthma, 17 moderate and 15 mild cases in the study group. All patients were subjected to restriction fragment length polymorphism analysis (polymerase chain reaction). The Hardy-Weinberg equilibrium and allele frequency of T2 locus were compared between the two groups. The correlation between BA severity and T2 locus polymorphism was observed.
Results:In regard to the gene frequency of the T2 locus, the subjects of the two groups had good group representation and all met the HardyWeinberg equilibrium. Among alleles of the T2 locus, the gene frequency of A of the study group was higher than that of the control group (P<0.05), and the high-risk factor for BA was the increase of the A allele. Among the T2 locus alleles, the AG genotype frequency of the children with severe or moderate asthma was higher than that of the children with mild asthma(P<0.05). The high-risk factor to distinguish the BA severity was the increase of AG allele.
Conclusion:There is a correlation between BA and T2 locus polymorphism of ADAM33 gene. Once the A gene frequency increases, the BA risk will increase accordingly. In addition, the severity of BA can be determined according to the increase of AG allele.

    Abstract ( 272 )   PDF (478KB) ( 70 )   Save
Objective:To explore the clinical effect of sodium valproate in the treatment of cerebral palsy combined with subclinical epileptiform discharge.
Methods:A total of 62 cases of cerebral palsy with subclinical epileptiform discharge were randomly divided into two groups,31 cases in each group. All the children received routine comprehensive rehabilitation treatment. On the basis of the above treatment, the observation group was given 20-40 mg/(kg·d) of sodium valproate oral solution, the blood concentration of which was controlled at 50-100 mg/L,and the course of treatment was 6 months. The control group did not use drugs. During the experimental period, each group was divided into two types according to whether there was clinical seizure or not. After six months of treatment, video EEG was examined to observe the control of epileptiform discharge. According to the results of EEG,the children with epileptiform discharge were divided into three categories: disappearance, improvement and no change. GMFM was used to observe the improvement of motor function.
Results:During the follow-up period, one case in observation group and two cases in control group were lost. Six cases in observation group were complicated by clinical epileptic seizure, obviously fewer than the control group(13 cases), the difference being statistically significant(P<0.05). After treatment, the re-examination of EEG showed that the effective rate in observation group was 83.33%(25/30) for controlling subclinical epileptiform discharge in children with cerebral palsy, significantly higher than that in control group(34.48%, 10/29)(P<0.05). Oral administration of sodium valproate in the observation group could improve the motion function more significantly, and there was statistical difference(P<0.05).
Conclusion:Long term oral administration of valproate can effectively control or reduce the occurrence of epileptiform discharge and reduce the probability of clinical seizure in children with cerebral palsy who have subclinical epileptiform discharge. It can improve the motor function of children and improve the effect of rehabilitation treatment.

    2021, 13 (1):  55.
    Abstract ( 328 )   PDF (513KB) ( 29 )   Save
Objective:To investigate and analyze the clinical distribution and drug resistance of carbapenem-resistant Klebsiella pneumoniae(CRKP) in children.
Methods:All kinds of submitted samples of children hospitalized in the hospital from December 2015 to December 2018 were collected. Isolation and culture of CRKP and bacterial identification were performed on the submitted samples. The K-B method and drug sensitivity card were used to test the sensitivity of the isolated CRKP strains. The distribution characteristics of CRKP infection in children were analyzed, and the resistance to commonly used antibacterial drugs was analyzed.
Results:A total of 156 strains of CRKP were isolated, including 149 strains detected by Carba NP test, with a positive rate of 95.51%. The main specimens were sputum and urine specimens, accounting for 51.28% and 16.67%, respectively. The main departments from which CRKP strain specimens came included the Neonatology Department(29.49%), the Respiratory Department (25.64%) and the ICU (14.10%). Among the other 16 antibiotics commonly used in the clinic, the resistance rates of CRKP to cephalosporins, penicillins and quinolone antibiotics were higher than 90%. The resistance to aminoglycoside, tetracyclines and sulfonamides was relatively low.
Conclusion:CRKP infection is widespread in children, especially in the Neonatology Department. In addition, the resistance of carbapenem-resistant Klebsiella pneumoniae is serious, and it has high resistance to most of the antibiotics. Therefore, it is necessary to choose antibiotics reasonably according to the results of drug sensitivity so as to effectively control infection.

    Abstract ( 458 )   PDF (654KB) ( 70 )   Save
Objective:To study the clinical significance of heart-ype fatty acid binding protein (h-ABP) and coronary artery Z value in early diagnosis of Kawasaki Disease in children.
Methods:From October 2018 to August 2020, 50 children with Kawasaki disease were confirmed and treated in Baotou Central Hospital of Inner Mongolia Autonomous Region, who were included as the observation group, and 55 children with respiratory tract infection were included as the control group. The h-ABP and coronary artery Z value of children with Kawasaki disease in the acute and recovery period were collected, and the h-ABP and coronary artery Z value of children with respiratory tract infection on admission was collected, and they were analyzed statistically.
Results:The h-ABP level in acute stage of observation group was higher than that of control group, there being statistical difference(P<0.05); there was no statistical difference in h-BP level in convalenscence stage between observation group and control group (P>0.05); the h-ABP level of observation group was significantly higher in acute stage than in convalescence stage(P<0.05). The Z value of left coronary artery(LCA) and left anterior descending branch(LAD) in acute stage of observation group was significantly higher than that of control group(P<0.05). There was no statistical difference between observation group and control group in the Z value of right coronary artery(RCA) in acute stage, the RCA- value in convalenscence stage, LCA- value or LAD- value(P>0.05). There was no relationship between h-ABP level and RCA, LCA or LAD(P>0.05). Linear regression analysis showed that RCA, LCA and LAD couldn't affect h-ABP level(P>0.05)
Conclusion:(1)There is an increase of h-ABP in children with Kawasaki disease, which may provide a basis for the early diagnosis of Kawasaki disease or atypical Kawasaki disease. (2)The level of h-P in convalescence stage in children with Kawasaki disease was significantly lower than that in acute stage, which can be used to evaluate whether there is myocardial damage in children with Kawasaki disease as early as possible. (3)The dilation of coronary artery can occur in the early stage of Kawasaki disease, and is most likely to cause LCA and LAD dilation, which can provide some clinical basis for the early diagnosis of Kawasaki disease or atypical Kawasaki disease. (4)If the level of h-BP can be added into the diagnosis standard of Kawasaki disease and combined with the Z value of coronary artery, Kawasaki disease may be found and confirmed earlier, especially atypical Kawasaki disease.
